[Angioma of the Gasserian ganglion. Morphological and histogenetic considerations on a case].

A cavernous haemangioma of the Gasserian ganglion in a 37-year-old man is presented. It is the third reported case of cavernoma in this region, where infrequently tumors develop. Carotid angiogram shows a network of neoformed vessels in the middle cranial fossa. The hypotheses about the malformative origin of this tumor and the differential diagnosis toward other highly vascularized meningeal tumors are discussed.

[Arachnoid cyst of cerebellar-pontine angle clinically mimicking Meniere's disease].

Arachnoid cysts represent about 1% of all intracranial space-occupying lesions. Usually they are located in the middle cranial fossa while their localization in the posterior fossa is uncommon and generally remain asymptomatic or those are non-specific. The case of a 51 years old female is reported who was diagnosed by IRM of a 4,5 x 2 cm arachnoid cyst, situated on the left cerebello-pontine angle, with tinnitus, hearing loss and vertigo that mimicked a Meniere's attack. We think thees benign tumors must be included in the differential diagnosis of Meniere's disease because they can be indist

Management of spontaneous temporal bone cerebrospinal fluid leak: A 30-year experience.

PURPOSE: The goal of this study was to describe the incidence of and our long-term results in the management of spontaneous temporal bone CSF leak and to determine if there is any association with an increase in body mass index (BMI). METHODS: This was an Institutional Review Board (IRB) approved retrospective review of 58 patients who underwent repair of spontaneous temporal bone CSF leaks by a single surgeon at a tertiary care academic medical center between 1988 and 2017. Patients with CSF leaks due to trauma, iatrogenic injury, or chronic infection were excluded. Surgical approaches inclu

Superior Semicircular Canal Dehiscence in a Patient with Ehlers-Danlos Syndrome: A Case Report.

Superior semicircular canal dehiscence (SSCD) is a bony defect in the middle cranial fossa floor that results in an abnormal connection between the inner ear and cranial vault. Although the etiology of SSCD remains unclear, an inappropriately thin or fragile temporal bone likely predisposes an individual towards developing SSCD. Ehlers-Danlos syndrome (EDS) constitutes a group of genetic connective tissue disorders caused by a defect in the production, processing, or structure of collagen, or its associated proteins. The possible association between SSCD and EDS has not been previously describ

New window for cochlear implant insertion.

Nine patients with profound bilateral hearing loss received a cochlear implant via the middle fossa. Subjects presented with a bilateral radical mastoidectomy cavity, fibroadhesive otitis media, otosclerosis, autoimmune inner ear disease, previous cranial trauma and genetic prelingual deafness. A classic middle fossa approach was adopted. A small cochleostomy was performed on the most superficial part of the basal turn. A Nucleus CI24M cochlear implant system (Cochlear Corporation) was inserted in four patients, a Lauraflex implant (Philips Hearing Implants) in three patients and a Combi 40 +

Exploring optimal microscopic keyhole access to the skull base: an anatomical evaluation of transciliary supraorbital and transpalpebral orbitofrontal craniotomy approaches.

PURPOSE: The past two decades have witnessed the rise of keyhole microscopic minimally invasive surgeries, including the transciliary supraorbital approach (TCA) and transpalpebral approach (TPA), commonly known as the transorbital approach. This study aims to elucidate the nuances, specific indications, and advantages of each approach. METHODS: A series of dissections were conducted on five formalin-fixed, alcohol-preserved cadaver heads. The TCA was performed on one side, and the TPA on the other. Virtual measurements of working angles for both approaches were recorded. Additionally, three

Intraosseous dural arteriovenous fistula of the skull base associated with hearing loss. Case report.

The most common clinical presentations of dural arteriovenous fistulas (DAVFs) are bruit, headache, increased intracranial pressure, and intracranial hemorrhage. In particular locations, such as the cavernous sinus or middle cranial fossa, cranial nerve involvement due to dural arterial steal or venous occlusion may develop. A case in which a DAVF is associated with hearing loss, however, has not previously been reported. The authors report a case in which an intraosseous DAVF and associated hearing loss probably resulted from cochlear nerve or vascular compression caused by the draining vein

Acoustic neuroma: nursing implications related to surgical management.

Acoustic neuromas are benign tumors originating from the Schwann cells of the eighth cranial nerve. Symptoms occur related to compression and stretching of cranial nerves which alters their function. Surgical management is by middle fossa, translabyrinthine, suboccipital or combined approach craniotomy. Prevention and early recognition of postoperative complications are important aspects of nursing care for these clients. A holistic approach to client and family nursing needs is presented. Discharge planning should include assessment of client learning and home care needs. The Acoustic Neuroma

Trans-orbital intra-cranial air gun injury.

PURPOSE: To describe a case of trans-orbital intra-cranial air gun injury with a discussion on air gun related morbidity and mortality. METHODS: Case report and literature review. RESULTS: The air gun pellet travelled through the orbit without penetrating the globe. It passed into the middle cranial fossa through the superior orbital fissure and lodged in the temporal lobe. The patient was managed conservatively with antibiotics and antiepilectis. CONCLUSIONS: Air gun design has changed in recent years resulting in an increased morbidity and mortality. New approach for cochlear implantation: cochleostomy through the middle fossa.

The middle fossa approach was used in 11 patients with profound bilateral hearing loss for insertion of a cochlear implant. Fibroadhesive otitis media (n = 1), bilateral cavity radical mastoidectomy (n = 1), autoimmune inner ear disease (n = 2), previous cranial trauma (n = 1), genetic prelingual deafness (n = 5), and otosclerosis (n = 1) were the causes of deafness. A cochleostomy was performed on the most superficial part of the basal turn, and the electrode array was inserted up to the cochlear apex. Speech perception tests (1-9 months after cochlear implant activation) yielded better resul

360° around the orbit: key surgical anatomy of the microsurgical and endoscopic cranio-orbital and orbitocranial approaches.

OBJECTIVE: Several pathologies either invade or arise within the orbit. These include meningiomas, schwannomas, and cavernous hemangiomas among others. Although several studies describing various approaches to the orbit are available, no study describes all cranio-orbital and orbitocranial approaches with clear, surgically oriented anatomical descriptions. As such, this study aimed to provide a comprehensive guide to the microsurgical and endoscopic approaches to and through the orbit. METHODS: Six formalin-fixed, latex-injected cadaveric head specimens were dissected in the surgical anatomy

Endonasal endoscopic management of a large meningocephalocele in a patient with concomitant middle skull base defect.

The presence of a skull base defect can lead to major complications such as cerebrospinal fluid leak, meningocele, encephalocele and meningitis. It is exceptional to find the existence of two concomitant defects in the skull base. We present the case of a patient with concomitant spontaneous defects of the anterior and middle skull base that were surgically repaired. After 18 years of right rhinorrhea the patient was referred after being diagnosed with a large right nasal fossa meningoencephalocele, which was surgically removed by functional endoscopic sinus surgery. Following the surgery the

Studies on the area cerebrovasculosa of anencephalic fetuses.

The area cerebrovasculosa has been examined in 35 anencephalic fetuses and coronal sections prepared in 15 cases. A bilaterally symmetrical cystic mass is found covering the anterior and middle cranial fossae. The mass is generally walled with glial tissue, partially lined with ependyma and filled with abnormally vascular choroid plexus. The coverings of the cyst are conspicuously vascular and the overlying squamous epithelium is in continuity with hairy skin at the margins of the lesion. The posterior cranial fossa contains a variable quantity of medulla oblongata which has no connection with

Risk Factors for Delayed Surgical Recovery and Massive Bleeding in Skull Base Surgery.

BACKGROUND: To determine factors that delay surgical recovery and increase intraoperative hemorrhage in skull base surgery. METHODS: Factors related to delayed postoperative recovery were retrospectively reviewed in 33 patients who underwent open skull base surgery. Early and late recovery phases were assessed as "days required to walk around the ward (DWW)" and "length of hospital stay (LHS)," respectively. Intraoperative blood loss was cal-culated every hour and analyzed in 4 steps, i.e., craniotomy and intracranial manipulation, cranial fossa osteotomy, extracranial osteotomy, and reconstr

Successful endoscopic endonasal surgery for very huge trigeminal schwannomas in nasopharynx.

We present a 60-year-old female diagnosed with a giant trigeminal tumor (5.2*6.4*8.2 cm) situated in the middle cranial fossa and nasopharyngeal area. The patient was operated on by endoscopic endonasal transmaxillary, transpterygoid and infratemporal approaches. Postoperatively she was stable, with no neurologic complication and no cerebrospinal fluid leakage. We review the literature on extremely large trigeminal schwannomas.

Persistent stapedial artery: CT findings.

CT of a patient with conductive hearing loss showed a small vascular structure leaving the carotid canal and passing through the stapes to join the facial nerve canal. The small bony channel paralleled the anterior tympanic segment of the facial nerve, continuing beyond the geniculate ganglion into the middle cranial fossa. Otoendoscopic photography documented the presence of a persistent stapedial artery.

Anatomical insights: meningo-orbital foramen and its clinical relevance.

OBJECTIVES: The meningo-orbital foramen (MOF) is an inconsistently present bony canal that connects the orbit with the cranial cavity, commonly the middle cranial fossa. It may transmit the meningolacrimal artery supplying orbital contents, making it clinically relevant in procedures involving the lateral orbital wall. This study aimed to investigate the morphometry and anatomical positioning of the MOF and discuss its surgical implications. METHODS AND ANALYSIS: A total of 127 adult dry human skulls (254 orbits) were examined. Gender was estimated morphometrically, while ethnicity was unknow
