A phase II trial of cyclosporin A in the treatment of refractory metastatic colorectal cancer.

Cyclosporin A (CSA) is an immunosuppressive agent that in experimental models has antiproliferative activity against colon cancer and other human neoplasms. A phase II trial was conducted to evaluate CSA in refractory colorectal malignancies. CSA was administered at a starting dose of 7.0 mg/kg twice daily (total dose, 14 mg/kg/day) and escalated to tolerance. Of 18 patients with measurable disease, 17 were evaluable. All had been treated with one fluorouracil-containing regimen. The European Cooperative Oncology Group (ECOG) performance status was 0 or 1 in 17 of the 18 patients. No objective

SMARCB1 (INI1)-Deficient Undifferentiated Rectal Carcinoma Mimicking Sarcoma: A Case Report and Literature Review.

Undifferentiated carcinomas with rhabdoid morphology and loss of SMARCB1 (INI1) are rare, highly aggressive neoplasms. In the colorectum, they account for < 1% of cases and carry a poor prognosis. We report a 41-year-old man with congenital hemophilia A who presented with pelvic fullness and altered bowel habits. Imaging revealed a large rectosigmoid mass without distant metastasis. Core needle biopsy suggested a high-grade sarcomatoid malignancy. Following multidisciplinary planning and perioperative factor VIII replacement, owing to progressive symptoms and concern for impending obstruction,

Sclerosing mesenteritis mimicking metachronous peritoneal metastases from descending colon adenocarcinoma.

BACKGROUND: Sclerosing mesenteritis is a non-neoplastic inflammatory disease that occurs in the bowel mesentery. Distinguishing sclerosing mesenteritis from neoplasms may be difficult because of the clinical and radiographic similarities between the two disease entities. CASE PRESENTATION: We report a case of sclerosing mesenteritis mimicking peritoneal metastases of colorectal carcinoma. A 73-year-old man with stage II descending colon adenocarcinoma with poor prognostic features was found to have developed left lower abdominal quadrant masses on computed tomography (CT) 9 months after under

Metachronous occurrence of colorectal cancer in a muir-torre syndrome patient presenting with recurrent sebaceous adenoma of the eyelid: case report and updated review of the literature.

BACKGROUND: Muir-Torre syndrome (MTS) is a rare genodermatosis considered a subtype of hereditary nonpolyposis colorectal cancer and traditionally associated with mutations in the mismatch repair genes. OBJECTIVE: We describe a 51-year-old male with primary manifestations of recurrent sebaceous adenoma of the upper eyelid, a positive cancer family history, and metachronous occurrence of colorectal cancer. METHOD: The diagnosis of MTS was established based on the clinical course, family history, and histopathologic findings, although further immunohistologic testing revealed the absence of MS

Aspirin and cancer risk: an update to 2001.

Evidence of a protective role of aspirin on the risk of colorectal and other common cancers has been building up since the end of the 1980s. There are now more than 15 epidemiological (case-control and cohort) studies indicating that long-term use of aspirin is associated with a reduced risk of colorectal cancer. The overall relative risk (RR) estimate for regular aspirin users was 0.71 (95% confidence interval (CI) 0.66-0.77) from case-control studies, and 0.84 (95% CI 0.72-0.98) from cohort studies. A recent meta-analysis reported a RR of breast cancer for aspirin use of 0.70 (95% CI 0.61-0.

The Genomic Topography of Appendiceal Cancers: Our Current Understanding, Clinical Perspectives, and Future Directions.

Background/Objectives: Appendiceal cancer (AC) is a rare and understudied malignancy with limited genomic data available to guide clinical interventions. Historically treated as a subtype of colorectal cancer, AC is now recognized as a distinct disease with unique histologic subtypes and molecular features. This review aims to consolidate current genomic data across AC subtypes and explore the clinical relevance of recurrent mutations. Methods: A systematic literature review was performed in accordance with general Preferred Reporting Items for Systemic Reviews and Meta-Analyses (PRISMA) guide

Fistula Between a Primary Sigmoid Large B-Cell Lymphoma and an Ovarian Teratoma.

Primary colorectal lymphoma is a rare neoplasm. We report the case of a fistula between a diffuse large B-cell lymphoma of the sigmoid colon and an ovarian teratoma. An emergent laparotomy for an acute abdomen in a 90-year-old woman was performed. A pelvic mass of 12 × 9 cm fistulized in the left colon was found with the presence of gas and free liquid within the abdomen. This is an extremely rare condition, and as far as we know, no cases of a fistula between a large B-cell colonic lymphoma and an ovarian teratoma are present in the literature.

Changes in the 6th edition of the World Health Organization classification of tumours of the digestive system.

The 6th Edition of the WHO Classification of Digestive System Tumours represents a significant update to the 5th edition. It integrates pathological, new molecular, and clinical insights to refine the taxonomy of digestive system neoplasms. The revised classification continues to emphasise standardisation in terminology, coding, and diagnostic criteria to facilitate global consistency in diagnosis, treatment, epidemiological reporting and research. Structural reorganisation of book chapters describes epithelial tumours by anatomical site, while separating neuroendocrine, mesenchymal and haemat

A case of metastatic rectal adenocarcinoma diagnosed by thyroid cytology.

Fine-needle aspiration (FNA) cytology of the thyroid gland has been widely used for the evaluation of thyroid nodules. Most of the nodules are primary thyroid lesions. However, up to 4% of thyroid FNA may harbor a metastatic neoplasm. The metastases are most commonly from lung, kidney, breast, and skin. Metastatic colorectal cancers are also seen in thyroid but less common. Here we report the cytologic features, differential diagnosis and clinical implications of a metastatic rectal adenocarcinoma to the thyroid gland diagnosed by FNA.

Uniportal video-assisted thoracic surgery approach for simultaneous lung cancer and thymic carcinoma: Case report and literature review.

Thymic carcinoma is an epithelial tumor derived from thymic epithelial cells. Thymic tumors may be associated with other simultaneous and/or metachronous extra-thymic tumors (e.g., lung cancer). Here, we report a case of simultaneous surgical management of lung and mediastinal neoplasm together with a review of the literature. During radiological follow-up for prostate and colorectal cancer, an 82-year-old man was diagnosed with lung cancer with simultaneous mediastinal suspected neoplasm. Both were surgically removed with a single intervention performed via a uniportal video-assisted thoracic

Mixed adenoneuroendocrine carcinoma of the tongue arising within a congenital enteric cyst.

BACKGROUND: Primary intestinal-type adenocarcinoma of the tongue is rare. This represents the first reported case of a primary mixed adenoneuroendocrine carcinoma (MANEC) of the tongue arising within a congenital enteric cyst. METHODS: A 52-year-old man presented with a midline tongue mass that was connected to the mucosal surface with an epithelial-lined sinus tract. Morphological examination and immunohistochemical profiling of the neoplasm were performed. RESULTS: Histopathological evaluation showed a primary MANEC arising within a preexisting congenital enteric cyst, comprising both aden

Retrospective analysis of Claudin 18.2 expression in 64 patients with advanced gastrointestinal peritoneal disease: a potential therapeutic target for high-grade appendiceal mucinous carcinoma.

AIMS: Specific to normal gastric epithelium, Claudin 18.2 (CLDN18.2) is a tight junction protein. CLDN18.2 immunohistochemistry is a companion diagnostic used to identify oesophageal and gastric adenocarcinoma patients eligible for targeted therapy with zolbetuximab. It has demonstrated survival benefits in advanced upper gastrointestinal (UGI) cancers, with many studies now exploring its potential use beyond the UGI tract. This study aimed to examine CLDN18.2 expression in a peritoneal malignancy patient cohort. METHODS: CLDN18.2 immunohistochemistry was performed on archived tissue from pat

Age-specific trends in colorectal, appendiceal, and anal tumour incidence by histological subtype in Australia from 1990 to 2020: a population-based time-series analysis.

BACKGROUND: Early-onset bowel cancer incidence (age <50 years) has increased worldwide and is highest in Australia, but how trends vary across age, histology, and anatomical site remains unclear. We investigated appendiceal, proximal colon, distal colon, rectal, and anal cancer incidence trends by age and histology in Australia. METHODS: Cancer incidence rate data were obtained from nation-wide cancer registries (1990-2020). Birth cohort-specific incidence rate ratios (IRRs) and annual percentage change were estimated using age-period-cohort modelling and joinpoint regression. FINDINGS: Comb

Incidental Appendiceal Adenocarcinoma Discovered After Ileocecectomy for Recurrent Small Bowel Obstruction: A Case Report.

We report a case of moderately differentiated appendiceal adenocarcinoma discovered on histopathologic examination following ileocecectomy for recurrent SBO in a woman in her early 50s with a history of stage III rectal adenocarcinoma in remission. Notably, serial surveillance imaging over two years had demonstrated a radiographically normal appendix prior to the obstructive presentation. Immunohistochemical and histopathologic features confirmed a metachronous primary malignancy rather than metastatic recurrence. This case exemplifies the diagnostic challenge posed by occult appendiceal neopl

Drug approach to treatment for obesity in adults: perspectives and considerations.

Obesity is considered a chronic disease and a serious public-health issue in Spain, as well as in many other countries, primarily because of it being a significant risk factor for multiple chronic conditions, including diabetes mellitus type 2, cardiovascular disease, respiratory disease, metabolic dysfunction-associated fatty liver disease, gastrointestinal conditions (gastroesophageal reflux, diverticulosis), musculoskeletal difficulties, infertility, and even development of some malignancies such as postmenopausal breast cancer, and colorectal, endometrial, renal, esophageal, pancreatic, he

Signet ring cell carcinoma of the rectum: atypical metastatic presentation.

Colorectal cancer is the third most common cancer in the world and the fourth most common cause of death related to cancer. Signet ring cell carcinoma represents an uncommon histological type for rectal cancer with less than 1% of all rectal neoplasms. It usually behaves aggressively and has an inferior prognosis. We present the case of a young man diagnosed with signet ring cell rectal carcinoma. He underwent neoadjuvant therapy with partial response, had surgery with curative intent and showed local recurrence after only 3 months. Disease progression happened only weeks after recurrence with

Complications after abdominal surgery and breast implant pelvic positioning: case report and literature review.

The recommended treatment for patients with locally advanced colorectal cancer (CRC) or other pelvic neoplasms often comprises extensive abdominal surgery and radiation therapy (RT). The major complications of these treatments are radiation enteritis and empty pelvis syndrome, which is the displacement of bowel loops in the pelvic dead space created after the intervention. To avoid these complications, diverse methods of pelvic floor reconstruction have been attempted, one of them being the silicon breast prosthesis pelvic placement. Since literature is scarce on complications secondary to bre
