452 previously healthy children aged over six months and under six years were admitted to the Sheffield Children's Hospital between January, 1972, and December, 1976, after their first convulsion associated with fever. 304 of the children had a lumbar puncture. 25 punctures were necessary according to defined clinical criteria--including those in 15 children who were subsequently shown to have meningitis. All 148 children who did not have a lumbar puncture made a prompt and uneventful recovery. Children with convulsions associated with fever should be referred to hospital. A lumbar puncture sh

The causes and short-term prognoses of neonatal convulsions in infants less than four weeks of age were studied in 77 full-term infants born in Stockholm in 1970--1976. In half of the infants (48%), hypoxia was considered to be the probable main etiology, while infection and metabolic disease including hypoglycemia and hypocalcemia were the next commonest cause, 12% for each condition. The etiology was unknown in 29% of the infants although 15 of those 22 included in this group had other additional diagnoses. The total mortality was 13%. At one year of age, 19 of the surviving 64 infants (30%)

A 6-month-old boy, having suffered repeated convulsive siezures since birth, died of severe epilepsy, the disease remaining unknown. However, when a brother, born after the death of the present case, displayed the same clinical symptoms and was diagnosed clinically as having pyridoxine dependent convulsion, the deceased became suspected of having been a case of familial pyridoxine dependency. Pathologically, bilateral cerebellar hemorrhages were found accompanied by subdural and subarachnoidal hemorrhages. Histologically, neuronal loss and diffuse astrocyte hyperplasia were found in the brain:

Three single CSF proteins with different molecular size (albumin, immunoglobulin G, and alpha2-macroglobulin) were determined by the method of electroimmunoassay in 61 children with febrile convulsions (FC) in order to evaluate the permeability of the blood-CSF barrier (B-CSF-B). Forty-two children with acute extracerebral infection served as controls. In contrast to a group of 22 children who suffered from acute meningoencephalitis or encephalitis, the CSF values of 48 children with FC were within normal limits. Thus even a very mild form of inflammatory encephalopathy-undetectable with conve

Convulsions have complicated the surgical management of life-threatening inflammatory bowel disease in five patients. These patients experienced a transient, generalized convulsive disorder and toxic ambylopia. Results of neurological examination were unremarkable. Treatment included metabolic stabilization along with the administration of oxygen and anticonvulsive medication. Various factors, such as hypoxia, fluid overload, cerebral edema, steroid administration, and deficits in calcium, magnesium, and potassium, may have contributed to the cause of this complication.

This longitudinal study included 113 patients seen through a period of 8 years with febrile convulsive crises, especially of the tonoclonic type, with predominance in males and with average age at the onset of the disease of 6 months. Infectious respiratory processes were considered as the main cause of fever and the drug given most often was sodium diphenylhydantoin. The value of EEG in febrile convulsive crises was confirmed since the first graphic traces were abnormal in 95% of cases and that generalized paroxysmal or focal alterations were considered suggestive of epilepsy and or organic d

An 18-year-old primipara developed acute hypertension leading to cerebral edema and convulsions following the IV injection of a bolus of 10 units of oxytocin with 0.2 mg methylergonovine maleate. Oxytocin in a dose of more than 2 units should not be administered IV in a single injection, as severe hypotension may result. If oxytocin is required, it can be injected either IM, or by IV pump or drip. The use of ergot in obstetrics should be limited to the treatment of life-threatening postpartum hemorrhage and be given only by the IM route. Ergot should not be administered to patients with cardia

The relationships of certain etiological factors (sex, age, family history of febrile convulsions or epilepsy, term, birth weight, prenatal or perinatal anomalies, temperature, cause of fever) to the duration and localization of the first febrile convulsion (FC) were studied in 402 patients. In patients with prolonged seizures (over 30 min), the mean age was younger, the proportion of girls, common infectious diseases of childhood, and immunization was higher and that of respiratory infections lower than in patients with brief convulsions. In patients with unilateral seizures, the proportion o
