Examination of Veteran Fathers' Parenting and Their Adolescent Children's Substance Use in the United States.

BACKGROUND: Adolescent children of U.S. military veterans may be at increased risk for engaging in substance use; however, this has yet to be examined using nationally representative data. Parental involvement and communication are potential protective factors to target with prevention efforts, but veterans' parenting has not been studied in general, nonclinical populations. OBJECTIVES: This study presents data on parenting characteristics among fathers who are veterans of the U.S. military and the substance use behaviors of their adolescent children. METHODS: Data were analyzed from approxi

Teen birth rates in sexually abused and neglected females.

OBJECTIVE: Prospectively track teen childbirths in maltreated and nonmaltreated females and test the hypothesis that child maltreatment is an independent predictor of subsequent teen childbirth over and above demographic characteristics and other risk factors. METHODS: Nulliparous adolescent females (N = 435) aged 14 to 17 years were assessed annually through age 19 years. Maltreated females were referred by Child Protective Services agencies for having experienced substantiated sexual abuse, physical abuse, or neglect within the preceding 12 months. Comparison females were matched on race, f…

Interventions to prevent skin cancer by reducing exposure to ultraviolet radiation: a systematic review.

The relationship between skin cancer and ultraviolet radiation is well established. Behaviors such as seeking shade, avoiding sun exposure during peak hours of radiation, wearing protective clothing, or some combination of these behaviors can provide protection. Sunscreen use alone is not considered an adequate protection against ultraviolet radiation. This report presents the results of systematic reviews of effectiveness, applicability, other harms or benefits, economic evaluations, and barriers to use of selected interventions to prevent skin cancer by reducing exposure to ultraviolet radia

Is neglect different? Social determinants of child protection plans by recorded maltreatment type in England: a longitudinal ecological study, 2014/2015-2022/2023.

OBJECTIVES: Neglect is the leading cause of acute child protection interventions across high-income countries. In England, reducing the number of children subject to these interventions is a policy priority. However, limited evidence on the health and social factors driving recent trends in neglect constrains effective prevention. We aimed to assess the associations of area-level adult mental health, child poverty and employment, with recorded neglect, and to compare these associations with those observed for other forms of maltreatment. DESIGN: Longitudinal ecological study using panel data.

Promoting Children's Mental, Emotional, and Behavioral (MEB) Health in All Public Systems, Post-COVID-19.

The COVID-19 pandemic exacerbates the mental, emotional, and behavioral (MEB) health problems of children and adolescents in the United States (U.S.). A collective and coordinated national economic and social reconstruction effort aimed at shoring up services to promote children's MEB, like the Marshall Plan that helped rebuild Europe post-World War II, has been proposed to buttress against the expected retrenchment. The plan prioritizes children's well-being as a social objective. We propose strategically reconstructing the public safety-net systems serving youth, including early education, m

Medical Marijuana for Minors May Be Considered Child Abuse.

The Food and Drug Administration categorizes marijuana (cannabis) as a Schedule I drug, meaning that it has no currently accepted medical use, a high potential for abuse, and no good data on safety. Other Schedule I drugs are heroin, lysergic acid diethylamide, peyote, methaqualone, and 3,4-methylenedioxymethamphetamine ("ecstasy"). The authors of some studies have shown that marijuana can reduce nausea and vomiting from chemotherapy, can improve food intake in patients with HIV, can reduce neuropathic pain, and may slow the growth of cancer cells. In many states, marijuana use is illegal. No

Psychosocial Stress, Course of Pregnancy and Pregnancy Outcomes in the Context of the Provision of Sexual Services.

Introduction There has been very little medical research into pregnancies which occur in the context of prostitution, even though the associated health risks for mother and child, e.g. violence or maternal drug abuse, are well known. The aim of this study was to compile and summarize what is known (inter-)nationally about this topic and identify key points of support as part of a uniform standard of healthcare in pregnancy. Material and Methods A selective search of the literature was done in Pubmed and Livivo/Medpilot and in the databases NIH, Cochrane, DARE, NHSEED and HTA on the factors inf

Responding to the needs of children in crisis.

This paper explores the issues confronting service providers setting up child and family mental health programmes in conflict, post conflict and disaster areas. Drawing on clinical experience and research in humanitarian settings, it calls for greater attention to the child's perspective, their individuality and the cultural, social and political context in which they live. It argues that those concerned with the psychopathology of children in crises should widen their frame of reference beyond narrowly defined traumatic reactions to include other mental health and psychosocial issues, includi

Incorporating a Healthy Living Curriculum within Family Behavior Therapy: A Clinical Case Example in a Woman with a History of Domestic Violence, Child Neglect, Drug Abuse, and Obesity.

Women reported to child protective service agencies frequently report problems that significantly interfere with the health and well-being of their children and themselves. Behavioral treatment programs appear to be effective in managing these co-existing problems, such as domestic violence and substance abuse. However, evidence-supported interventions are rarely exemplified in complicated clinical cases, especially within child welfare settings. Therefore, in this case example, we describe the process of adapting an evidence-supported treatment to assist in managing significant co-existing he

Recent advances in intubation and mechanical ventilation.

The ideal care of the critically ill infant or child should demonstrate a seamless service starting with recognition of potential critical illness and initiation of early resuscitative interventions, escalating to advanced life support and skilled transfer to an appropriate location where intensive care can be continued. This article discusses advances in the package of respiratory care commencing with the decision to provide respiratory support, which includes the decision whether or not to intubate, leading to the choice of mechanical ventilatory support mode in the critically ill child. Intrafamilial Child Sexual Abuse: A Unique Silent Epidemic. Perspectives of Victim-Friendly Court Professionals in Marondera District, Zimbabwe.

Intrafamilial child sexual abuse is the commonest, though the under-reported, form of child sexual abuse in Marondera District and Zimbabwe generally. However, little is known about what drives it and what practitioners know about it. This study explored the factors associated with the incidence of intrafamilial child sexual abuse in Marondera based on perceptions of Victim-Friendly Court professionals in the district. A semi-structured questionnaire was administered to twenty-five professionals from thirteen agencies implementing the Victim-Friendly Court initiative in Marondera. Relevant cou

Impact of a new mandatory reporting law on reporting and identification of child sexual abuse: A seven year time trend analysis.

Child sexual abuse is widespread and difficult to detect. To enhance case identification, many societies have enacted mandatory reporting laws requiring designated professionals, most often police, teachers, doctors and nurses, to report suspected cases to government child welfare agencies. Little research has explored the effects of introducing a reporting law on the number of reports made, and the outcomes of those reports. This study explored the impact of a new legislative mandatory reporting duty for child sexual abuse in the State of Western Australia over seven years. We analyzed data a

Case reports unlocked: Leveraging retrieval-augmented generation with large language models to advance research on psychological child maltreatment.

BACKGROUND: Research on psychological child maltreatment is impeded by a lack of high-quality structured data. Crucial information is often documented in child protective services (CPS) case files, but only in narrative form. Recent research on the utilization of retrieval-augmented generation (RAG) methods with large language models (LLMs) for extracting structured data from narratives has demonstrated significant potential. RAG methods can facilitate automated classification, thereby eliminating the need for laborious annotation. OBJECTIVE: We aimed to extract structured data from narrative…

Intimate Partner Violence, Sexual Assault, and Child Abuse Resource Utilization During COVID-19.

INTRODUCTION: Key measures in preventing spread of the virus that causes coronavirus disease 2019 (COVID-19) are social distancing and stay-at-home mandates. These measures along with other stressors have the potential to increase incidences of intimate partner violence (IPV), sexual assault, and child maltreatment. METHODS: We performed a retrospective review of county police dispatches, emergency department (ED) visits, Sexual Assault Nurse Examiner (SANE) consults, Domestic Violence Healthcare Project (DVHP) team consults, and Child Protection Team consults at a large, tertiary, Level I tr

CDC grand rounds: Creating a healthier future through prevention of child maltreatment.

Child maltreatment is abuse or neglect of a child by a parent or other caregiver that results in potential or actual harm or threats of harm to a child. Maltreatment encompasses both acts of commission (abuse) and omission (neglect). Child maltreatment is divided into four types: 1) physical abuse (e.g., hitting, kicking, shaking, or burning); 2) sexual abuse (e.g., rape or fondling); 3) psychological abuse (e.g., terrorizing or belittling); and 4) neglect, which involves the failure to meet a child's basic physical, emotional, or educational needs (e.g., not providing nutrition, shelter, or m

Referral pathways as mechanisms of child maltreatment identification: evidence from a German Childhood-House.

BACKGROUND: Adverse childhood experiences-related child maltreatment, including abuse and neglect, are major threats to children's safety, health, and development. Their identification depends on recognition and referral processes occurring across multiple professional systems. Childhood-Houses, based on Child Advocacy Center and Barnahus principles, provide multidisciplinary assessment and support for children with suspected maltreatment experiences. OBJECTIVE: This study examined whether identified child maltreatment profiles and multidisciplinary assessment processes differed according to

The Impact of an Emergency Department Bruising Pathway on Disparities in Child Abuse Evaluation.

OBJECTIVES: Previous research has shown racial, ethnic, and socioeconomic disparities in provider medical evaluations and reporting to child protective services (CPS) and law enforcement (LE) for cases of suspected child physical abuse. Our hospital standardized evaluation and reporting of high-risk bruising using a clinical pathway. We aimed to assess whether standardization impacted disparity. METHODS: We performed a retrospective observational study including children evaluated in the emergency department who had a social work consult for concern for child abuse or neglect between June 201…

Predicting child maltreatment recurrences during treatment.

UNLABELLED: The primary purpose of the child protective services system is to protect children from the recurrence of child maltreatment. Understanding more about what predicts recurrence may help us more adequately target interventions to reduce the risk of future maltreatment. OBJECTIVE: The specific objective of this study was to identify correlates of recurrence during CPS intervention for families who were provided continuing intervention following a confirmed index report of physical abuse or neglect. METHOD: This nonconcurrent prospective study selected 446 subject families who met st…

Characteristics of Child Abuse Cases Referred to a Hospital-Based Child Protection Teams in Korea and the Role of Psychiatric Intervention.

PURPOSE: Child protection teams (CPTs) have been implemented in hospital settings to effectively intervene in cases of child abuse. We investigated the changes in child abuse reporting rates and examined whether there was an increase in follow-up psychiatric outpatient visits. Furthermore, we aimed to identify the characteristics of children referred to CPTs and the factors that significantly influenced psychiatric outpatient visits. MATERIALS AND METHODS: We retrospectively reviewed cases of patients under 18 years old who were referred to the CPT from 2014 to 2021. We examined the demograph
