Young adults living with chronic noncommunicable diseases (NCDs) in Ghana face unique challenges that extend beyond physical and psychosocial suffering to include unmet sexual and reproductive health (SRH) needs. Yet, SRH remains a neglected aspect of palliative and end-of-life (EoL) care in the country. This article explores why SRH needs are rarely discussed in palliative and EoL care settings. It also examines the factors contributing to the high-unmet need for SRH needs, the repercussions, and possible actions that can be implemented to elevate SRH as a core dimension of palliative and EoL

Systematic reviews suggest that stand-alone hypnotic suggestions may improve pain outcomes compared with no treatment, waitlist, or usual care. However, in clinical practice, hypnosis is often provided adjunctively with other interventions, which might have different effects than those reported in previous reviews. This systematic review aimed to summarize the analgesic effects of adjunctive hypnosis in adults with clinical pain. Seven databases (MEDLINE, Embase, PsycINFO, Emcare, SCOPUS, CENTRAL, Cochrane) were searched up to January 2024. Randomised controlled trials comparing the analgesic

INTRODUCTION: Approximately 15% to 40% of persons with chronic pain as a primary disorder experience brain fog. Prior research has investigated the etiology of "brain fog" in conditions in which pain presents as a key feature (e.g., fibromyalgia). However, it remains understudied in the context of chronic 10 musculoskeletal pain. Following current scoping review guidelines, we obtained stakeholder input from patient and health care professionals (HCPs) to define this phenomenon. Specific aims of this review were to (1) identify factors contributing to brain fog, (2) identify the functional cor

BACKGROUND: Osteoarthritis is a common, painful and disabling long-term condition. Delivery of high-quality guideline-informed osteoarthritis care that successfully promotes and maintains supported self-management is imperative. However, osteoarthritis care remains inconsistent, including under use of core non-pharmacological approaches of education, exercise and weight loss. Community pharmacies are an accessible healthcare provider. United Kingdom government initiatives are promoting their involvement in a range of long-term conditions, including musculoskeletal conditions. It is not known w

Since the enactment of the Affordable Care Act (ACA) in 2010, the Centers for Medicare & Medicaid Services (CMS) have embraced value-based care as the organizing principle of national payment reforms. Yet, after more than a decade, the value-based movement has, at best, generated modest returns and at worst increased costs on patients and negatively impacted access. Pursuant to its authority under the ACA, CMS proposed the Ambulatory Specialty Model (ASM) in the CY 2026 Physician Fee Schedule rulemaking and subsequently finalized the model, establishing the agency's first mandatory alternative

The economic burden of chronic liver disease is rising; however, the financial impact of chronic liver disease on patients and families has been underexplored. We performed a scoping review to identify studies examining financial burden (patient/family health care expenditures), financial distress (material, behavioral, and psychological consequences of financial burden), and financial toxicity (adverse health outcomes of financial distress) experienced by patients with chronic liver disease and their families. We searched MEDLINE, Embase, Cochrane Library, and the Web of Science online databa

We compared prospective risk adjustment models for adjusting patient panels at the San Francisco Department of Public Health. We used 4 statistical models (linear regression, two-part model, zero-inflated Poisson, and zero-inflated negative binomial) and 4 subsets of predictor variables (age/gender categories, chronic diagnoses, homelessness, and a loss to follow-up indicator) to predict primary care visit frequency. Predicted visit frequency was then used to calculate patient weights and adjusted panel sizes. The two-part model using all predictor variables performed best (R = 0.20). This mod

BACKGROUND: Health care for individuals experiencing homelessness is typically fragmented, passive, reactionary, and lacks patient-centeredness. These challenges are exacerbated for people who experience chronic medical conditions in addition to behavioral health conditions. The objective was to evaluate an innovative healthcare delivery model (The Mobile, Medical, and Mental Health Care [M3] Team) for individuals experiencing homelessness who have trimorbid chronic medical conditions, serious mental illness, and substance use disorders.
METHODS: We assessed changes in study measures before a

PURPOSE: The United States is facing an unprecedented opioid epidemic. The Affordable Care Act (ACA) included several provisions designed to increase care coordination in state Medicaid programs and improve outcomes for those with chronic conditions, including substance use disorders. Three states-Maryland, Rhode Island, and Vermont - adopted the ACA's optional Medicaid health home model for individuals with opioid use disorder. The model coordinates opioid use disorder treatment that features opioid agonist therapy provided at opioid treatment programs (OTPs) and Office-based Opioid Treatment

IMPORTANCE: Accessible and effective management of atopic dermatitis (AD) across geographic boundaries is important. Scalable, collaborative care models that integrate patients, primary care practitioners, and specialists remain an unmet need.
OBJECTIVE: To determine whether an online, team-based connected health (TCH) model provides equivalent improvements in AD severity compared to in-person care.
DESIGN, SETTING, AND PARTICIPANTS: In this pragmatic, randomized clinical equivalence trial, 300 patients with AD were enrolled from 8 outpatient dermatology clinics in California from August 201

As the landscape of modern medicine evolves, new health care delivery models are needed to meet the growing complexity of medical care. Primary care clinicians are increasingly managing patients with multiple chronic conditions, a challenge that will intensify as the population ages. Complex medical care involves multiple factors that contribute to difficulties in providing optimal quality of care for patients facing these types of medical problems. To address this need, models of consultative general internal medicine have emerged at various tertiary care institutions across the United States

BACKGROUND: Naloxone is a life-saving opioid antagonist. Chronic pain guidelines recommend that physicians co-prescribe naloxone to patients at high risk for opioid overdose. However, clinical tools to efficiently identify patients who could benefit from naloxone are lacking.
OBJECTIVE: To develop and validate an overdose predictive model which could be used in primary care settings to assess the need for naloxone.
DESIGN: Retrospective cohort.
SETTING: Derivation site was an integrated health system in Colorado; validation site was a safety-net health system in Colorado.

BACKGROUND: Familial hypercholesterolemia (FH) confers lifelong elevation in low-density lipoprotein-cholesterol (LDL-C) and markedly increases the risk for premature atherosclerotic cardiovascular disease. Because atherosclerosis begins early in FH, timely initiation of statin therapy reduces lifetime LDL-C exposure, slows disease progression, and improves cardiovascular event-free survival.
SOURCES OF MATERIAL: Relevant studies were identified through PubMed and Embase searches (2010-2025) using the terms FH, transition of care, and lipid management, supplemented by guidelines and consensus
