[Confusions and reflections on the diagnosis and treatment model of differentiated thyroid cancer].

Differentiated thyroid cancer (DTC), the most common subtype of thyroid cancer, exhibits marked regional variations in epidemiology worldwide, with management strategies evolving in response to evidence-based advances in diagnosticsand therapeutic protocols. This article synthesizes global and China-specific epidemiological patterns in DTC, evaluates temporal trends in disease-specific survival across populations, and reviews the evolution of evidence-based guidelines. Building on emerging clinical evidence, it analyzes the rationale for therapeutic de-escalation and critically assesses its imp

Shifting definitions in dentistry: a source of clinical confusion. A critical review.

OBJECTIVE: Definitions play a pivotal role in scientific disciplines, including dentistry, by providing clarity and precision to the termino … cies in the meanings and connotations of clinically significant terms. This can potentially lead to confusion among individuals who have been familiar with the original definition for an extended period. These changes have the potential to expose patients to various risks and may ultimately call into question the scientific basis of dentistry as a disciplin …

Spiritual confusion in the era of artificial intelligence: a psychology of religion perspective.

The rise of artificial intelligence (AI) presents unprecedented challenges to traditional religious understanding and practice, especially in terms of religious experience and psychological behavioural patterns. This study examines the various dimensions of spiritual confusion arising from AI integration across diverse religious traditions, employing a psychology of religion framework alongside Western and Eastern philosophical spectrums, enriched by psychobiographical analyses of religious practitioners' developmental trajectories in the context of the Fourth Industrial Revolution (4IR), with…

Osteomesopyknosis: A Bone-Sclerosing Disorder that May be Confused with Bone Metastases.

Sclerosing bone disorders encompass a range of genetic and acquired diseases. The potential for bone metastases is often a significant concern, especially when multiple discrete lesions are present. Several non-malignant disorders can also produce similar patterns of bone abnormalities. Among these is osteomesopyknosis, a rare condition characterized by multiple sclerotic lesions in the axial skeleton. Only a few cases of this presumably genetic disease have been documented. In this report, we present a new case and review previously published cases to enhance understanding and facilitate reco

[On 'confused persons' and misunderstood behaviour: leads for health and safety services].

BACKGROUND: For some ten years now the term ‘confused persons’ has gained prominence in the Netherlands. Introduced by the police, it gathered attention due to rising numbers of police calls, to become entangled with broader sentiments of unsafety due to serious incidents with people with mental disorders. Mental health services have struggled with the term, since it is no diagnosis and it often leads to misunderstandings and controversies. AIM: To explain 1. the origins of the term, 2. the numbers, 3. the impact on police, juicidal system and society, 4. possible explanations for

Confused subcutaneous nodules in children: Differential diagnosis of pilomatricoma in children.

BACKGROUND: Pilomatricoma is a common but easily misdiagnosed tumor in children. AIMS: To differentiate pilomatricoma from other common subcutaneous nodules in children. PATIENTS/METHODS: Misdiagnosed subcutaneous nodules in four children were recorded. RESULTS: A red mass on a 7-year-old boy's head which had been misdiagnosed pyogenic granuloma was proved to be pilomatricoma. A red mass on an 8-month-old boy's face which had been misdiagnosed infantile hemangioma also turned to be pilomotricoma. A red mass on a 21-month-old girl's breast, which had been misdiagnosed pilomatricoma, was prov

Clinical Misdiagnosis of COVID-19 Infection with Confusing Clinical Course.

BACKGROUND: Similarities in the febrile course and other manifestations of some diseases may lead to clinical misdiagnosis of COVID-19 infection. Here, we report a case in a young child with a potentially confusing clinical course. Case Presentation. A 29-month-old boy presented with a 2-month history of fever. His PCR test for COVID-19 was positive, and there was pleural effusion plus positive findings in the lower left lobe of the lung on computed tomography scan. Mid-sized splenomegaly was found on abdominal ultrasound, and laboratory tests disclosed pancytopenia. In light of the atypical l

Central hypothyroidism or subclinical hyperthyroidism: can they be confused with each other?

SUMMARY: Isolated, adult-onset central hypothyroidism is very rare, and its diagnosis can be challenging. A 42-year-old patient was referred for evaluation of a 2.8 cm thyroid nodule. She referred symptoms that could be attributed to hypothyroidism and thyroid tests showed low TSH and normal-low levels of free T4. However, evaluation of the remaining pituitary hormones and pituitary MRI were normal, yet a radionuclide scanning revealed that the thyroid nodule was 'hot' and the tracer uptake in the remaining thyroid tissue was suppressed. Interpretation of these studies led to a misdiagnosis of

Impaired Motor Learning in a Disorder of the Inferior Olive: Is the Cerebellum Confused?

An attractive hypothesis about how the brain learns to keep its motor commands accurate is centered on the idea that the cerebellar cortex associates error signals carried by climbing fibers with simultaneous activity in parallel fibers. Motor learning can be impaired if the error signals are not transmitted, are incorrect, or are misinterpreted by the cerebellar cortex. Learning might also be impaired if the brain is overwhelmed with a sustained barrage of meaningless information unrelated to simultaneously appearing error signals about incorrect performance. We test this concept in subjects

Primary hyperparathyroidism-jaw tumor syndrome: a confusing and forgotten diagnosis.

BACKGROUND: Primary hyperparathyroidism is caused by the excessive growth of parathormone secretion, its consequence being hypercalcemia. The parathyroid adenoma is responsible for over half of primary hyperparathyroidism cases. The mandibular tumor can be the initial sign in the case of primary hyperparathyroidism. CASE PRESENTATION: We present the case of a 33 year old patient with history of a mandibular operated tumor, repetitive pathological fractures and hypercalcemia manifestations. The level of the parathormone at the first measurement indicated a very high value. The parathyroid scin

[Confusion Over the Term "Contamination Rate" as It Pertains to Blood Cultures].

The blood culture contamination rate is often used to validate specimen-collection procedures. CUMITECH has set its optimal target to be 2% to 3%. However, the term "contamination rate" has been defined in many ways, limiting its generalizability. The definitions used in earlier studies can be divided into two categories; definitions based on clinical judgements, and those based on preset rules. According to each principle, the equation must be composed of a defined numerator and denominator. The problem with clinical definitions is that the decision is inevitably subjective, and the process i

A Re-Examination of the History of Etiologic Confusion between Dengue and Chikungunya.

Contrary to the perception of many researchers that the recent invasion of chikungunya (CHIK) in the Western Hemisphere marked the first episode in history, a recent publication reminded them that CHIK had prevailed in the West Indies and southern regions of the United States from 1827-1828 under the guise of "dengue" (DEN), and that many old outbreaks of so-called "dengue" actually represented the CHIK cases erroneously identified as "dengue." In hindsight, this confusion was unavoidable, given that the syndromes of the two diseases-transmitted by the same mosquito vector in urban areas-are v…

Aggressive aneurysmal bone cyst of the maxilla confused with telangiectatic osteosarcoma.

Aneurysmal bone cyst (ABC) is a benign, expansile lesion typically affecting the long bones and vertebrae of patients younger than 20 years. Approximately 2% of ABCs occur in the head and neck region, most commonly affecting the mandible. Although the most common co-existing lesion associated with ABCs is the giant cell tumor, ABCs can be radiologically confused with telangiectatic osteosarcoma in cases of aggressive behavior and rapid growth. Here, we report a case of an aggressive ABC of the maxilla confused with telangiectatic osteosarcoma in a patient who underwent several operations for a

Temporal lobe epilepsy confused with panic disorder: implications for treatment.

A 34-year-old woman with a 9-year history of unprovoked attacks of anxiety and dyspnea associated with symptoms of depersonalization and derealization is presented. The attacks increased in frequency and were associated with internal derogatory voices, vivid frightening imagery, and suicidal ideation, leading to 3 emergency psychiatric hospitalizations in a period of less than 3 months. She had been treated unsuccessfully for a presumptive diagnosis of panic disorder without agoraphobia, prompting a reconsideration of this diagnosis. Although electroencephalography and magnetic resonance imagi

Sacroiliitis infected with group A streptococcus in a child presenting with confusion and combativeness.

Group A streptococcus-associated severe invasive infection (streptococcal toxic shock syndrome) has been described. Streptococcal toxic shock syndrome occurs when the infecting strain of group A streptococcus produces superantigens. Confusion and combativeness are well known as the common symptoms of streptococcal toxic shock syndrome. We encountered a child who suffered from pyogenic sacroiliitis, with confusion and combativeness. Group A streptococcus was isolated from the patient's blood culture. However, his disease did not fulfill the criteria of streptococcal toxic shock syndrome. Pyogen…
