[Clinical thinking and decision making in practice. The internist as a consultant for a patient with unexplained anemia in the cardiology department].

A man aged 73, admitted because of unstable angina pectoris also had a anaemia with a haemoglobin concentration of 2.8 mmol/l. The department of Consultative Internal Medicine was asked to elucidate this anaemia. The anamnesis proved to contain extensive diagnostic tests concerning the anaemia which had had no results. In spite of a negative anamnesis, the low serum iron level had prompted an unsuccessful search for a source of haemorrhage in the proximal and distal parts of the digestive tract. The bone marrow had been examined three times without a clear diagnosis. It was also found that the

[Clinical thinking and decision-making in practice. A patient with arthritis, skin lesions and a perforated nasal septum].

A man aged 38 years had arthritis, skin lesions and a perforation of the nasal septum. He had slight deterioration of renal function and a few granular casts in the urine. Antineutrophil cytoplasmic antibody and other immune parameters were normal. A biopsy of the nasal septum did not show granulomatous lesions. The differential diagnosis on the available clinical grounds could be narrowed to Wegener's disease, Henoch-Schönlein vasculitis or microscopic polyangiitis. A biopsy of the skin showed leukocytoclastic vasculitis, with IgA and IgM depositions compatible with Henoch-Schönlein vasculiti

Critical decisions: A mixed-methods study of decision-making among diverse gynecologic cancer patients considering therapeutic clinical trial enrollment.

OBJECTIVE: Participation in therapeutic clinical trials does not reflect the diversity of gynecologic cancer patients, limiting access to novel therapeutics and generalizability of results. Reasons for inequities in participation among historically underrepresented populations remain undertheorized, as studies have shown equal willingness to participate among groups. We sought to apply a precarity framework to conceptualize the factors that impact patients' desire to enroll, to improve equity in gynecologic oncology clinical trial participation. METHODS: Gynecologic cancer patients at a singl

Clinical Decision-Making in Laparoscopic Common Bile Duct Exploration: Choosing Between Primary Closure and T-Tube Drainage.

AIM: Choledocholithiasis, or common bile duct (CBD) stones, is increasingly prevalent in aging populations worldwide. Laparoscopic common bile duct exploration (LCBDE) is a well-established minimally invasive procedure; however, the optimal method for choledochotomy closure-primary closure (PC) versus T-tube drainage (TTD)-remains controversial. METHODS: This narrative review summarizes the available evidence from randomized controlled trials, meta-analyses, and observational studies to compare perioperative practices, clinical outcomes, and patient-specific variables associated with PC and T

Posterior Canal and Atypical Benign Paroxysmal Positional Vertigo: Development of a Predictive Model for Clinical Decision-Making.

BACKGROUND: Benign paroxysmal positional vertigo (BPPV) is the most common cause of dizziness. Despite its prevalence, clinical diagnosis can be challenging because of difficulty discerning the direction and intensity of eye movements, resulting in diagnostic uncertainty and suboptimal treatment outcomes. The aim of this study was to develop and evaluate a data-driven model designed to differentiate between typical and atypical variants of BPPV. METHODS: A retrospective cross-sectional design included data from 26 patients diagnosed clinically with either typical BPPV or atypical variants of
