[Clinical Experience of Endovascular Stent-graft Treatment for Stanford Type A Acute Aortic Dissection].

BACKGROUNDS: Recently, thoracic endovascular aortic repair (TEVAR) is widely accepted and performed in patients with complicated and uncomplicated Stanford type B aortic dissection. However, TEVAR for Stanford type A aortic dissection is not commonly performed even in the endovascular era. This report describes patients who underwent TEVAR for Stanford type A dissection. PATIENTS: Since 2016, 5 patients underwent TEVAR for retrograde acute Stanford type A dissection. A covered stent-graft was placed in the descending aorta for primary entry coverage. An additional bare-stent was placed in the

[Progress in surgery of the aortic arch].

We summarize our experience out of 133 operations involving the aortic arch which were performed in 130 patients throughout the last 13 years. Aortic pathology was aneurysmal disease in 57 cases, acute aortic dissection in 44, and chronic aortic dissection in 29 patients. Operative strategies included partial replacement or repair of the aortic arch in 80 cases and subtotal or total replacement of the transverse arch in 53 patients. In 19 cases presenting with aneurysms of the descending thoracic aorta, implantation of an elephant trunk prothesis was performed simultaneously. The operations we

[Bicuspid aortic valve].

Bicuspid aortic valve (BAV) is the most common congenital heart disease, whose natural history is characterized by the incidence of clinically relevant valvular (stenosis, regurgitation, endocarditis) and/or vascular complications regarding the thoracic aorta (dilation, aneurysm, dissection) and, rarely, intracranial and epiaortic arteries. BAV may be heritable, with an autosomal dominant pattern of inheritance with reduced penetrance; moreover, some data suggest that BAV and thoracic aorta aneurysm are independent manifestations of a single gene defect. The prevalence of BAV and its susceptib

[Aortic Dissection In The Cardiology Department Of The Point G University Hospital (Mali): Cardiovascular Emergency About 4 Observations].

Aortic dissection (AD) is a medicosurgical emergency whose diagnosis has been considerably facilitated in recent years by the development of reliable and easily available diagnostic methods such as echocardiography, spiral CT angiography, and even magnetic resonance imaging. The prognosis remains serious, particularly when the ascending aorta is involved (type A AD), therefore immediate surgical treatment can considerably reduce its mortality. The prognosis for dissections of the descending thoracic aorta (type B) is the best; treatment is primarily medical in the absence of complications. We

[Estimation of atherosclerotic lesions in the thoracic aorta by transesophageal echocardiography].

The thoracic aorta is one of the portions frequently involved with atherosclerotic lesions. We estimated atherosclerotic lesions in the thoracic aorta in consecutive 137 patients (59 +/- 10 years old) using transesophageal echocardiography. The study population consisted of 60 with coronary artery disease (CAD), 36 with hypertension (HT), 28 with hypercholesterolemia (HC, total cholesterol greater than or equal to 230 mg/dl), eight with dissecting aneurysm, nine with true aneurysm and two with aortitis syndrome. We used a single transverse scan-plane probe with ultrasound frequency of 5 MHz. W

Homemade Candy Plug Using a Zenith Alpha Thoracic Stent-Graft for False Lumen Distal Occlusion in Acute-on-Chronic Type B Aortic Dissection.

Purpose: To report a candy-plug technique using a Zenith Alpha stent-graft to occlude the distal false lumen in a patient with a complicated chronic type B aortic dissection. Case Report: A 50-year-old male smoker presented with chest pain due to rapidly growing complicated chronic type B aortic dissection. Computed tomography angiography (CTA) showed the dissection extending from distal to the origin of the left subclavian artery (LSA) down to the left femoral artery. There was fusiform aneurysmal dilatation of the proximal descending aorta measuring up to 5.8 cm in diameter. He underwent lef

A 20-year experience with thoracic endovascular aortic repair.

BACKGROUND: Endovascular approaches (thoracic endovascular aortic repair) have revolutionized treatment of thoracic aortic disease. OBJECTIVE: We report our 20-year experience with this therapy. METHODS: Four hundred twenty patients (mean age = 69.0 years; 54% male) underwent thoracic endovascular aortic repair (1993-2013), predominantly for fusiform aneurysm (n = 144), saccular aneurysm (n = 94), acute (n = 64) or chronic (n = 36) dissection, or traumatic injury (n = 39). Rupture was present in 80 patients (19.1%). Most patients (78.3%) were at high risk for open repair. Mean aortic diamete

Single-stage reoperative repair of chronic type A aortic dissection using the arch-first technique.

BACKGROUND: Management of the enlarged, chronically dissected aorta after previous repair of acute ascending aortic dissection or after a previous cardiac operation may present a formidable technical challenge and the optimal method of management is not clearly established. METHODS: Twenty-one patients with chronic type A aortic dissection (mean age 57 years) underwent resection of the ascending aorta, the aortic arch, and varying segments of the descending thoracic aorta. Single-stage replacement with perfusion of the aortic arch first to minimize the duration of brain ischemia and a bilater

[Two-stage treatment of a patient with type I aortic dissection: an open operation combined with endovascular prosthetic repair].

Presented herein are the results of clinical follow up of a patient with type I aortic dissection, subjected to two-stage treatment: the first stage consisted in prosthetic repair of the ascending portion of the aorta and aortic arch, the second stage being endovascular prosthetic repair of the thoracic portion of the aorta with a stent graft. The ostium of the left subclavian artery entrapped into the zone of implantation of the stent graft was occluded by means of the "Amplatzer" device.

[Thoracic aortic dissection revealed by systemic cholesterol embolism].

Systemic cholesterol embolism is a rare complication of atherosclerosis, and has various presentations. Arterial catheterisms are a common cause. However, the association with an aortic dissection has been exceptionally reported. We report the observation of a 70 year-old man, with coronary artery disease, hypertension, diabetes and dyslipidemia. Six months before hospitalization, a coronary angioplasty was performed due to recurrent angina. The association of purpuric lesions on the feet, with acute renal failure confirmed cholesterol embolism syndrome. Transoesophageal echocardiography showe

Pulmonary artery dissection complicating aortic dissection in a patient with bicuspid aortic valve and aortic coarctation.

We report a case of a type A aortic dissection with extension into the main pulmonary artery through a sinus of Valsalva fistula. Echocardiography and computed tomographic angiography of the chest were performed and bicuspid aortic valve, hemopericardium, and dilatation of the aortic root were found. A Stanford type A dissection was seen, extending to the distal transverse thoracic aorta, and there was a communication between the dissection at the left sinus of Valsalva and the main pulmonary artery, where a dissection flap was detected at computed tomographic angiography. This case report rev

Management of an enlarging aortic aneurysm in the presence of radiation induced retroperitoneal fibrosis.

Despite a thoracoabdominal retroperitoneal approach to an enlarging symptomatic infrarenal aortic aneurysm, proximal aortic dissection was hazardous due to radiation induced retroperitoneal fibrosis. Iliac artery ligation and thoracic aorta to iliac artery bypass has resulted in successful management during 14 months of follow-up.

High heritability of ascending aortic diameter and trans-ancestry prediction of thoracic aortic disease.

Enlargement of the aorta is an important risk factor for aortic aneurysm and dissection, a leading cause of morbidity in the developed world. Here we performed automated extraction of ascending aortic diameter from cardiac magnetic resonance images of 36,021 individuals from the UK Biobank, followed by genome-wide association. We identified lead variants across 41 loci, including genes related to cardiovascular development (HAND2, TBX20) and Mendelian forms of thoracic aortic disease (ELN, FBN1). A polygenic score significantly predicted prevalent risk of thoracic aortic aneurysm and the need

Possible mechanical roles of glycosaminoglycans in thoracic aortic dissection and associations with dysregulated transforming growth factor-β.

BACKGROUND: Four distinguishing histopathological characteristics of thoracic aortic aneurysms and dissections (TAADs) are the fragmentation or degradation of elastic fibers, loss of smooth muscle, pooling of glycosaminoglycans, and remodeling of fibrillar collagens. Of these, pooling of glycosaminoglycans appears to be unique to these lesions. METHODS: This review acknowledges the importance of dysregulated transforming growth factor-β (TGF-β) in TAADs and offers a complementary hypothesis that increased TGF-β could contribute to the accumulation of glycosaminoglycans in the media of the pro

Aortic dissection manifested as leg ischemia.

The ischemic leg as the only clinical manifestation of an aortic dissection is rare. Without the usual symptoms of an aortic dissection, this diagnosis may easily by overlooked during routine peripheral angiography for the ischemic limb. Three cases are reviewed in which the patient was thought to have a spontaneous peripheral vascular occlusion, but was later found to have an aortic dissection without the usual thoracic or abdominal symptoms. The radiologist should observe carefully for signs of dissection such as fusiform narrowing of a vessel, nonfilling of branches of the aorta, or displac

Single-stage total thoracoabdominal endovascular repair for acute non-A non-B aortic dissection with retrograde extension to the ascending aorta and concomitant abdominal dissecting aneurysm: a case report.

BACKGROUND: Acute non‑A non‑B aortic dissection represents a complex and high-risk condition. Traditional treatment protocols mandate emergency open surgical repair for any dissection involving the ascending aorta (Stanford Type A or DeBakey Type I/II). With ongoing advancements in endovascular devices, less invasive options are emerging as viable alternatives for select high-risk patients with ascending aortic involvement, potentially offering advantages over total arch replacement. CASE PRESENTATION: A 73-year-old male presented with one week of chest and back pain and was diagnosed with ac

Takayasu's arteritis mimicking acute aortic dissection.

A 17-year-old girl presented with a history of dyspnea on exertion and fever of 1-week duration. She was evaluated elsewhere with transesophageal echocardiography and helical computed tomographic scan, and she had been diagnosed with an acute type I dissection of the aorta. She had also been diagnosed with severe aortic regurgitation and a suspected aortic root abscess. On the operating table, we found no evidence of dissection, but we did find that her aorta was severely thickened and inflamed. The patient's aortic valve was replaced. In view of the left main stem ostial stenosis, we harveste
