We describe a patient whose complaints were related to pericardial effusion due to prostatic carcinoma. An 80-year-old man was admitted to our hospital because of chest discomfort and dyspnea. The chest radiograph revealed cardiomegaly and computed tomographic scan showed a large pericardial effusion. Pericardiocentesis revealed sanguinous exudates. Cytologic study suggested metastatic adenocarcinoma or malignant mesothelioma. He died suddenly because of ventricular tachycardia. A cranial mediastinal mass was observed radiographically in two 5-year-old adult male ferrets (Mustela putorius furo), both with histories of chronic episodic vomiting, dyspnea, and lethargy. Malignant lymphoma, a common neoplasm observed in ferrets, was considered the most likely diagnosis. Cardiomegaly and splenomegaly were also present. Histologically both mediastinal masses were composed of thymic epithelial cells and small lymphocytes, establishing a diagnosis of thymoma. Thymoma should now be included in the differential diagnosis of a cranial mediastinal mass in ferrets.

A prospective study of congestive cardiac failure in 221 elderly Nigerians seen at Ahmadu Bello University Hospital, Zaria, and Ife University Teaching Hospitals Complex, Ile-Ife from 1974 to 1979 is reported. Hypertension, iodopathic cardiomegaly and pulmonary heart disease were major causes of cardiac failure. No case of coronary atherosclerotic heart disease was seen. Ventricular conduction defect was the commonest E.C.G. abnormality recorded. In over half of the patients, there was coexisting disease, mild to moderate anaemia being the most common.

The rare occurrence of delayed hemorrhagic pericardial effusion requiring treatment is reported following significant blunt chest trauma (AIS greater than or equal to 3) in three patients from 12 to 15 days after injury. Five children under 10 years of age were operated on with a Björk-Shiley disc valve. The mitral valve was replaced in four patients and the tricuspid valve in one. No hospital death, late death or thromboembolic episode was observed during the 155 patient-months of the follow-up. Moreover, a marked reduction in cardiomegaly was demonstrated to parallel the clinical improvement and low risk seen in adults. We feel that our experience might justify a more aggressive approach in the management of valvular disease in children of the younger age group.

The authors review the anatomy of the pleural reflection of the right lung against the middle mediastinum in childhood. In 505 consecutive normal frontal radiographs of children, from birth to sixteen years, this pleural reflection (also known as the pleuro-esophageal line) was visible in 83%.
