Detecting cardiomyopathies in pregnancy and the postpartum period with an electrocardiogram-based deep learning model.

AIMS: Cardiovascular disease is a major threat to maternal health, with cardiomyopathy being among the most common acquired cardiovascular diseases during pregnancy and the postpartum period. The aim of our study was to evaluate the effectiveness of an electrocardiogram (ECG)-based deep learning model in identifying cardiomyopathy during pregnancy and the postpartum period. METHODS AND RESULTS: We used an ECG-based deep learning model to detect cardiomyopathy in a cohort of women who were pregnant or in the postpartum period seen at Mayo Clinic. Model performance was evaluated using the area

Complication rates following ventricular tachycardia ablation in ischaemic and non-ischaemic cardiomyopathies: a systematic review.

BACKGROUND: Catheter ablation of ventricular tachycardia (VT) is associated with potential major complications, including mortality. The risk of acute complications in patients with ischaemic cardiomyopathy (ICM) and non-ischaemic cardiomyopathy (NICM) has not been systematically evaluated. METHODS: PubMed was searched for studies of catheter ablation of VT published between September 2009 and September 2019. Pre-specified primary outcomes were (1) rate of major acute complications, including death, and (2) mortality rate. RESULTS: A total of 7395 references were evaluated for relevance. Fro

Intracardiac thrombus formation and thromboembolic events in children with cardiomyopathies: A single-center case series.

Intracardiac thrombosis and distant thromboembolic events are rare complications of patients diagnosed with any type of cardiomyopathies. The low prevalence of this entity makes it challenging and unfortunate for the patients and their families. This review aims to add to the current limited available data describing similar clinical entities.

Left ventricular 2D speckle tracking echocardiography for detection of systolic dysfunction in genetic, dilated cardiomyopathies.

AIMS: Genetic, dilated cardiomyopathy (DCM) can be caused by a large variety of mutations. Mutation carriers are often asymptomatic until DCM is well established, presenting with heart failure, arrhythmias, or sudden cardiac death. Preventive strategies can only be applied if DCM can be detected early. Echocardiographic, left ventricular (LV) global longitudinal strain (GLS) is a promising tool for early diagnosis, i.e. before a decrease in LV ejection fraction (EF) has occurred. We, therefore, investigated the role of LV GLS as an early disease marker in genetic DCM. METHODS AND RESULTS: Gen

Cardiomyopathies Due to Left Ventricular Noncompaction, Mitochondrial and Storage Diseases, and Inborn Errors of Metabolism.

The normal function of the human myocardium requires the proper generation and utilization of energy and relies on a series of complex metab … toskeletal proteins and is also associated with metabolic abnormalities, is discussed. In addition, cardiomyopathies resulting from mitochondrial dysfunction, metabolic abnormalities, storage diseases, and inborn errors of metabolism are described.

A Systematic Review of Phenotypic Features Associated With Cardiac Troponin I Mutations in Hereditary Cardiomyopathies.

BACKGROUND: Genetic investigations have established that mutations in proteins of the contractile unit of the myocardium, known as the sarco … tations in cardiac troponin I (cTnI; TNNI3), which is a recognized sarcomeric disease gene in all 3 cardiomyopathies. RESULTS: The results of this review did not identify specific genotype-phenotype relations in HCM or DCM, and cTnI appeared to be the most frequent disease gene in RCM. CONCLUSIONS: To further explore if there is a genotype-phenotype relation, lo …

Mitochondrial cardiomyopathies: how to identify candidate pathogenic mutations by mitochondrial DNA sequencing, MITOMASTER and phylogeny.

Pathogenic mitochondrial DNA (mtDNA) mutations leading to mitochondrial dysfunction can cause cardiomyopathy and heart failure. Owing to a high mutation rate, mtDNA defects may occur at any nucleotide in its 16 569 bp sequence. Complete mtDNA sequencing may detect pathogenic mutations, which can be difficult to interpret because of normal ethnic/geographic-associated haplogroup variation. Our goal is to show how to identify candidate mtDNA mutations by sorting out polymorphisms using readily available online tools. The purpose of this approach is to help investigators in prioritizing mtDNA var

Human cardiac mesoangioblasts isolated from hypertrophic cardiomyopathies are greatly reduced in proliferation and differentiation potency.

AIMS: Our objective was to test whether progenitor cell proliferation and differentiation potential may vary depending upon the disease of the donor. METHODS AND RESULTS: Human cardiac mesoangioblasts were isolated from cardiac muscle biopsies of patients undergoing open heart surgery for correction of mitral regurgitation following an acute myocardial infarction (MR-MI) or correction of mitral and aortic regurgitation with ensuing left ventricular hypertrophy (MAR-LVH). The cells express surface markers and cardiac genes similar to mouse cardiac mesoangioblasts; they have limited self-renewi

Helicobacter pylori-induced immunological responses in patients with duodenal ulcer and in patients with cardiomyopathies.

The interaction between the bacteria and the host is a key factor determining the clinical consequences of H. pylori infection. The immune system plays an important role in either promoting or preventing the disease. The mucosal production of TNF-alpha, IL-6, IL-8 and IL-10 and the CagA status were investigated in H. pylori-positive patients with duodenal ulcer (DU). The concentrations of these cytokines in gastric antral mucosal specimens from patients infected with H. pylori (n = 40) were determined by ELISA and compared with data on mucosal specimens from H. pylori-negative patients (n = 12

[Prevalence of detection of autoantibodies to beta-1 adrenoreceptors in patients with myocarditides and cardiomyopathies].

BACKGROUND: Autoantibodies to the second extracellular loop of a cardiac beta1-adrenoreceptors has been detected in some patients with cardiomyopathies and myocarditides. In animal experiments these autoantibodies have been shown to cause reduction of myocardial contractility. AIM: To estimate prevalence of detection of autoantibodies to beta1-adrenoreceptors in patients with myocarditides and cardiomyopathies. MATERIAL: Forty three patients including 18 with dilated cardiomyopathy (DCM), 14 with ischemic heart disease (IHD), 3 with myocarditides, 1 with alcoholic cardiomyopathy and 7 after …

[Current respective role of surgery and permanent endocavitary stimulation in the treatment of hypertrophic cardiomyopathies].

Surgery and cardiac pacing are the two main non-drug treatments for hypertrophic cardiomyopathy. Various surgical techniques have been proposed over the last 35 years: myotomy, myotomy-septal myomectomy, isolated mitral valve replacement, heart transplantation. Patients eligible for surgery are those with severe symptoms (NYHA stage III or IV) and refractory or no longer responding to drug treatment. The choice between the various techniques is based on morphological and haemodynamic criteria (significant subaortic gradient associated with increased septal thickness, severe and/or organic mitr

Rhythm disturbances in hypertrophic cardiomyopathies: relationship to symptoms and the effect of 'complete' beta blockade.

Potentially lethal arrhythmias (PLA) are common in hypertrophic cardiomyopathies and are implicated in sudden death. We have demonstrated that propranolol is effective in controlling symptoms, but have not previously evaluated its effect on PLA. Our protocol for long-term management has been applied to 50 patients followed for 2 to 14 years (mean 5.9 years). No patient was excluded because of disease severity or complications. The only reason for exclusion was non-compliance with the protocol. At entry, Holter monitoring revealed 16 PLA in 13 patients (sustained supraventricular tachycardia (S…

Coexistence of Hypertrophic Cardiomyopathy and Hereditary Transthyretin Amyloid Cardiomyopathy.

BACKGROUND: Left ventricular wall thickening may result from disease such as hypertrophic cardiomyopathy (HCM) or infiltrative cardiomyopathies such as transthyretin amyloidosis (ATTR). When features of both conditions coexist, distinguishing the dominant pathology can be challenging and may influence management. CASE SUMMARY: A 72-year-old man presented with progressive exertional dyspnea and syncope. He was found to have severe left ventricular outflow tract obstruction with systolic anterior motion of the mitral valve. He experienced marked symptomatic improvement after initiation of mavac…

Stress cardiomyopathy in a patient with hypertrophic cardiomyopathy and myocardial bridging.

Stress cardiomyopathy is an acquired cardiomyopathy of unknown aetiology. It usually occurs in women over the age of 70 who have experienced physical or emotional stress. It is most commonly characterised by a transient, left ventricular systolic dysfunction in the apical portion and hyperkinesia in the basal segments, without obstructive coronary artery disease. Its association with obstructive hypertrophic cardiomyopathy and myocardial bridging is rare. Herein, we present such a case.
