Reimmunization after blood or marrow stem cell transplantation.

Protective immunity to diseases preventable by routine vaccination is lost over time following allogeneic and autologous blood and marrow transplantation. Adoptive transfer of immunity from donors to recipients after allogeneic transplantation is not sufficient to prevent this decline. Systematic reimmunization is necessary at appropriate time intervals following transplantation to re-establish immunity. Response to vaccination depends upon the type of transplant, the source of cells, the immune status of the patient, and the vaccine being used. While inactivated or subunit vaccines are safe i

[Present state of cord-blood stem cell transplantation].

Allogeneic bone marrow transplantation (BMT) can cure some hematologic and solid malignancies, but its success depends on the prompt identification of a suitable donor and the avoidance of severe graft-versus-host (GVH) disease. Transplantation using hematopoietic stem cells from umbilical cord blood may overcome these problems. Over the past several years, cord blood transplantation (CBST) from siblings and unrelated donors has been performed in more than 300 patients. The probability of event-free survival was almost similar to that for BMT. This review described the recent status of CBST in

Hematopoietic stem cell transplantation in Australia in 1993.

The status of hemopoietic stem cell transplantation in Australia from January 1, 1993 to December 31, 1993 is reported. A total of 523 hematopoietic stem cell transplants were carried out throughout Australia during this 12 month period: 296 autologous transplants, 223 allogeneic transplants, and 4 syngeneic transplants. These were carried out by transplant programs in 20 hospitals, and the total number of transplants represented a 9.4% increase on the total number performed in 1992 (n = 478). Of the 296 autologous transplants, 133 utilized blood stem cells only, 125 marrow stem cells only, an…

Current status of peripheral blood progenitor cell transplantation.

Peripheral blood hematopoietic progenitor cells are primitive cells capable of both self-renewal and terminal differentiation. Cells can be collected in large numbers from the peripheral blood following their stimulation by cytokine administration or during the hematologic recovery phase following chemotherapy administration. Following their collection from the peripheral blood, these cells have been used both as a supplement and an alternative to bone marrow in providing hematologic reconstitution following high-dose chemotherapy and/or radiation. This article describes the biology of these c

Fetal liver cell transplantation in various murine models.

Allogeneic fetal liver cell transplantation has been shown to be able to reconstitute lymphopoietic systems of mice when these systems are defective or destroyed. Lethally irradiated mice or mice with inherited severe combined immunodeficiency disease (SCID) were grafted with 14 days gestation allogeneic fetal liver cells, then subjected to a follow-up for the immune tolerance to the donor and the normal or subnormal immune reconstitution allowing prevention of diabetes in NOD mice or cure of leukemia in AKR mice and of immunodeficiency in SCID mice. Briefly, when normal CBA mice were lethally…

Bone marrow transplantation for severe sickle cell anaemia.

Five children with sickle cell anaemia underwent bone marrow transplantation (BMT) for severe clinical disease. The conditioning regimen for BMT was in busulfan plus cyclophosphamide. The allograft contained more than 5 x 10(8) nucleated cells per kg recipient. Prophylaxis of GVHD consisted of methotrexate and cyclosporin A. Therapy was well tolerated. Duration of neutropenia (less than 0.5 x 10(9)/l) was short (14-25 d). Platelet recovery (greater than 50 x 10(9)/l) occurred between day 12 and 45. The patients have been followed up for 8-28 months. No major infections occurred and long-term B

Hematopoietic Cell Transplantation for Blastic Plasmacytoid Dendritic Cell Neoplasm: Clinical Practice Guidelines From the American Society for Transplantation and Cellular Therapy.

Blastic plasmacytoid dendritic cell neoplasm (BPDCN) is an aggressive hematologic malignancy with complex clinical manifestations, frequently involving multiple organs. Treatment of BPDCN has evolved over the years from conventional chemotherapy to novel targeted therapies including anti-CD123 antibody drug conjugates, namely tagraxofusp and pivekimab sunirine, and the BCL-2 inhibitor, namely venetoclax. However, because anticipated long term disease control is not generally possible, it is standard practice to offer allogeneic hematopoietic cell transplantation (HCT) as a consolidation strate…

Graft-versus-host disease after autologous stem cell transplantation in a recipient who underwent allogeneic stem cell transplantation 20 years earlier.

A literature review does not provide information about the safety of autologous hematopoietic stem cell transplantation (HSCT) in a recipient who has previously received allogeneic HSCT. We treated a 69-year-old woman with diffuse large B cell lymphoma. The patient received autologous stem cell transplantation (ASCT) in the second complete remission of malignant lymphoma. The patient had undergone allogeneic hematopoietic SCT (allo-HSCT) for chronic myeloid leukemia 20 years ago. Chronic myeloid leukemia had been in complete remission for the previous 20 years. Thus, the patient received autol…

Sustained molecular remission by non-myeloablative stem cell transplantation after autologous hematopoietic stem cell transplantation in a patient with multiple myeloma.

Multiple myeloma (MM) is refractory to conventional chemotherapy. To achieve a sustained complete remission, we performed planned non-myeloablative allogeneic stem cell transplantation (NST) after autologous hematopoietic stem cell transplantation (HSCT) in a patient with stage III MM. Autologous HSCT was performed using high-dose melphalan after conventional chemotherapy, followed by NST from an HLA-identical sibling using low-dose total body irradiation (200 cGy) for conditioning. Cyclosporine and mycophenolate mofetil were used for graft-vs-host disease (GVHD) prophylaxis. Acute GVHD was tr…

Fractionated stem cell infusions for patients with plasma cell myeloma undergoing autologous hematopoietic cell transplantation.

We conducted a phase II trial investigating the impact of fractionated hematopoietic cell infusions on engraftment kinetics and symptom burden in patients with plasma cell myeloma (PCM) undergoing autologous hematopoietic cell transplant (AHCT). We hypothesized that multiple hematopoietic cell infusions would reduce duration of neutropenia and enhance immune recovery resulting in a better tolerated procedure. Twenty-six patients received high-dose melphalan followed by multiple cell infusions (Days 0, +2, +4, +6) and were compared to PCM patients (N = 77) who received high-dose melphalan and a

Matched sibling donor stem cell transplantation for sickle cell disease: Results from the Spanish group for bone marrow transplantation in children.

OBJECTIVES: The prevalence of sickle cell disease (SCD) in Spain is markedly inferior compared with other European and Mediterranean countries. However, the diagnosis of new patients with SCD is expected to increase. In this multicenter retrospective study, we analyze the hematopoietic stem cell transplantation (HSCT) results obtained in Spain. METHODS: Forty-five patients who underwent a matched sibling donor (MSD) HSCT between 1999 and 2018 were included. Primary endpoint was event-free survival (EFS), and secondary endpoints included acute and chronic graft-versus-host disease (GvHD) and o…

Current status and development of hematopoietic stem cell transplantation in China: a report from Chinese Hematopoietic Stem Cell Transplantation Register Group.

Allogeneic hematopoietic stem cell transplantation (allo-HSCT) has three decades history in China. During these periods, the number of HSCT has been increasing, donor and stem cell sources were expanded, indication of diseases and patients for HSCT extended. Forty-two HSCT units offered their data 1–6 times from July 2007 to June 2010. The annual increase rates were 8.8% to 10.8%. Matched sibling donor is 41%, mismatched related/haploidentical donor is 24%, unrelated volunteer donor is 16%, and umbilical cord blood is 2%. The indications of major disease entities are acute myeloid leukemia (AM…

Haploidentical Hematopoietic Cell Transplantation Combined with Umbilical Cord Blood Versus Haploidentical Hematopoietic Cell Transplantation in T-Cell Acute Lymphoblastic Leukemia Patients: A Multicenter Real-World Study.

Allogeneic hematopoietic cell transplantation (allo-HCT) is a promising treatment option for T-cell acute lymphoblastic leukemia (T-ALL). Co-infusion of umbilical cord blood (UCB) in haploidentical cell transplantation (haplo HCT) (haplo-cord HCT) for acute myeloid leukemia and B-cell acute lymphoblastic leukemia is a promising treatment in previous studies, but the clinical outcomes of T-ALL patients with haplo HCT whether combined with UCB remain unclear. Between 2012 and 2022, a total of 281 T-ALL patients including 190 with UCB (haplo-cord HCT group) and 91 without UCB (haplo HCT group) in…

Passenger Lymphocyte Syndrome in Pediatric Lung Transplantation: A Recipient With Previous Hematopoietic Stem Cell Transplantation.

BACKGROUND: Passenger lymphocyte syndrome (PLS) is a rare complication following solid organ transplantation (SOT) that has not been reported in pediatric lung transplantation. It consists of hemolysis by antibodies from donor B-lymphocytes against recipient red blood cells (RBCs). METHODS: We report the case of a 12-year-old girl who received bilateral lung transplantation for bronchiolitis obliterans secondary to graft-versus-host disease. She had undergone allogeneic matched unrelated hematopoietic stem cell transplantation (HSCT) 6 years prior for myelodysplastic syndrome (MDS). Details a…

Immunosuppression-Free Status at 1 Year after Haploidentical Hematopoietic Cell Transplantation with Post-Transplantation Cyclophosphamide.

The development of chronic graft-versus-host disease (GVHD) in 1-year survivors after matched related or unrelated hematopoietic cell transplantation was shown to be associated with higher nonrelapse mortality (NRM) and worse overall survival (OS). The impact of chronic GVHD requiring immunosuppression (IS) for recipients of haploidentical transplantation (HIDT) with post-transplantation cyclophosphamide (PTCy) who have survived to 1 year post-transplantation has not been studied previously and was investigated for this analysis. A total of 322 adult patients who underwent HIDT at our center w…

A Case of T/NK-Cell Post-Transplantation Lymphoproliferative Disease 7 Years after Heart Transplantation.

Post-transplant lymphoproliferative diseases (PTLD) are potentially fatal complications after cardiac transplantation. Most cases are Epstein-Barr virus (EBV)-related B-cell tumors, and reduction of immunosuppression treatment as well as the use of rituximab in combination with other chemotherapy are effective. However, patients with T/NK-cell PTLD post-cardiac transplantation are rarely reported. We had a patient with a fever that lasted for three weeks, with lung infiltrations and hepatosplenomegaly, who had EBV-associated hemophagocytosis 7 years after heart transplantation and was eventual

Lung transplantation for bronchiolitis obliterans after hematopoietic stem cell transplantation: a retrospective single-center study.

BACKGROUND: Bronchiolitis obliterans (BO) is one of the most common late non-infectious pulmonary complications after hematopoietic stem cell transplantation (HSCT). Lung transplantation (LT) is the only cure for patients with end-stage BO, but the overall efficacy is rarely reported. Our study aims to conclude and elucidate the clinical experience of our single center and provide a reference for the current selection of treatment. METHODS: We retrospectively analyzed the medical records of six patients with post-HSCT BO who received LT in our center from 2015 to 2019. The collected informati…
