Integration of vitamin A supplementation with the Expanded Programme on Immunization: lack of impact on morbidity or infant growth.

UNLABELLED: Vitamin A deficiency is associated with increased morbidity and mortality from diarrheal disease, measles, and malaria. It has been proposed that vitamin A supplementation could be linked with childhood immunization programs to improve child health. We conducted a randomized, double-blind, placebo-controlled clinical trial to evaluate the impact of linking vitamin A supplementation with the Expanded Programme on Immunization on morbidity and child growth. In West Java, Indonesia, 467 six-week-old infants were randomized to receive 7.5 mg retinol equivalent (RE), 15 mg RE, or placeb

Transjugular intrahepatic portosystemic shunt in patients with end-stage liver disease: results in 85 patients.

Transjugular intrahepatic portosystemic shunt (TIPS) is becoming an accepted procedure as a bridge to orthotopic liver transplantation (OLT) in patients with end-stage liver disease (ESLD) and bleeding from portal hypertension. It allows the immediate control of acute bleeding and decreases the risk of recurrent acute bleeding while the patient is awaiting OLT. We review in this report, our experience with 85 patients who underwent a TIPS procedure for gastrointestinal variceal bleeding from September 1991 until April 1994. All patients had liver cirrhosis and all had previous sclerotherapy be

Empirical vulnerability strata in under-five mortality in Cameroon: Evidence from DHS data.

Despite substantial declines in under-five mortality in sub-Saharan Africa, child survival remains marked by persistent and socially structured inequalities. Variable-centered survival models document average effects of risk factors but provide limited insight into how multiple disadvantages combine to generate heterogeneous mortality risks. This study aims to identify and characterize empirical configurations of vulnerability in under-five mortality in Cameroon. Using pooled data from the 2011 and 2018 Cameroon Demographic and Health Surveys, we analyze time to death before age 59 months. A r

The type-reproduction number of sexually transmitted infections through heterosexual and vertical transmission.

Multiple sexually transmitted infections (STIs) have threatened human health for centuries. Most STIs spread not only through sexual (horizontal) transmission but also through mother-to-child (vertical) transmission. In a previous work (Ito et al. 2019), we studied a simple model including heterosexual and mother-to-child transmission and proposed a formulation of the basic reproduction number over generations. In the present study, we improved the model to take into account some factors neglected in the previous work: adult mortality from infection, infant mortality caused by mother-to-child

Preceding birth intervals and child survival: searching for pathways of influence.

The importance of the length of preceding birth intervals for the survival chances of young children has been established, but the debate concerning the causal biomedical or behavioral mechanisms continues. This article uses data from 17 Demographic and Health Surveys to investigate the effect of birth intervals on child mortality: Anthropometry of children, recent morbidity of children, and use of health services are examined in addition to child survival data for children born in the five years before the survey. Various methodological approaches are used to investigate the relative importan…

[The effect of propranolol on portal hypertension].

The review of literature on propranolol (Inderal) pharmacological action on the portal vein pressure is reported in this article. The patients with liver cirrhosis and esophagogastric varices have 25-33% risk of initial bleeding, risk more than 70% of variceal re-bleeding and associated mortality over 50% in each episode. The use of nonselective beta adrenergic blockers according to previous data, descreases portal vein pressure and mostly prevents the initial bleeding. In the prevention of esophageal re-bleeding, nonselective beta adrenergic blockers (Inderal) are recommended in patients with

Transjugular intrahepatic portasystemic shunt vs surgical shunt in good-risk cirrhotic patients: a case-control comparison.

HYPOTHESIS: In good-risk patients with variceal bleeding undergoing portal decompression, surgical shunt is more effective, more durable, and less costly than angiographic shunt (transjugular intrahepatic portasystemic shunt [TIPS]). DESIGN: Retrospective case-control study. SETTING: Academic referral center for liver disease. PATIENTS: Patients with Child-Pugh class A or B cirrhosis with at least 1 prior episode of bleeding from portal hypertension (gastroesophageal varices, portal hypertensive gastropathy). INTERVENTION: Portal decompression by angiographic (TIPS) or surgical (portacaval

Economic and healthcare determinants of under-five mortality in low-income countries.

BACKGROUND: Under-five mortality (U5MR) remains a critical development challenge, particularly in low-income countries (LICs), where children face the highest risk of preventable deaths. This study explores the influence of three key variables, per capita Gross Domestic Product (PGDP), DTP1 immunisation coverage, and Government Healthcare Expenditure (GHE), on U5MR across 19 LICs from 2000 to 2020, providing a clearer understanding of their individual and combined effects. METHODS: A balanced panel dataset was analysed using both fixed-effects and random-effects panel regression models. Additi

Uncertainties in the path to 2030: Increasing trends of under-five mortality in the aftermath of Millennium Development Goal in Eastern Ethiopia.

BACKGROUND: Although Ethiopia was applauded for achieving the Millennium Development Goal (MDG) target of reducing child mortality, whether the gains sustained beyond the MDG era was rarely studied. In this study, we reported the trends and determinants of under-five mortality (U5M) from 2015 to 2020 in a population based cohort under the Kersa Health and Demographic Surveillance System (HDSS), eastern Ethiopia. METHODS: We followed pregnant women and their pregnancy outcomes from 2015 to 2020. Each year, data related to death and live births among the follow up population was retrieved. Auto…

Verbal/social autopsy analysis of causes and determinants of under-5 mortality in Tanzania from 2010 to 2016.

BACKGROUND: Tanzania has decreased its child mortality rate by more than 70 percent in the last three decades and is striving to develop a nationally-representative sample registration system with verbal autopsy to help focus health policies and programs toward further reduction. As an interim measure, a verbal and social autopsy study was conducted to provide vital information on the causes and social determinants of neonatal and child deaths. METHODS: Causes of neonatal and 1-59 month-old deaths identified by the 2015-16 Tanzania Demographic and Health Survey were assessed using the expert …

Socioeconomic factors contributing to under-five mortality in sub-Saharan Africa: a decomposition analysis.

BACKGROUND: In sub-Saharan Africa, socioeconomic factors such as place of residence, mother's educational level, or household wealth, are strongly associated with risk factors of under-five mortality (U5M) such as health behavior or exposure to diseases and injuries. The aim of the study was to assess the relative contribution of four known socioeconomic factors to the variability in U5M in sub-Saharan countries. METHODS: The study was based on birth histories from the Demographic and Health Surveys conducted in 32 sub-Saharan countries in 2010-2016. The relative contribution of sex of the ch

"Every Newborn-INDEPTH" (EN-INDEPTH) study protocol for a randomised comparison of household survey modules for measuring stillbirths and neonatal deaths in five Health and Demographic Surveillance sites.

BACKGROUND: Under-five and maternal mortality were halved in the Millennium Development Goals (MDG) era, with slower reductions for 2.6 million neonatal deaths and 2.6 million stillbirths. The Every Newborn Action Plan aims to accelerate progress towards national targets, and includes an ambitious Measurement Improvement Roadmap. Population-based household surveys, notably Demographic and Health Surveys (DHS) and Multiple Indicator Cluster Surveys, are major sources of population-level data on child mortality in countries with weaker civil registration and vital statistics systems, where over …

[Non-immunological hydrops fetalis].

Hydrops foetalis is an uncommon condition, occurring in 1/1400-1/4000 of all pregnancies. The possible etiologies to the disease are numerous and up to now the prognosis has been very dull, with a mortality-rate of 40-90%. A successfully treated case story is presented. It is concluded, that it is essential from the very beginning to treat the child aggressively to restore normal respiratory and circulatory function. When this is performed the child will have a good chance of surviving and developing normally.

Unintentional injury deaths among children under five in Hunan Province, China, 2015-2020.

Injury is the most common cause of preventable morbidity and death among children under five. This study aimed to describe the epidemiological characteristics of injury-related mortality rates in children under five and to provide evidence for future preventive strategies. Data were obtained from the Under Five Child Mortality Surveillance System in Hunan Province, China, 2015-2020. Injury-related mortality rates with 95% confidence intervals (CI) were calculated by year, residence, gender, age, and major injury subtype (drowning, suffocation, traffic injuries, falls, and poisoning). And crude…

Reproduction in the Baka pygmies and drop in their fertility with the arrival of alcohol.

To understand the diversity of human growth and development from an evolutionary point of view, there is an urgent need to characterize the life-history variables of vanishing forager societies. The small body size of the Baka pygmies is the outcome of a low growth rate during infancy. While the ages at sexual maturity, menarche, and first delivery are similar to those in other populations, fertility aspects are unknown. In the Le Bosquet district in Cameroon, thanks to systematic birth records kept from 1980 onwards, we were able to assign ages to individuals with certainty. This study, based
