The dusky pigmy rattlesnake (Sistrurus miliarius barbouri) is a pit viper (Crotalidae family) along with diamondback rattlesnakes, water moccasins, and copperheads. Although it is a small snake, it is responsible for envenomations requiring hospitalization. We present the case of a 54-year-old man who was bitten in the left index finger with onset of coagulopathy successfully treated with crotalidae polyvalent immune fab (CPIF) antivenom.

He has bite marks and pain on his rapidly swelling middle finger of his right hand. Our initial treatment is immobilization of the patient. Snakebite victims have been described previously in orthopedic literature in regard to complications such as compartment syndrome and carpal tunnel syndrome. We introduce a previously unreported complication of stenosing flexor tenosynovitis in a patient bitten by a rattlesnake. After being bitten in her right forearm, the patient experienced mild systemic symptoms of fever and nausea and was assessed at an outside hospital, where it was determined that she did not suffer from envenomation and therefore did not require antivenin therapy. A retrospective study was done of poisonous snakebite in Utah to determine the current epidemiology and scope of treatment, reviewing emergency department logs and other sources statewide for a 69-month period. Of 61 cases of poisonous snakebite identified, 13 occurred in snake hobbyists or venom laboratory personnel and were considered nonaccidental, and 48 were inflicted by native noncaptive snakes. This report is a retrospective study of the epidemiology of snakebite cases recorded from 2007 to 2014 in the Rio Grande do Norte State, northeastern Brazil. Data was collected from the Injury Notification Information System database of the Health Department of Rio Grande do Norte. A total of 3,019 cases were studied. Cases were distributed over all months of the period studied and occurred mainly in rural areas.
