Immediate Hypersensitivity Reaction to Chemotherapeutic Agents.

An increase by 70% in new cases of cancer is expected over the next 2 decades, with more than 8 million people dying each year from the disease, an estimated 13% of all deaths worldwide. Several new chemotherapeutic agents have been rapidly developed and many of them approved through a fast-track process. In parallel, adverse reactions to chemotherapy have increased dramatically worldwide, frequently preventing the use of first-line therapy for cancer and jeopardizing patients' treatment outcomes. The present review focuses on immediate hypersensitivity reactions (IHRs) induced by chemotherape

Hypersensitivity and immunologic reactions to biologics: opportunities for the allergist.

OBJECTIVE: There has been a great expanse in the use of biological agents during the past decade. However, there are significant differences between biologics and typical pharmaceutical drugs. This review focuses on 3 separate types of adverse reactions to biologics, namely high cytokine reactions, hypersensitivity reactions, and secondary immunodeficiency. DATA SOURCES: A PubMed literature search restricted to the previous 10 years using combinations of search terms, including omalizumab, rituximab, TGN1412, biologic agent, anaphylaxis, hypogammaglobulinemia, desensitization, and cytokine st

Diagnosis of eosinophilic esophagitis in an infant undergoing milk oral immunotherapy - a case report.

Although the standard of care for cow's milk (CM) allergy is strict food avoidance, oral immunotherapy (OIT) is being widely investigated as an alternative management option in certain cases. Immediate adverse reactions to OIT have been described, but its long-term effects are much less often reported. We present the case of a girl diagnosed with IgE-mediated CM allergy that was proposed for our CM OIT protocol at the age of 3 years. The first sessions (dose escalation up to 5 ml) were well tolerated, however eight hours after her daily morning dose of 5 ml CM the child developed late episodes

Factors influencing the response to specific immunotherapy for asthma in children aged 5-16 years.

BACKGROUND: The aim of this study was to evaluate the factors predicting the response to allergen-specific immunotherapy (ASIT) in children with asthma. METHODS: The case notes of children with asthma who received ASIT for 2 years were retrospectively reviewed. The cases were then divided into an effective clinical response group, defined as absence of asthma symptoms without requirement for medication for at least 6 months during follow up; and an ineffective clinical response group. At the time of initiating treatment, blood was collected for analysis of serum total IgE. Family history of a

[Nut allergy - a difficult problem for the clinician].

Nuts belong to the most significant causes of food anaphylaxis in Finland. Diagnosis of nut allergy is complicated by the fact that for those having birch allergy, skin prick tests and serum tests yield a positive reaction for peanut and hazelnut without the nut causing the allergy reactions. For fear of anaphylaxis, avoidance of nuts on the basis of conventional tests measuring allergic sensitization leads to an unnecessary therapeutic diet. Attempts must be made to recognize patients for whom the ingestion of even minute doses of nuts may be life-threatening. For patients having severe sympt

Successful desensitization to imiglucerase of an adult patient diagnosed with type I Gaucher disease.

Gaucher disease is the most common lysosomal storage disorder, and enzyme replacement therapy, such as administration of imiglucerase, is the standard therapy. Anaphylaxis to imiglucerase is rarely reported. Here, we report a 26-year-old female who was diagnosed with type 1 Gaucher disease and referred to our Allergy Outpatient Clinic because of an anaphylactic reaction due to imiglucerase enzyme therapy. A desensitization protocol was administered with two different dilutions with an increasing rate of administration delivered in 10 consecutive steps by intravenous infusion in an intensive ca

Cardiovascular prophylaxis and aspirin "allergy".

Aspirin is an important antiplatelet agent in the treatment of cardiovascular disease. Aspirin "allergy" often directs the physician away from this potentially life-saving modality. The majority of patients with a history of "reactions to aspirin" have aspirin/nonsteroidal anti-inflammatory drug (NSAID)-induced gastritis, easy bruisability, or other side effects. The minority of these patients has a "true allergy," referred to as a hypersensitivity reaction. The former group can be started on aspirin without the need for special challenge. Adding a proton-pump inhibitor can often mitigate the

Insect venoms.

Insect venoms applied by stings of social Hymenoptera, like honey bees, vespids or ants are -together with foods and drugs - the most frequent elicitors of anaphylaxis in humans. Besides taxonomy, the biology of the responsible social Hymenoptera is important: guidelines based upon its knowledge allow to reduce the risk of further stings in patients with a history of venom anaphylaxis. Epidemiology of venom anaphylaxis has special aspects with regard to prevalence, fatality and natural history. An estimated 200 individuals die every year in Europe from anaphylaxis following Hymenoptera stings.

Future perspectives in the treatment of atopic dermatitis.

Atopic dermatitis is a chronically relapsing inflammatory skin disease with an increasing prevalence. The guidelines for the treatment of atopic dermatitis enable us to manage many patients with atopic dermatitis under a global consensus, which also contributes to improving their quality of life. However, there remain some patients with atopic dermatitis whose symptoms cannot be satisfactorily controlled using the therapeutic management recommended by the guidelines. Recent genetic, immunological and physiological insights into the pathomechanisms of atopic dermatitis are expected to overcome

Selection of aspirin dosages for aspirin desensitization treatment in patients with aspirin-exacerbated respiratory disease.

BACKGROUND: Aspirin desensitization followed by daily aspirin therapy is effective add-on treatment for patients with aspirin-exacerbated respiratory disease. Prior studies used 650 mg of aspirin twice daily, but studies at lower dosages were inconclusive. OBJECTIVE: We sought to determine the optimal daily dosage of aspirin treatment. METHODS: We studied 137 patients who had undergone successful aspirin desensitization and randomized them into 2 groups, 650 mg twice daily versus 325 mg twice daily. After 1 month, patients either increased or decreased their dosage based on their symptom con

Intradermal testing and sublingual desensitization for nickel.

Thirty-nine patients with nickel allergy as diagnosed by results of clinical history and intradermal testing with nickel sulfate were treated by sublingual hyposensitization. Intradermal testing was accurate and titration showed the degree of sensitivity. The immunologic principle of oral tolerance was used in treatment. Eighty-five percent of the thirty-nine patients showed subjective improvement in their dermatitis and all showed objective evidence of decreased intradermal sensitivity. None of the patients' conditions worsened. The use of oral treatment with nickel sulfate deserves broader c

Trimethoprim/sulfamethoxazole desensitization.

Although alternative therapy for PCP remains limited, the role of TMP/SMX desensitization becomes increasingly important in patients with AIDS. Various successful desensitization protocols have been described in this article. As there are no established guidelines, it appears that the desensitization procedure can occur in small successive doses given each day or one small dose given daily. An evaluation of the severity of allergic reaction can be used to determine the type of dosing regimen. We believe that protocols starting with low doses and slow titration to full-dose therapy, as used at

Pulmonary infiltration and eosinophilia associated with sulfasalazine therapy for ulcerative colitis: a case report and review of literature.

We report a 52-yr-old man with ulcerative colitis who developed sulfasalazine-induced pulmonary infiltration with eosinophilia (PIE syndrome), which resolved completely after withdrawal of this drug. Desensitization to sulfasalazine was successful, and allowed the patient to receive this drug without recurrence of the pulmonary toxicity. This is the first case of the sulfasalazine-induced PIE syndrome in Japan; a review of the world literature found no previous cases of successful desensitization following sulfasalazine-induced PIE syndrome.

Soluble copolymers of yellow jacket, yellow hornet and white faced hornet with human albumin for venom immunotherapy.

Soluble polymers have been prepared by polymerizing human serum albumin (HSA) with either yellow jacket venom (YJV), yellow hornet venom (YHV) or white faced hornet venom (WFHV). The venom-albumin polymer preparations were fractionated on Sephacryl S-300 to have a molecular weight range higher than catalase (2.4 x 10(5) daltons). Rabbits were immunized with either yellow jacket venom-albumin polymer (YJVAP), yellow hornet venom-albumin polymer (YHVAP) or white faced hornet venom-albumin polymer (WFHVAP). Rabbits immunized with venom-albumin polymer (VAP) produced IgG antibody to VAP and venom,
