Diazepam is a drug commonly used for sedation in surgery done under local anesthesia. Since its primary metabolites are excreted in the urine, its safety for use in functionally anephric patients has not been determined. This report describes the results of diazepam use in chronic hemodialysis patients undergoing angioaccess procedures. Thirty consecutive angioaccess procedures with an average duration of 1 hour and 27 minutes were done in 22 different patients using an average of 25 mg diazepam per patient. Rats were trained to discriminate between saline and either 0.3, 1.0, 3.0 or 6.0 mg/kg of diazepam in a two-choice, discrete-trial avoidance procedure. Parents with children who had presented with a simple febrile convulsion were advised to give their children rectal diazepam, in case of fever. The results (21 families with an average follow-up of 2 years) were compared with those in two groups of controls. The efficacy and inocuousness of prophylactic treatment are remarkable. However, its indication should be more precisely stated, after a better information of families and practitioners, since the expected effect on familial apprehension was not satisfactorily obtained.

Binding of diazepam to the blood fractions (erythrocytes and plasma proteins) in man, rat and mouse was studied. Only little dependence of binding on total drug concentration was found. We have studied the influence of intravenously administered diazepam on the horizontal (HVOR) and vertical (VVOR) vestibulo-ocular reflexes of the rabbit. The HVOR and VVOR were evoked by sinusoidal oscillation of rabbits on a rate table (0.01 to 0.8 Hz, +/- 10 degrees), and eye movements were measured with an infrared light-projection technique. The compatibility and stability of diazepam injection were studied following dilution to 10 different concentrations in dextrose 5% in water, normal saline, Ringer's injection and lactated Ringer's injection. Diazepam is separated from the bulk of the biological material by adsorption at Amberlite XAD-2 and subsequent desorption with ethyl acetate. Plasma levels of diazepam and N-desmethyldiazepam were investigated in 19 children by a gas chromatographic method permitting the use of capillary sample.
