Asymmetric Petro-occipital Venous Plexus Thickening: A Valuable Diagnostic Sign of Reduced Flow in the ICA.

Thickening of the venous plexus surrounding the petrous ICA, known as Rektorzik venous plexus (RVP), is a marker of inward ICA remodeling from a substantial reduction in blood flow, but clinical application of its detection remains unexplored. We reviewed contrast-enhanced vessel wall MRI imaging (VWMRI) and CTA examinations of the head and neck acquired at our institutions to identify cases with asymmetric RVP thickening that influenced the diagnosis or medical management of neurovascular diseases. Five representative patients were included. Two patients with intracranial MRA or CTA interpret

Resolution of long standing tinnitus following radiofrequency ablation of C2-C3 medial branches--a case report.

Tinnitus is described as an auditory phantom perception analogous to central neuropathic pain. Despite the high prevalence of this debilitating symptom, no intervention is recognized that reliably eliminates tinnitus symptoms; a cause has yet to be determined. A 65-year-old healthy man presented with a 3 year history of left-sided tinnitus. Full workup performed by the primary care physician including blood tests for electrolyte imbalance, consultations by 2 independent otholaryngologists, and imaging did not reveal abnormalities to provide etiology of the tinnitus. No other complaints were no

Fibrotendinous band causing neurogenic Thoracic Outlet Syndrome in adolescent with bilateral cervical ribs.

Thoracic Outlet Syndrome (TOS) describes a variety of symptoms caused by the compression of the neurovascular structures in the cervicoaxillary region as they leave the thorax toward the upper limbs. Causes of TOS are vascular and neurogenic, with neurogenic symptoms being the most common presentation (95% of patients). Symptoms related to neurogenic TOS include intrinsic hand muscle and thenar region atrophy and weakness, as well as medial hand and forearm numbness, which are all suggestive of lower trunk involvement. This condition is very rare in the pediatric population. We are reporting a

Surgical approach to the spinal accessory nerve for brachial plexus reconstruction.

An easy, safe, and less invasive surgical approach to the spinal accessory nerve for brachial plexus reconstruction is described. The technique avoids a longitudinal unsightly scar over the neck and preserves the branches innervating the upper part of the trapezius.

Ansa cervicalis nerve: review of the topographic anatomy and morphology.

In recent years, there has been a proliferation of techniques utilizing the ansa cervicalis nerve to reinnervate the paralyzed larynx. The anatomic course and morphology of the ansa cervicalis are complicated by the variable course and location along the great vessels of the neck, as well as the significant differences observed in the arrangement of its contributing roots and regional branching patterns. Herein, we review the surgical anatomic course of ansa cervicalis and its innervation of the muscles of the neck, and develop specific recommendations with respect to the use of this nerve in

[Traction injuries of the brachial plexus: radiologic diagnosis using myelo-CT and MR].

The exact radiographic localisation of supraganglionic lesions of the brachial plexus provides important information for the prognosis and clinical management of these injuries. The authors report on the results of enhanced CT scanning and MRI of the cervical spine in five patients with surgically proven root avulsions caused by traction injuries. All lesions were correctly diagnosed by enhanced CT scanning. MRI, by comparison, identified only about 70% of the neural lesions.

Paravertebral brachial plexus block in sheep: a cadaveric and in vivo study.

The Objective of the present study was to evaluate paravertebral brachial plexus block in sheep. A group of 13 healthy sheep with 20.10 ± 2.20 kg weight and five months of age were used. In phase I, in five sheep, an insulated needle attached to a nerve stimulator was directed to the location of C6, C7, C8 and T1 nerves and a 1.50 mL of a solution containing 1:1 methylene blue 1.00% and lidocaine 1.00% was injected at each site. Then, the cervical and thoracic areas were dissected and assessed in the cadavers. In phase II, cervical paravertebral block with 2.00% lidocaine and subsequent assess

Traumatic Cervical Nerve Root Avulsion with Pseudomeningocele Formation.

Cervical nerve root avulsion is a well-documented result of motor vehicle collision (MVC), especially when occurring at high velocities. These avulsions are commonly traction injuries of nerve roots that may be accompanied by a tear in the meninges through the vertebral foramina with associated collections of cerebrospinal fluid (CSF), thereby resulting in a pseudomeningocele. We present a case of a 19-year-old male who experienced an MVC and was brought to the emergency department (ED) with right arm paralysis and other injuries. A neurological examination demonstrated intact sensation but 0/

Nerve sparing radical hysterectomy in early stage cervical cancer. Latest developments and review of the literature.

BACKGROUND: Cervical cancer is the second most common malignancy of the female genital tract worldwide. Radical hysterectomy with pelvic lymphadenectomy exemplifies the treatment of choice for early stage disease, whereas even if it is performed by gynaecologist-oncologist, still has the drawback of significant postoperative morbidity, especially for urinary bladder function. Nerve-sparing radical hysterectomy (NSRH) is a technique in which the neural part of the cardinal ligament which encloses the inferior hypogastric plexus, as well as the bladder branch (distal part of the plexus), remains

[CT study of the cervical spine with intravenous administration of the contrast medium].

Computed tomography (CT) without contrast medium is largely applied to the study of intervertebral disk pathology in the lumbar spine, but has not been widely accepted in the cervical spine, due to technical and anatomical limitations. For these reasons many neuroradiologists still prefer myelography or myelo-CT. CT may yield better results if combined with iv contrast medium injection, which allows a better visualization of disk herniation. This technique is aimed at enhancing the density of the venous plexus which is located close to the intervertebral disk, the vertebral bodies and the neur

Contralateral C7 transfer to lower trunk via a subcutaneous tunnel across the anterior surface of the chest and neck for total brachial plexus root avulsion: a cadaveric study.

BACKGROUND: Restoration of hand function after total brachial plexus root avulsion (tBPRA) is a difficult problem in surgical management. A new modified approach in repairing tBPRA is to use a subcutaneous tunnel across the anterior surface of the chest and neck, and then transfer the contralateral C7 root (cC7) to the lower trunk. However, the anatomical details of this method have not yet been fully described and assessed. The objective of this study was to quantitatively describe the nerve transfer using a cadaveric surgical simulation. MATERIALS AND METHODS: Brachial plexuses were dissect

Laparoscopic pelvic anatomy of nerve-sparing radical hysterectomy.

Many reports regarding nerve-sparing radical hysterectomy have been published. However, most reports have been based on systematic descriptions via laparotomy or cadaver dissection. The aim of this work was to describe the pelvic anatomy of nerve-sparing radical hysterectomy via laparoscopy, with specific focus on the inferior hypogastric plexus. This study is based on 125 patients with FIGO stage IB cervical cancer who had undergone laparoscopic nerve-sparing radical hysterectomies since 1999. The inferior hypogastric plexus was demonstrated via laparoscopy and was comprised of afferent fiber

Detection of nerve rootlet avulsion on CT myelography in patients with birth palsy and brachial plexus injury after trauma.

OBJECTIVE: Recent advances in neurosurgical treatment of traumatic and birth-related brachial plexus injuries require differentiation of preganglionic nerve rootlet avulsion from postganglionic lesions. The purpose of this study was to evaluate the efficacy of thin-section high-resolution CT myelography for revealing cervicothoracic nerve rootlet avulsion in patients with brachial plexus injuries before surgery. MATERIALS AND METHODS: We evaluated eight patients with posttraumatic or birth-related brachial plexus injury on cervical plain film myelography and high-resolution CT myelography bef

Foraminal Origin of the Dorsal Scapular Nerve: An Anatomical Study.

BACKGROUND: Although distal dorsal scapular nerve (DSN) anatomy has been well characterized, a paucity of literature exists detailing its proximal origin. To our knowledge, this is the first study examining DSN origin and its anatomy relative to the C5 nerve root, which may help localize pathology and provide insight into timing of DSN or C5 nerve root clinical and electrophysiological recovery. METHODS: Eighteen cadaveric dissections were performed using a posterior-midline approach. Calipers were used for DSN branching and course characterization with statistical analysis completed for the

The breathing hand: obstetric brachial plexopathy reinnervation from thoracic roots?

It has been found that in cases of obstetric brachial plexopathy, injured phrenic nerve or C3/4/5 roots may sprout into the adjacent injured upper and middle trunks of the brachial plexus. This aberrant regeneration produces co-contraction of the diaphragm and proximal upper limb muscles. This phenomenon, referred to as respiratory synkinesis or "the breathing arm", may not be limited to the upper cervical roots. We present two cases, identified through electromyographic investigations, of respiratory synkinesis selectively affecting intrinsic hand muscles, and propose that upper thoracic root
