Clinical Reasoning: A 17-Year-Old Adolescent Boy With New Altered Mental Status in the Pediatric Intensive Care Unit.

We present the case of a 17-year-old adolescent boy admitted to the Pediatric Intensive Care Unit with an extensive necrotizing soft tissue infection who subsequently developed altered mental status and autonomic instability. Altered mental status is a common occurrence in critically ill children with a broad differential of etiologies. After ruling out organic causes of encephalopathy, management is typically focused on avoiding deliriogenic agents, including benzodiazepines. Dopamine antagonist medications may also be administered adjunctively to manage agitation or delirium that is refracto

Lessons in clinical reasoning - pitfalls, myths, and pearls: shoulder pain as the first and only manifestation of lung cancer.

OBJECTIVES: Lung cancer is the leading cause of cancer-related death and poses significant challenges in diagnosis and management. Although muscle metastases are exceedingly rare and typically not the initial clinical manifestation of neoplastic processes, their recognition is crucial for optimal patient care. CASE PRESENTATION: We present a case report in which we identify the unique scenario of a 60-year-old man with shoulder pain and a deltoid muscle mass, initially suggestive of an undifferentiated pleomorphic sarcoma. However, further investigations, including radiological findings and m

Clinical Reasoning: Hyperventilation-Induced Alternating Hemiplegia With Concomitant Hemispheric EEG Slowing in a 7-Year-Old Girl With Headache.

A 7-year-old right-handed girl presented to the pediatric neurology outpatient clinic after 5 episodes of headache over the previous 3 months. Her family history was positive for migraine in the mother and maternal grandmother and for febrile seizures in the older sister. The neurologic examination and cognitive profile were normal. Five seconds after the end of hyperventilation, video-EEG showed high-amplitude delta waves predominantly over the left hemisphere with concomitant acute aphasia and right-sided weakness. After the event, which self-resolved over 8 minutes, the girl showed intact r

Lessons in clinical reasoning - pitfalls, myths, and pearls: the contribution of faulty data gathering and synthesis to diagnostic error.

OBJECTIVES: Errors in clinical reasoning are a major factor for delayed or flawed diagnoses and put patient safety at risk. The diagnostic process is highly dependent on dynamic team factors, local hospital organization structure and culture, and cognitive factors. In everyday decision-making, physicians engage that challenge partly by relying on heuristics - subconscious mental short-cuts that are based on intuition and experience. Without structural corrective mechanisms, clinical judgement under time pressure creates space for harms resulting from systems and cognitive errors. Based on a ca…

Assessing musculoskeletal examination skills and diagnostic reasoning of 4th year medical students using a novel objective structured clinical exam.

BACKGROUND: Medical students have difficulty performing and interpreting musculoskeletal physical examinations and interpreting the findings. Research has focused on students' knowledge deficits, but there are few direct assessments of students' ability to perform a hypothesis-driven physical examination (HDPE). We developed a novel musculoskeletal Objective Structured Clinical Exam (OSCE) focusing on HDPE skills for disorders of the shoulder, back and knee, and used it to explore medical student diagnostic reasoning. METHODS: A multidisciplinary group of musculoskeletal specialists developed

The structure of novice and expert occupational therapists' clinical reasoning before and after exposure to a domain-specific protocol.

BACKGROUND/AIM: The research aimed to determine the influence of a protocol designed for use in the domain of upper limb hypertonia due to brain injury on novice and expert occupational therapy clinical reasoning. METHOD: Individual, structured repertory grid interviews were completed with 13 novice and eight expert occupational therapists prior to, and following, exposure to a domain-specific clinical reasoning protocol. Data were subjected to quantitative analyses (Principal Components Analysis, Generalised Procrustes Analysis). RESULTS: Novice participants demonstrated statistically signi…

Cognitive strategies in the Emergency Medical Services: proposal for a conceptual integrated framework to explicate clinical reasoning in EMS.

BACKGROUND: Clinical reasoning is fundamental to safe and effective Emergency Medical Services (EMS) care. EMS providers operate in dynamic, time-pressured environments characterised by diagnostic uncertainty, limited information, and high-stakes decisions. These conditions necessitate rapid yet adaptable reasoning strategies. The SPART model was developed through focused ethnography and content validation to describe observable diagnostic and therapeutic activities in EMS practice. It provides a structured representation of the care process from dispatch to transfer of care, serving as a basis…

Managing the Complexity-Coherence Trade-Off in Clinical Reasoning: A Two-Stage Graph Ontology Based on WHO International Classifications.

BACKGROUND: Clinical reasoning in contemporary practice often involves ill-structured, poorly defined problems that span the biopsychosocial domain and require reasoning under high relational complexity. Despite sustained scholarly attention to clinical reasoning, few models are both theoretically grounded and usable for clinicians who face this complexity. At the same time, the World Health Organization Family of International Classifications (WHO-FIC) offers a rich but complex ontology that was not designed primarily to support clinical reasoning in practice. AIM: To develop a two-stage WHO…

Learning to reflect together: a video-facilitated reflective inquiry and linguistic analysis of collaborative clinical reasoning in intensive care.

BACKGROUND: Collaborative clinical reasoning (CCR) is essential to safe and effective practice in intensive care units, where decisions are shaped by uncertainty, high cognitive load, and interprofessional dynamics. Although CCR is widely recognised as a collective cognitive process, little is known about how clinicians learn to reason together or how reflective educational interventions may influence the language and social organisation through which CCR is enacted. METHODS: This exploratory mixed-methods Groundwork study examined the association between a video-facilitated reflective (VFR) …

A comparative analysis of descriptive podcasts vs. interview-based podcasts in enhancing clinical reasoning skills of undergraduate medical students.

BACKGROUND & OBJECTIVE: Audio podcasts offer flexibility and align with adult learning principles, yet their role in enhancing clinical reasoning and optimal delivery format remains unclear. This study compared descriptive and interview-based podcasts to evaluate their effectiveness in improving clinical reasoning in undergraduate medical students and determine the superior podcast format. METHODOLOGY: The study conducted from March to May 2024 used a pretest-posttest crossover design to evaluate the effectiveness of two podcast formats (descriptive and interview-based) in enhancing clinical …

The role of cardiac ultrasound virtual simulation technology in the construction of clinical diagnostic reasoning of structural heart diseases.

BACKGROUND: Clinical reasoning education is a very systematic and highly logical medical learning process, but the existing teaching models and methods often divide this process into several stages for separate training using a virtual reality (VR)simulator can simulate and reconstruct the cardiac anatomical structure, ultrasonic section operation, and color doppler ultrasound parameter measurement, then integrate it into a single disease or case. In this case, the students' operation skills,decision-making and communication skills were trained by cardiac physical examination, inquiry and inte…

Gut feeling: A grounded theory study to identify clinical educators' reasoning processes in putting students on a learning contract.

AIM: To develop a substantive theoretical explanation that makes sense of the decision-making process that clinical instructors use to place students on a learning contract. BACKGROUND: Clinical instructors are challenged with the task of objectively evaluating students using subjective tools such as anecdotal notes, diaries, unstructured observations and verbal feedback from other nurses. Clinical instructors' assessment decisions have a considerable impact on a variety of key stakeholders, not least of all students. DESIGN: Grounded theory method and its heuristic tools including the logic

Non-Small Cell Lung Carcinoma: Clinical Reasoning in the Management of a Patient Referred to Physical Therapy for Costochondritis.

BACKGROUND AND PURPOSE: Chest pain, a frequent complaint for seeking medical care, is often attributed to musculoskeletal pathology. Costochondritis is a common disorder presenting as chest pain. Initial physical therapist examination emphasizes red flag screening. Reexamination throughout the episode of care is critical, particularly when patients are not progressing and/or in the presence of complex pain presentations. The purpose of this case report is to describe the clinical reasoning process in the management of a patient referred to physical therapy with a medical diagnosis of costochon…

Bayesian clinical reasoning: does intuitive estimation of likelihood ratios on an ordinal scale outperform estimation of sensitivities and specificities?

RATIONALE: Bedside use of Bayes' theorem for estimating probabilities of diseases is cumbersome. An alternative approach based on five categories of powers of tests from 'useless' to 'very strong' has been proposed. The performance of clinicians using it was assessed. METHODS: Fifty clinicians attending a course of tropical medicine estimated powers of tests and post-test probabilities using the classical vs. the categorical Bayesian approach. The estimation of post-test probability was assessed for real and dummy diseases in order to avoid the bias of previous knowledge. Accuracy of answers

[Clinical reasoning and decision-making in practice. A 76 year old woman with gastric carcinoma and cardiac valve disease].

A 76-year-old woman with combined aortic and mitral valve disease presented with anaemia due to a gastric carcinoma. Further staging revealed no evidence for metastatic disease. Approval for surgery for the carcinoma was obtained after a cardiologist and anaesthesiologist were consulted. On the day of surgery, however, the attending anaesthesiologist estimated the operative risk to be unacceptable. The patient reversed her decision and decided not to have the oncological operation, as she felt well at the moment and considered the limited additional survival time not worth the anxiety. The exp

[Clinical reasoning and decision-making in practice. A man with jaundice, renal function disorder, thrombocytopenia, muscular pain and confusion].

A 51-year-old man became jaundiced, after experiencing flu-like symptoms for a week. On admission anaemia, thrombocytopenia, and hepatic and renal function disorders were present. One month previously he had had sinusitis. Thrombotic thrombocytopenic purpura (TTP)/haemolitic-uraemic syndrome (HUS) was suspected, but the thrombocyte count rose spontaneously. The leukocytosis indicated an infectious cause of the symptoms and the jaundice appeared to originate from intrahepatic cholestasis. All symptoms could be traced to leptospirosis icterohaemorrhagica (known as 'Weil's disease'). The source w
