Effect of head position and surgical dissection on sinus irrigant penetration in cadavers.

BACKGROUND: Effective treatment for recalcitrant rhinosinusitis requires unobstructed surgical marsupialization of sinus cavities and use of delivery systems that will topically penetrate the sinuses. AIMS: To determine the extent of sinus penetration achieved with nasal irrigation by varying the ostial size and head position. METHODS: Ten thawed fresh-frozen cadaver heads were dissected in a staged manner. After each stage of dissection, sinus squeeze-bottle irrigations were performed in three head positions, and endoscopes placed via external ports into the sinus cavities viewed the sinus …

Fresh frozen cadavers in surgical teaching: a gelatine arterial infusion technique.

BACKGROUND: Fresh frozen human cadavers have been used at the Clinical Training and Evaluation Centre, The University of Western Australia, Perth, WA for years and are an excellent model for surgical dissection thanks to their representative tissue quality. Differentiation between artery and vein can be difficult as both collapse post mortem. A historical technique was therefore refined to increase arterial rigidity using gelatine prior to freezing. METHODS: Two fresh human cadavers were selected after ethical approval. Gelatine was infused into the carotid artery in one, and into the common …

Sciatic nerve division: a cadaver study in the Indian population and review of the literature.

INTRODUCTION: The sciatic nerve is the largest nerve, with a long course in the inferior extremity. Its division into the tibial and common peroneal nerves can occur at any level from the sacral plexus to the inferior part of the popliteal space. These anatomical variations may contribute to clinical conditions such as piriformis syndrome, sciatica and coccygodynia. METHODS: This study was performed on cadavers in order to study the level of sciatic nerve division. The inferior extremities of 43 cadavers were classified into six groups depending on the level of sciatic nerve division in the g…

A prospective controlled trial of pulsed nasal nebulizer in maximally dissected cadavers.

BACKGROUND: Nasal douching is common practice in treatment of chronic sinusitis and after endoscopic sinus surgery. Current nasal delivery techniques show inconsistent sinus penetration. The aim of this study was to compare sinonasal penetration of nasal douching to an optimized nasal nebulizer in an operated cadaver model. METHODS: Fourteen preserved cadavers were used receiving complete sphenoethmoidectomies with a Draf III, wide maxillary antrostomy, or medial maxillectomy. Seven control cadavers received nasal douching with one standardized squeeze bilaterally of a 200-mL nasal irrigation…

Arthroscopic visualization of the posterior subtalar joint in the prone position: a cadaver study.

PURPOSE: Subtalar arthroscopy in the prone position is thought to allow broader access to the posterior aspect of the joint than in the standard supine or lateral position. This study is aimed to assess if using the posteromedial (PM) portal in the prone position can increase the working area available through the dorsal and ventral posterolateral (PL) portals commonly used in the standard techniques. TYPE OF STUDY: In vitro cadaver study. METHODS: Six below-knee cadaver specimens were subjected to experimental arthroscopy on the posterior subtalar joint in a simulated prone position. On bot…

Modification of the Zitelli bilobed flap: a comparison of flap dynamics in human cadavers.

OBJECTIVE: To investigate the effects on overall flap dynamics of altering the length of the primary flap in the Zitelli bilobed flap procedure, because the standard Zitelli design can result in distortion of the alar margin after repair of cutaneous nasal alar defects. METHODS: Identical 1.5-cm circular cutaneous defects were made on the nasal alae and cheeks of 6 fresh human cadavers. A rectangle was inked around each cheek defect in a standardized fashion. A standard-design Zitelli bilobed flap was used to close the nasal and cheek defects on 1 side of each specimen (n = 6). The contralate…

An anatomical protrusion exists on the posterior hypopharyngeal wall in some elderly cadavers.

Dysfunction of the cricopharyngeus is a common cause for dysphagia in the elderly and results in the appearance of the cricopharyngeal bar on a barium swallow radiological examination. However, manometrically normal relaxation of the cricopharyngeus has been observed in patients with a permanent cricopharyngeal bar, which implies that some structural changes may occur in the pharyngoesophageal wall. The aim of this study was to examine the macro-microscopic morphological features in the posterior hypopharyngeal wall of elderly human cadavers. Using dissection, histology, and E12 sheet plastina…

Frontal sinus anatomical variations with potential consequences for the orbit. Study on cadavers.

Anatomical variations of the frontal sinuses that may play a role in the correlation between frontal sinusitis and orbital complications are described. There were used 18 cadavers during routine educational cadaver dissections, one month after theirs withdrawal from the formol basin. The dissections of the cranial cavity and the orbits were performed so as to reveal the frontal sinuses. An unusual bilateral posterior extension of the frontal sinus mucosa was found in two of the cadavers. The projection of the mucosa was covering half of the orbital roof in a 60 year old male and one third of t…

Effect of pelvic tilt on acetabular retroversion: a study of pelves from cadavers.

Pelvic inclination is difficult to control on a standard radiograph of the pelvis and has a direct influence on the appearance of acetabular version. By defining the normal range of the distance between the symphysis and the sacrococcygeal joint on 86 standard anteroposterior radiographs of pelves a technique was developed to evaluate pelvic inclination. A statistically significant correlation between this distance and pelvic inclination was shown in four cadaver pelves. Acetabular retroversion signs (cross-over, posterior wall signs) were evaluated on normal pelves from cadavers (two females,…

Filum terminale fusion and dural sac termination: study in 27 cadavers.

The standard approach for sectioning of the filum terminale for a tethered spinal cord can be achieved via a limited S(1) exposure. This is performed with the commonly believed idea that the filum fuses with the dura at S(2). We dissected 27 cadavers to exclusively look at the level at which the filum pierces/fuses with the dura and also the level at which the dural sac ends. Most of the fila fused at S(2) with a range from L(5) to S(3). The majority of dural sacs ended at S(2) with a range from S(1) to S(3). However, 15% of the fila (4 of 27) fused above the S(1) level. In addition, 11% of th…

Endoscopic axillary lymph node dissection: an experimental study in human cadavers.

BACKGROUND: The role of axillary lymph node dissection in the staging of patients with breast carcinoma is currently under evaluation. As a result of recent advances in minimally invasive techniques, an endoscopic approach to axillary lymph node dissection may be an attractive alternative to lymphadenectomy performed via a standard "open" axillary incision. The purpose of the present study was to evaluate the technical feasibility and safety of such an approach in human cadavers. STUDY DESIGN: Twelve axillary dissections (right in seven, left in five) were performed in seven cadavers. A 2.5-c…

Anatomy of the sural nerve in a sample of South African human adult cadavers.

The sural nerve (SN) is a sensory nerve in the lower limb with notable variability in its origin, course, and branching patterns. This variability has important implications for diagnostic procedures, nerve grafting, and surgical planning, yet it remains underexplored in Southern African populations. This cadaveric study examined 90 lower limbs from 45 embalmed adult human cadavers (24 males, 21 females) at the University of Pretoria in South Africa. Each specimen was assessed for SN formation type, anatomical location, and morphometric data, including its contributing branches (medial sural c…
