[Dissecting aortic aneurysm at Marfan syndrome--case report].

The authors present the case report of a Marfan syndrome patient with aneurysm of ascending, dissecting aneurysm of descending and abdominal aorta. Ascending aorta replacement with aortic valve sparing procedure was performed at first surgery. Later on descending and abdominal aorta were replaced. The patient was reoperated because of a chylus collection in retroperitoneal space. Three months following surgery the patient is completely recovered.

[A case of total aortic replacement in a patient with Marfan's syndrome].

A 32-year-old male with Marfan's syndrome was admitted to our Department in June, 1989 due to severe back pain that was diagnosed as type III-b dissecting aneurysm. His infrarenal abdominal aorta and bilateral iliac arteries had been replaced in another clinic in 1984 due to true aortic aneurysm. Two years later, Carbrol's operation was performed in our Department for the late development of DeBakey type II dissecting aneurysm. DSA and CT studies indicated the need for total replacement of the entire remaining aorta, since diffuse and extensive dilatation of the false lumen and the presence of

Bilateral Renal Artery Thrombosis With Distal Abdominal Aortic Dissection as the First Presentation of Takayasu Arteritis.

BACKGROUND: Takayasu arteritis (TAK) is a chronic granulomatous large-vessel vasculitis that predominantly affects the aorta and its major branches, leading to stenosis, occlusion, or aneurysmal changes. Renal artery disease is common, but acute bilateral renal artery thrombosis causing renal dysfunction is rare. CASE PRESENTATION: A 38-year-old South Asian male presented with a two-week history of abrupt-onset bilateral loin pain and severe azotemia (creatinine 8.25 mg/dL). Inflammatory markers were high (ESR 102 mm/h; CRP 45 mg/L). Ultrasound showed preserved renal size; Doppler suggested g

Totally laparoscopic spleen-preserving splenic hilum lymph nodes dissection in radical total gastrectomy: an omnibearing method.

OBJECTIVE: To investigate the feasibility and safety of laparoscopic spleen-preserving splenic hilum lymph nodes (LNs) dissection for advanced proximal gastric cancer using an omnibearing method. METHODS: Between August 2013 and December 2014, 16 patients with advanced proximal gastric cancer treated in Guangdong Province Hospital of Chinese Medicine, were enrolled and subsequently underwent laparoscopic radical total gastrectomy (TG) with spleen-preserving splenic hilum LNs dissection. During dissecting Nos. 10 and 11 LNs, we divided them into two parts, namely LNs anterosuperior and posteri

Laparoscopic Radical Extended Right Hemicolectomy Using a Caudal-to-Cranial Approach.

BACKGROUND: Due to the emphasis of oncologic principle, a medial-to-lateral approach for laparoscopic right hemicolectomy was recommended.1 (,) 2 This approach, however, is technically challenging and involves several limitations with overweight patients, whose mesocolon may be too thick for identification of the vessel landmarks. Moreover, it is difficult for inexperienced surgeons to enter the retroperitoneum space accurately. This report describes a caudal-to-cranial approach for laparoscopic radical extended right hemicolectomy. METHODS: First, a "yellow-white borderline" between the righ

Endovascular treatment of spontaneous isolated common and external iliac artery dissections with preservation of pelvic blood flow.

Spontaneous isolated dissection of iliac arteries is very rare, with few reports in the literature. Medical, surgical, and endovascular treatment modalities have all been used to manage iliac artery dissections. We report a case of symptomatic, isolated, spontaneous dissection of the common iliac and external iliac arteries. Both dissections were successfully treated by separate percutaneous stent-graft placement, preserving hypogastric artery flow. This technique is interesting because it provides adequate sealing of proximal and distal dissection sites while preserving hypogastric artery and

A Modified Technique of Laparoscopic Lateral Lymph Node Dissection Combining Fascia-Oriented Dissection and Routine Upfront Distal Visceral Vessels Ligation for Mid- to Low-Lying Rectal Cancer.

INTRODUCTION: Lateral pelvic recurrence can be a cause of local failure after surgery for low rectal cancer. Lateral lymph node dissection is often performed in East Asia for patients with enlarged lateral lymph nodes or because of the presence of risk factors. However, the outcomes of the conventional lateral lymph node dissection are unsatisfactory, with a considerably high local recurrence rate for patients with positive lateral nodes. Here, we introduce a modified technique to improve lateral nodes clearance. TECHNIQUE: This modified technique has 4 key steps: 1) separation of the uretero

Successful management of a giant dissecting aortic aneurysm in a patient with Behcet's disease.

Thoracic aortic aneurysm with subsequent dissection is a very rare but dangerous complication of Behcet's disease, which is a systemic syndrome that can involve blood vessels of all sizes. Surgical experience in dealing with this complication in patients with Behcet's disease has been limited. We report a 20-year-old patient with Behcet's disease who presented with a giant dissecting thoracic aortic aneurysm. The patient was successfully treated with an aortic root and arch replacement using a frozen elephant trunk technique. Timely surgical intervention and appropriate long-term medical manag

Delayed cardiogenic shock following spontaneous coronary artery dissection in a young, healthy female: a case report.

BACKGROUND: Spontaneous coronary artery dissection (SCAD) is a rare, non-atherosclerotic cause of acute coronary syndrome that predominantly affects young, healthy women without conventional cardiovascular risk factors. Reporting such cases is essential to raise awareness of their variable presentation and diagnostic challenges, particularly when initial findings are unremarkable. CASE PRESENTATION: We discuss a case of SCAD in a 37-year-old Iranian woman with no prior medical history or cardiovascular risk factors. The patient was initially presented with non-specific symptoms and a normal E

Clinical outcomes of anemia in patients with spontaneous coronary artery dissection: a retrospective cohort analysis.

Spontaneous coronary artery dissection (SCAD) is an increasingly recognized cause of acute coronary syndrome, especially in younger patients and women. Anemia, a common comorbidity in patients with cardiovascular disease, may modulate outcomes in SCAD, but its influence remains poorly defined. We identified adult hospitalizations for SCAD in the Nationwide Inpatient Sample (2016-2022), stratified by anemia status. Multivariable logistic regression models were constructed to examine the relationship between anemia and in-hospital outcomes, while adjusting for demographics, comorbidities, and ho

Genomic and proteogenomic insights into Spontaneous Coronary Artery Dissection (SCAD): A systematic review of emerging multi-omic evidence.

BACKGROUND: Spontaneous coronary artery dissection (SCAD) is a major cause of myocardial infarction in young women without traditional cardiovascular risk factors (Hayes et al., 2018; Adlam et al., 2018 [1, 2]). Despite growing awareness, its biological underpinnings remain incompletely understood, and clinical management is largely based on observational evidence rather than mechanistic insight (Saw et al., 2014; Lettieri et al., 2015; Steg et al., 2024 [3-5]). OBJECTIVES: To systematically integrate genomic, epitranscriptomic, proteomic, and metabolomic data in order to characterize the mul

[A successful treatment of Cabrol's procedure for annuloaortic ectasia with an acute aortic dissection (Stanford type A)].

A 67-year-old female who has an Annuloaortic ectasia with an acute aortic dissection was urgently admitted to our hospital. She was also suspected to have developed into an acute cardiac tamponade. An aortic dissection was arised from just above the right coronary artery orific and extended to the innominate artery and the right common carotid artery. In this case Cabrol's procedure with cerebral perfusion technique was considered to be a good indication, but standard Cabrol's procedure provided almost satisfactory results without utilizing a cerebral perfusion technique because disappearance

Supercharged pectoralis major musculocutaneous flap.

The pectoralis major musculocutaneous (PMMC) flap was once considered the workhorse for head and neck reconstruction; however, because of the proliferation of free tissue transfer, it has rightly taken on a secondary role. Nevertheless, in certain head and neck reconstructions, the PMMC flap remains the last-line treatment and the only salvage option in do-or-die scenarios. The conventional harvesting method of the PMMC flap cuts the lateral thoracic artery and all intercostals branches from the internal mammary vessel to avoid compromising pedicle length. Nonetheless, the dissection of these

Analysis of the vascular anatomy of the palm and its clinical relevance in Morbus Dupuytren.

The triangular center of the palm is supposedly hypovascularized. Intraoperative clinical studies have shown numerous vessels and nerves rising through the palmar aponeurosis into the skin in this area. To investigate these findings we have dissected 7 cadaver hands in order to analyze the vascular and neural structure of the palm. On an average we have found 30 bundles in the center of the palm. At least half of them were vascular-nerve bundles, the other bundles consisted either of a nerve, an artery or a vein. By means of histological testing we have also found lymphatic vessels accompanyin

Endovascular repair of thoracoabdominal aortic aneurysms in the 21st century.

Over the last two decades, there have been dramatic advancements in fenestrated and branched technology for endovascular repair of the thoracoabdominal aorta. The global development of these minimally invasive techniques decreased their short-term morbidity and mortality, compared to open aortic replacement. However, the need for secondary reinterventions remains frequent, although they are typically percutaneous. Continual device modifications, improved implantation techniques, and greater understanding of the endovascular tenets constantly improve rates of long-term success. Challenges exist
