BACKGROUND: Multiple myeloma is often complicated by pulmonary hypertension through a variety of mechanisms. These mechanisms include pulmonary hypertension (PH) due to concomitant cardiac amyloid, high output heart failure due to anemia or lytic bone lesions, chronic thromboembolic pulmonary hypertension (CTEPH), toxicity from medications to treat multiple myeloma, and congestive heart failure. This case series highlights the various mechanisms through which multiple myeloma patients develop pulmonary hypertension.
OBJECTIVES: To identify the etiologies of pulmonary hypertension and their ma

Hemodynamic dose-response effects of intravenous amrinone were studied in 22 male patients aged 38 to 62 years with left ventricular failure occurring within 18 hours of acute myocardial infarction. After hemodynamic confirmation of a raised left-sided cardiac filling pressure--pulmonary artery occluded pressure greater than 20 mm Hg--patients were randomized to either low-dose infusion of amrinone (200 micrograms/kg/hr for 30 minutes, 400 micrograms/kg/hr for 30 minutes and then 800 micrograms/kg/hr for 30 minutes) or high-dose infusion of the drug (800, 1,600 and 3,200 micrograms/kg/hr seque

BACKGROUND: Adrenomedullin is a recently discovered endogenous peptide with hypotensive and natriuretic actions in normal animals. Circulating and ventricular adrenomedullin are elevated in congestive heart failure, suggesting a possible role in the pathophysiology of this disease. No studies have previously examined the effects of adrenomedullin in heart failure.
METHODS AND RESULTS: Eight sheep with pacing-induced heart failure received human adrenomedullin(1-52) at 10 and 100 ng x kg(-1) x min(-1) I.V. for 90 minutes each. Compared with vehicle control data, adrenomedullin increased plasma

Cardiac tamponade is a life-threatening condition requiring emergent intervention, such as a pericardial window, to drain the pericardial effusion, subsequently improving cardiac output. A rare complication of this procedure is pericardial decompression syndrome which results in a paradoxical cardiovascular collapse. A 65-year-old male with bacterial endocarditis status posts mitral and aortic valve replacement presented for an emergent subxiphoid pericardial window to relieve cardiac tamponade. After draining 850mL of pericardial fluid, the patient suffered a cardiac arrest secondary to acute

Background and Objectives: Pulmonary sequestration (PS) is a rare congenital lung anomaly with anomalous systemic arterial supply. Surgical resection is the standard treatment, but some children have contraindications. Endovascular embolization (EE) is an established alternative; published pediatric experience is limited, particularly in neonates. We report a two-centre experience with extended follow-up and quantitative hemodynamic data. Methods: Six pediatric patients (five male; median age 6 months, range 11 days to 4 years and 8 months) underwent EE for PS at two centres in Gdańsk, Poland,

This study aimed to investigate the uniformity of adaptive changes in cardiorespiratory fitness and anaerobic power to high-intensity interval interventions (HIIT) designed using techniques specified for soccer players. Thirty well-trained athletes (age = 25 ± 3.1 years; body mass = 82.4 ± 3.4 kg; height = 183 ± 2.1 cm) were randomly assigned to two experimental groups engaging in interval intervention individualized using 30-15 Intermittent Fitness Test [HIITvIFT (two sets of 5-8 min intervals, comprising 15 s of running at 95% of VIFT followed by 15 s of passive recovery)], and small-sided g

INTRODUCTION: Postoperative hemodynamic instability and low cardiac output syndrome are common complications following cardiac surgery. Conventional volume management guided by central venous pressure and mean arterial pressure has well-documented limitations. Noninvasive cardiac output monitoring (NICOM) and critical care ultrasound (CCUS) provide complementary, real-time hemodynamic and volumetric assessments. This randomized controlled trial evaluated whether NICOM+CCUS-guided fluid management improves clinical outcomes in cardiac surgery patients with postoperative hemodynamic instability.

We examine 2 means by which the healthy respiratory system contributes to exercise limitation. These include the activation of respiratory and locomotor muscle afferent reflexes, which constrain blood flow and hasten fatigue in both sets of muscles, and the excessive increases in pulmonary vascular pressures at high cardiac outputs, which constrain O2 transport and precipitate maladaptive right ventricular remodeling in endurance-trained subjects.

PURPOSE OF REVIEW: To examine the evolving landscape of cardiac surgery, focusing on the increasing complexity of patients and the role of mechanical circulatory support (MCS) in managing perioperative low cardiac output syndrome (P-LCOS).
RECENT FINDINGS: P-LCOS is a significant predictor of mortality in cardiac surgery patients. Preoperative risk factors, such as cardiogenic shock and elevated lactate levels, can help identify those at higher risk. Proactive use of MCS, rather than reactive implementation after P-LCOS develops, may lead to improved outcomes by preventing severe organ hypope

We examined two groups of fetuses in which echocardiography had been performed and in which ventricular volume overload eventually led to fetal hydrops. The first group (18 fetuses) had atrioventricular valve regurgitation and almost all of the fetuses had structural heart disease. No fetus in this group survived the neonatal period; only two of the pregnancies in this first group were terminated. A second group of three fetuses had ventricular volume overload from sacrococcygeal teratomas at 21 to 24 weeks' gestation. These fetuses also had nonimmune hydrops (or it developed), but they did no

BACKGROUND: Protected cardiac surgery is a proactive strategy that applies temporary mechanical circulatory support to prevent perioperative low cardiac output syndrome in high-risk cardiac surgery. Rather than escalating vasoactive agents after hemodynamic decline, the approach emphasizes early identification of physiologic vulnerability and timely initiation of support.
METHODS: We conducted a structured literature review of peer-reviewed studies published from 2000 to 2024 on temporary mechanical circulatory support in adult cardiac surgery. There were 52 studies selected based on relevanc

High-frequency (HF) jet ventilation through transtracheal catheter has been used for the therapy of acute respiratory failure in 30 patients ith myocardial infarction complicated by pulmonary edema and cardiogenic shock. Gas exchange and central hemodynamics have been assessed, using a Swan-Ganz catheter fixed in the pulmonary artery. HF ventilation led to a decrease of hypoxemia, pulmonary blood shunting and to a moderate increase in the cardiac output.

Detailed hemodynamic, metabolic and blood volume studies were performed in a patient with hydatidiform mole who developed pulmonary edema associated with a high cardiac output. Several factors including hyperthyroidism, hypervolemia, and the molar state probably contributed to the left ventricular failure in this patient. results of these studies suggest that patients with hydatidiform mole and pulmonary edema need correction of the hypervolemia as well as removal of the molar tissue.

BACKGROUND: This study examined whether the presence of a sinus of Valsalva equivalent in the KONECT RESILIA aortic valved conduit (Edwards Lifesciences, Irvine, Calif) improves valve hemodynamics, kinematics, and performance.
METHODS: A 28-mm KONECT RESILIA aortic valved conduit was used to create an in vitro flow test model, and the same aortic valved conduit model without a sinus section was used as a control. Particle image velocimetry and hydrodynamic characterization experiments were conducted in the vicinity of the valves in a validated left-heart simulator at 3 cardiac output levels.
