PURPOSE: Acute diverticulitis is a disease with a wide clinical spectrum, ranging from a phlegmon (stage Ia), to localized abscesses (stages Ib and II), to free perforation with purulent (stage III) or feculent peritonitis (stage IV). While there is little debate about the best treatment for mild episodes and/or very severe episodes, uncertainty persists about the optimal management for intermediate stages (Ib and II). The aim of our study was therefore to define the role of computed tomography (CT) and to analyze its impact on the management of acute diverticulitis.
METHODS: We retrospective

Since the middle to the end of the 80 s, the sonographic detection of diverticulitis has been increasingly improved. In a paper including a larger number of patients published in 1992, W. B. Schwerk demonstrated a high sensitivity and specificity. The detection of diverticulitis was the final entry into the chapter of acute abdominal sonography, after the diagnosis of gastrointestinal perforation, acute appendicitis and ureterolithiasis had been achieved with high reliability. Until then, diverticulitis was a classic surgical disease and a contrast enema with water-soluble contrast medium the

The purpose of this study is to evaluate the application of computed tomography (CT) in the diagnosis of colonic diverticulitis in the emergency department (ED). Clinical information and CT images of 38 proved cases of colonic diverticulitis were reviewed retrospectively. The most common CT findings were pericolic fat infiltration (100%), thickened fascia (78.9%), muscular hypertrophy (26.3%), and arrowhead sign (23.7%). The CT findings were correlated with clinical prognosis and 3 parameters were found to be indicators for poor prognosis: older age, abscess formation, and presence of free air

BACKGROUND: The diagnostic procedures proposed in the evaluation of sigmoidovesical fistulas complicating diverticulitis are various and their effectiveness is still not well established.
METHODS: Personal experience is based on 14 cases of colovesical fistulas secondary to sigmoid diverticulitis. Several diagnostic tools were employed: plain abdominal film (10 cases), large bowel enema (12), urography (3), cystography (2), sonography (4), and CT (5). The semeiotics of these fistulas were distinguished in direct, indirect, secondary, and related to the underlying disease.
RESULTS: The fistul

The colo-uterine fistula is a rare complication of diverticular disease of the colon; the literature review has shown only few well studied cases. The fistula, among the complications of the sigma diverticulitis, is 20% of the observed cases; generally, the bladder is the most involved organ, but also the skin or gut can be interested. If we consider the aetiology of the colo=uterine fistula of the observed case, the presence of the sigma locked stenosis with an endocolic pressure increase, associated with a peridiverticulitis condition, seems to have a relevant rule. The clinical symptomatolo

BACKGROUND: The clinical diagnosis of acute colonic diverticulitis (ACD) can be difficult and ultrasonography by experts is valuable in establishing the diagnosis. This prospective observational trial aimed to assess the diagnostic accuracy and clinical value of ultrasonography performed routinely by surgical residents in training.
METHODS: The clinical course of 187 unselected consecutive patients admitted with suspected ACD was studied prospectively. Patients who had surgery for generalized peritonitis were excluded, leaving 143 for evaluation. Ultrasonographic findings and clinical assessm

BACKGROUND: Diverticular disease of the colon and its complications are well known and readily considered when patients present with the proper clinical scenario. Conversely, complications of small bowel diverticula are very uncommon entities and are not often thought of as a cause of bleeding, obstruction, or an acute abdomen.
OBJECTIVE: To report two patients presenting with an acute abdomen caused by acute jejunal diverticulitis who were treated nonsurgically as opposed to surgically as the literature dictates.
METHODS: Two patients presented with sudden onset of acute periumbilical pain

To assess the ultrasonographic (US) patterns of acute colonic diverticulitis, we prospectively examined 21 patients. US was performed in all of them, CT in 13/21, contrast enema in 18/21 and endoscopy in 2/21 patients. US follow-up was carried out in 15/17 patients managed with conservative treatment. To analyze the US patterns of simple diverticula, we carried out an in vitro study of two surgical specimens with simple colonic diverticulosis. Wall thickening was seen in 21/21 cases, diverticula in 19/21, changes in pericolic fat in 11/21, local tenderness on gradual compression in 19/21 and c

The diagnosis of acute diverticulitis is obvious if the patient presents with the typical symptoms of acute pain in the left lower abdominal quadrant and fever. A high index of suspicion, however, is required to diagnose diverticulitis with atypical presentation, which may be present in the elderly or immunocompromised patient. The detection of complications, most commonly diverticular abscess and colovesical fistulas, requires a careful and repeated examination and diagnostic skill.

In a retrospective study we attempted to find prognostic factors for the mortality in perforated colonic diverticulitis. All patients of the First Surgical Department of Vienna University from 1965-1986 with complicated diverticulitis were included in this study. 47 patients had a perforated diverticulitis with a local abscess or a generalized peritonitis. In 35 patients a resection, in 12 patients a faecal diversion was performed. The mortality was 27%. A marked influence of age and preoperative complications on the prognosis was shown statistically. The operative procedure seems of secondary

A case is reported of a patient without previous gastrointestinal symptoms, who presented with polymicrobial septicemia caused by Escherichia coli, Enterococcus faecalis, Clostridium difficile and Bacteroides vulgatus. Septicemia occurred during acute diverticulitis. A strain of Clostridium difficile, which was of the same serogroup C as the blood culture isolate and also produced toxin, was recovered from the stools, but the pathogenic role of this organism in the gastrointestinal symptomatology was not clearly established. Other reported cases of Clostridium difficile septicemia are also rev

In two cases of suspected appendicitis, laparotomy revealed an inflammatory mass medially in the caecum. Colonic resection was performed in both cases and the final diagnosis was solitary caecal diverticulitis. When inflammation of a caecal diverticulum is recognized at laparotomy, simple diverticulectomy is the procedure of choice, but colonic resection is recommended if malignancy cannot be excluded or inflammatory changes are severe.

The report of diverticulosis of right colon in a patient formerly operated on for perforation of caecum single ulcer appeared to the authors as a confirmation of a common etiology between both lesions. The single ulcer would probably represent a form of manifestation for the acute phlogosis of a diverticulum.

We report the second case of a malignant fibrous histiocytoma of the large bowel. The neoplasm constituted an incidental finding during the diagnostic work-up for diverticulitis of the sigmoid colon. There was a massive abdominal recurrence of the tumor four months after colectomy, and the patient died nine months after the initial operation. The literature on fibroblastic sarcomas of the colon is also reviewed.

Perforation in the course of diverticulitis of the right side of the large bowel is a rare disease, which is mostly misinterpreted as appendicitis and generally disregarded in the differential diagnosis of pathological processes of the lower right abdomen. The results of radiological investigations (plane film, barium enema, CT) are demonstrated in two cases and diagnostic findings are discussed. Plane film of the abdomen allows only unspecific diagnosis of an inflammatory process in the right lower quadrant, barium enema reveals changes in the wall of the colon due to abscess formation. CT is

Three patients had normal bowel function with the sigmoid colon exteriorized through an abdominal wound for 39, 42 and 72 days. Colostomy was thus avoided during the time the patient was recovering from peritonitis and inflammation in the involved segment of colon. Ready access was provided to the distal limb of the colon at second operation.
