It is not always appendicitis.

BACKGROUND: Patients who are suspected of having acute appendicitis usually undergo surgery in order to avoid life-threatening complications such as perforation and peritonitis. However, acute appendicitis is difficult to distinguish from other sources of right-sided abdominal pain. The clinical picture is almost indistinguishable from appendiceal diverticulitis, which is a rare entity and remains a difficult diagnostic problem. PATIENTS AND METHODS: We describe the case of a 39-year-old male with perforated appendiceal diverticulitis. The patient was admitted to our surgical unit with acute

Perforated sigmoid diverticulitis in a lumbar hernia after iliac crest bone graft--a case report.

BACKGROUND: The combination of perforated diverticulitis in a lumbar hernia constitutes an extremely rare condition. CASE PRESENTATION: We report a case of a 66 year old Caucasian woman presenting with perforated sigmoid diverticulitis localized in a lumbar hernia following iliac crest bone graft performed 18 years ago. Emergency treatment consisted of laparoscopic peritoneal lavage. Elective sigmoid resection was scheduled four months later. At the same time a laparoscopic hernia repair with a biologic mesh graft was performed. CONCLUSION: This case shows a very seldom clinical presentation

[Acute perforated sigmoid diverticulitis in childhood].

Colonic diverticulosis is a pathology of high incidence in normal population during the last decades of life. About 2-5 per 100 of patients admitted because of this disease are below 40 years old, and in this group of patients diverticulitis tends to be very aggressive with a high number of complications. We report a clinical case of acute perforation of sigmoid colon due to diverticulitis in a ten year old child who was treated surgically by means of resection of the sigma and primary end-to-end anastomosis. We did not find in the literature consulted any cases of this illness under twenty ye

Smoking increases the incidence of complicated diverticular disease of the sigmoid colon.

BACKGROUND AND AIMS: The aim of this study was to establish whether smoking is associated with complicated diverticular disease and adverse outcomes of operative treatment of diverticular disease. Smoking has been associated with increased rate of perforations in acute appendicitis as well as failure of colonic anastomosis in patients resected for colonic tumours. It has also been suggested that smoking is a risk factor for complicated diverticular disease of the colon. MATERIAL AND METHODS: Retrospective investigation of records of 261 patients electively operated for diverticular disease in

Acute perforated diverticulitis: primary resection and anastomosis?

Surgical management of patients with acute colonic diverticulitis is evolving from multiple towards single operations. The patterns of presentation and treatment of 146 patients with acute perforated diverticulitis have been reviewed (1983-1988) using the Hinchey classification system (Stages I-IV). This paper focuses on the six patients who presented with fecal peritonitis (Stage IV disease), half of whom were treated by primary resection and anastomosis and the remainder by a Hartmann procedure. The mean length of stay was 18.7 +/- 7.9 days and 12.7 +/- 4.8 days with a mortality of zero and

Preoperative diagnosis of colouterine fistula secondary to diverticulitis by sonohysterography with contrast medium.

Colouterine fistulae secondary to sigmoid diverticulitis are unusual. Methods for diagnosis remain to be established. We report a case with a colouterine fistula in which sonohysterography detected the flow of ultrasound contrast medium between the uterine cavity and the sigmoid colon through the posterior uterine wall, thus confirming the diagnosis. The diagnosis was further substantiated by a charcoal challenge test. The patient underwent en bloc resection of the uterus, Fallopian tubes, ovaries and sigmoid colon, the organs involved with diverticulitis. This is the first report to describe

Appendiceal diverticulitis masked as acute appendicitis.

Diverticula of the appendix have been the subject of study since the early 1900s. Appendiceal diverticulitis has varying presentations that can be acute or chronic and has features that differentiate it from classical acute appendicitis. We present the case of a 61-year-old female who was incidentally found to have appendiceal diverticulitis. Appendiceal diverticula are divided into two types, congenital and acquired. This patient developed an acquired appendiceal diverticula. The exact etiology and pathogenesis of acquired appendiceal diverticulosis is unknown. Risk factors associated with ac

[Conservative and interventional therapy of acute diverticulitis with reference to pathophysiology].

Diverticular disease is most common in the sigmoid colon. Its etiology is multifactorial and probably related to low-fiber diets, age dependent changes of the colonic wall, hypermotility and myochosis with subsequent increase in intraluminal pressure. Acute diverticulitis results from inflammation of a pseudo-diverticulum. It can progress to pericolitis and perforation with abscess formation. Therapy of uncomplicated diverticulitis is a conservative regimen with bowel rest and intravenous broad spectrum antibiotics. In subjects with complicated diverticulitis, preoperative percutaneous image-g

Complicated diverticulitis: medical management or segmental resection?

INTRODUCTION: Diverticulitis is the most common complication related to the diverticulosis of the colon. Since there are some concerns about the management of complicated diverticulitis, the aim of this review was to analyze current medical and surgical approaches to complicated diverticulitis and evolving advances in its management. AREA COVERED: An analysis of the current PubMed literature about the medical and surgical management of complicated diverticulitis was performed. EXPERT OPINION: Attentive evaluation of the characteristics of complicated diverticulitis may make the difference wh

Novel polygenic risk score associates with diverticulitis in a multi-institutional, ancestrally diverse cohort.

Diverticulitis is a common and morbid colorectal disease that recurs after an initial attack in up to 30% of patients. Elective surgery to remove the affected portion of the colon is an effective intervention to prevent disease recurrence, but appropriate patient selection is challenging given the limited ability to predict which patients are likely to recur, or the severity with which they may do so. Genetics influence diverticulitis and can be used for risk stratification, but this has only been studied in European ancestry populations. Using state-of-the-art techniques, we created a polygen

Diverticulitis: new insights on the traditional point of view.

Diverticulosis of the colon is a common disease with an increasing incidence in Western countries. Recent literature has shown some changes in the traditional approach of this disease. The theory that diverticulosis is caused by a reduced intake of dietary fibre, is doubtful. There might be some chemical and histological overlap between diverticulitis, inflammatory bowel disease and irritable bowel disease. High quality clinical study found no effect for antibiotics in acute, uncomplicated diverticulitis. Cyclic administration of mesalazine and rifaximin result in reduced symptoms of diverticu

Calcifying bowel inflammation: a case report.

We report about a previously healthy 72-year-old woman, presented with 6 days of left lower quadrant abdominal pain and constipation. There was no report of fever, melena, hematochezia or change in appetite. The physical exam demonstrated a distended abdomen with palpable left lower quadrant pain, without guarding. CT showed images compatible with a sigmoid diverticulitis and a calcification of the sigmoid colon. After antibiotic threatment, a colonoscopy was performed which revealed the presence of a shell in the sigmoid colon. Our case illustrates the need for a colonoscopy following an atta

Double tracking in the sigmoid colon.

Double tracking of barium in the sigmoid colon has been associated with granulomatous colitis by some; others consider it a sign of peridiverticulitis. Patients who showed no evidence of inflammatory disease elsewhere in the large intestine were analysed. The pathological diagnosis was diverticulitis in 29 of 35 cases, and granulomatous diverticulitis in only 1. In 5 the underlying process was perforated carcinoma of the sigmoid colon. In those with paracolonic sinus tracts due to peridiverticulitis, pathological study demonstrated tracts extrinsic to the muscularis and usually traceable to a

Diarrhea-predominant irritable bowel syndrome is associated with diverticular disease: a population-based study.

OBJECTIVES: A subset of patients with colonic diverticular disease have chronic gastrointestinal symptoms, and some have a clinical diagnosis of irritable bowel syndrome (IBS), but whether IBS and diverticular disease are linked is uncertain. We aimed to evaluate this association in the community. METHODS: A population-based, cross-sectional survey was conducted by mailing a valid symptom questionnaire to the eligible residents of Olmsted County, MN, aged 30-95 years. Colonic diverticular disease (diverticulosis and diverticulitis) was ascertained through a review of the complete medical hist

Concomitant Sigmoid Diverticulitis and Periampullary Duodenal Diverticulitis Complicated by Lemmel Syndrome: A Case Report.

Diverticular disease is a major cause of hospitalizations, especially in the elderly. Although diverticulosis and its complications predominately affect the colon, the formation of diverticula in the small intestine, most commonly in the duodenum, is well characterized in the literature. Although small bowel diverticula are typically asymptomatic, and diagnosed incidentally, a complication of periampullary duodenal diverticulum is Lemmel syndrome. Lemmel syndrome is an extremely rare condition whereby periampullary duodenal diverticula, most commonly without diverticulitis, leads to obstructio
