Perioperative Circulating Tumor DNA in Resectable Gastric and Gastroesophageal Junction Adenocarcinoma: Molecular Residual Disease, Recurrence Prediction, and a Trial-Ready Framework for Treatment Adaptation.

Recurrence remains a major cause of treatment failure after curative-intent therapy for resectable gastric and gastroesophageal junction adenocarcinoma. Clinicopathological staging, pathological response, serum tumor markers, and imaging incompletely identify patients with molecular residual disease (MRD). Circulating tumor DNA (ctDNA) has therefore emerged as a minimally invasive biomarker for perioperative risk stratification, response assessment, postoperative MRD detection, and recognition of molecular recurrence before radiographic relapse. In gastric and gastroesophageal junction cancer,…

Stage-specific efficacy of osimertinib in treatment-naïve EGFR-mutant non-small cell lung cancer according to baseline genetic alterations in circulating tumor DNA.

The impact of clinical stage on the effectiveness of osimertinib for epidermal growth factor receptor (EGFR) mutation-positive non-small cell lung cancer (NSCLC) remains unexamined. We investigated osimertinib therapeutic efficacy variation between stage IVA or lower and stage IVB EGFR mutation-positive lung cancers, focusing on differences in pretreatment co-occurring genetic alterations in circulating tumor DNA. This was a secondary analysis of the ELUCIDATOR study, a multicenter prospective observational study in Japan that assessed the mechanisms underlying resistance to osimertinib as a f…

Photoelectrochemical and fluorescent dual-mode sensitive detection of circulating tumor cells based on aptamer DNA-linked CdTe QDs/Bi2MoO6/CdS "double Z-scheme" system.

Herein, a novel dual-mode biosensor integrating photoelectrochemical (PEC) and fluorescence (FL) sensing detection was developed for circulating tumor cells (CTCs) based on aptamer DNA-linked CdTe QDs (Apt-CdTe QDs)/Bi2MoO6/CdS "dual Z-scheme" sensing system. Apt-CdTe QDs were assembled on FTO/CdS/Bi2MoO6/cDNA electrode through hybridization with cDNA, resulting in the formation of a double Z-scheme CdS/Bi2MoO6/CdTe heterostructure that significantly enhanced the separation of photo-generated charge carriers, thereby improving photocatalytic efficiency. Upon the presence of MCF-7 cells, Apt-QD

Phased Variant-Supported Circulating Tumor DNA as a Prognostic Biomarker After First-Line Treatment in Large B-Cell Lymphoma: Findings From the DIRECT Study.

PURPOSE: Circulating tumor DNA (ctDNA) is emerging as a promising tool to monitor treatment response in large B-cell lymphoma (LBCL). Tracking tumor-specific phased variants (PVs) allows ultrasensitive detection of minimal residual disease (MRD) that may enhance the accuracy of response assessment. Previous studies have been constrained by small cohort size, retrospective design, or assays limited to a single commercial provider. PATIENTS AND METHODS: DIRECT was a prospective, multisite study evaluating the utility of ctDNA in patients with LBCL. We developed a lymphoma-customized, open-sourc…

Dynamic circulating tumor DNA profiling of ESR1 mutations in HR+/HER2- advanced breast cancer: from prognostic biomarker to clinical decision-making - a comprehensive review.

BACKGROUND: The combination of CDK4/6 inhibitors (CDK4/6i) and endocrine therapy (ET) is the standard first-line regimen for hormone receptor-positive, human epidermal growth factor receptor 2-negative (HR+/HER2-) metastatic breast cancer (MBC). Resistance remains a challenge, with estrogen receptor 1 gene (ESR1) mutations serving as a central mechanism. Liquid biopsy enables non-invasive monitoring via circulating tumor DNA (ctDNA), with ESR1 mutations increasingly recognized as dynamic molecular events rather than static markers. OBJECTIVE: This comprehensive review evaluates the prognostic…

Multiselective Recognition of Metal Ion-Nucleic Acid Complexes by CRISPR/Cas12a and Quantum Dots Enables the Profiling of Circulating Tumor DNA in Breast Cancer.

The rapid, noninvasive detection of circulating tumor DNA (ctDNA) is vital for the diagnosis and staging of breast cancer (BC). In this study, we developed a homogeneous CRISPR/Cas12a fluorescent platform using a hierarchical grape-cluster rolling circle amplification (GCRCA) nanomaterial to detect the PIK3CA E542K mutation. A pivotal discovery of this study is that activated Cas12a efficiently cleaves metal ion-mediated cytosine-Ag+-cytosine base pairs, which enables direct coupling between enzymatic activity and signal transduction. GCRCA, assembled by precise hybridization of long-chain RCA…

PURPOSE: Circulating tumor DNA (ctDNA) holds promise for prognostication and disease monitoring in solid tumors. However, rare and heterogenous tumors with low mutational burden like soft tissue sarcomas (STS) present a technical challenge for ctDNA analysis. We investigated the ability of personalized ctDNA analysis to predict outcomes and track response to radiotherapy and pembrolizumab in patients with localized STS. METHODS: We analyzed 313 plasma samples collected pretreatment, postradiotherapy/presurgery, and postsurgery from 106 patients treated with preoperative radiotherapy with or w…

Circulating Tumor DNA-Based Molecular Residual Disease in Early-Stage Non-Small-Cell Lung Cancer: Biology, Assays, and the Path Toward MRD-Guided Care.

Complete surgical resection remains the cornerstone of curative therapy for early-stage non-small-cell lung cancer (NSCLC), yet a substantial proportion of patients relapse despite a macroscopically complete operation and negative conventional staging. This residual risk reflects micrometastatic disease that is below the resolution of computed tomography and that current clinicopathologic risk factors only coarsely predict. Circulating tumor DNA (ctDNA) provides a direct, blood-based readout of this molecular residual disease (MRD), its detection after definitive treatment is among the stronge…

Bladder Cancer with Undetectable Circulating Tumor DNA After Radical Cystectomy May Be Amenable to a Less Intense Imaging Surveillance Protocol: A Diagnostic Accuracy Study.

BACKGROUND AND OBJECTIVE: Circulating tumor DNA (ctDNA) has shown promise as a prognostic biomarker for bladder cancer management. Current surveillance guidelines for patients with muscle-invasive bladder cancer recommend intense postoperative surveillance. Our aim was to assess the diagnostic performance of ctDNA in comparison to imaging studies for detection of disease recurrence after radical cystectomy. METHODS: We analyzed patients who underwent robot-assisted radical cystectomy and prospective tumor-informed ctDNA analysis (Signatera™) between 2021 and 2023 at a single institution. Pati…

Changes in Circulating Tumor DNA Reflect Clinical Benefit Across Multiple Studies of Patients With Non-Small-Cell Lung Cancer Treated With Immune Checkpoint Inhibitors.

PURPOSE: As immune checkpoint inhibitors (ICI) become increasingly used in frontline settings, identifying early indicators of response is needed. Recent studies suggest a role for circulating tumor DNA (ctDNA) in monitoring response to ICI, but uncertainty exists in the generalizability of these studies. Here, the role of ctDNA for monitoring response to ICI is assessed through a standardized approach by assessing clinical trial data from five independent studies. PATIENTS AND METHODS: Patient-level clinical and ctDNA data were pooled and harmonized from 200 patients across five independent …

Blood-Based Surveillance Monitoring of Circulating Tumor DNA From Patients With SCLC Detects Disease Relapse and Predicts Death in Patients With Limited-Stage Disease.

INTRODUCTION: Most patients (70%) with limited-stage SCLC (LS-SCLC) who are treated with curative-intent therapy suffer disease relapse and cancer-related death. We evaluated circulating tumor DNA (ctDNA) as a predictor of disease relapse and death after definitive therapy in patients with LS-SCLC. METHODS: In our previous work, we developed a plasma-based ctDNA assay to sequence 14 genes (TP53, RB1, BRAF, KIT, NOTCH1-4, PIK3CA, PTEN, FGFR1, MYC, MYCL1, and MYCN) that are frequently mutated in SCLC. In this work, we evaluated 177 plasma samples from 23 patients with LS-SCLC who completed defi…
