Dizziness and imbalance are amongst the most common complaints in older people, and are a growing public health concern since they put older people at a significantly higher risk of falling. Although the causes of dizziness in older people are multifactorial, peripheral vestibular dysfunction is one of the most frequent causes. Benign paroxysmal positional vertigo is the most frequent form of vestibular dysfunction in the elderly, followed by Meniere's disease. Every factor associated with the maintenance of postural stability deteriorates during aging. Age-related deterioration of peripheral …

BACKGROUND: Vertigo and dizziness are often not fully explained by an organic illness, but instead are related to psychiatric disorders. This study aimed to evaluate psychiatric comorbidity and assess psychosocial impairment in a large sample of patients with a wide range of unselected organic and non-organic (ie, medically unexplained) vertigo/dizziness syndromes.
METHODS: This cross-sectional study involved a sample of 547 patients recruited from a specialised interdisciplinary treatment centre for vertigo/dizziness. Diagnostic evaluation included standardised neurological examinations, str…

Virtual reality (VR) training that provides optic flow stimuli and visuo-vestibular conflict has been suggested as a way to treat patients with inappropriate visual dependence (sometimes called visual vertigo even though spinning sensation is often absent). We propose a simple framework based on a hypothesis that the degree of dizziness depends on the offsetting between the destabilizing effect of optical flow and the stabilizing effect provided by stationary objects in the visual field. We define a total destabilizing potential (TDP), which is the ratio of the destabilizing effect over the st…

INTRODUCTION: Dizziness can be due to multiple causes. However, the aetiology often remains unclear. At the same time, there is a lack of evidence-based treatment options. The aim of this study was to investigate the frequency of dizziness-related diagnoses, referrals and prescriptions in a general practice database.
METHODS: Data from computerized patient records of 138 general practices participating in the MedViP project were used for cross-sectional analysis of the time period April 2001 until December 2002. The identification of dizzy patients was performed via ICD-10 diagnoses, free tex…

BACKGROUND: Prevention of falls in the elderly is a major health care target. There are theoretical reasons why older dialysis patients may be at high risk of falls: co-morbidity, medication, and post-dialysis hypotension, which have not been well tested. Dialysis patients are also at higher risk of fracture if they do fall.
METHODS: We prospectively interviewed all our centre haemodialysis patients over a 6 month period to see if they reported falls, syncope, pre-syncope or dizziness. Routine blood pressure (BP) and other clinical data were recorded.
RESULTS: A total of 78 patients complete…

Fear of heights results in the experience of dizziness and measurable body sway. We investigated the relationship between fear, dizziness, and body sway during height exposure 16 m above ground. Thirty five healthy participants stood on a force-plate to measure sway before, during, and after exposure and an ECG was recorded. Self-report measures were anticipated fear and dizziness before exposure, as well as actual fear and dizziness during the three situations. For all participants, fear, dizziness, and body sway were increased during exposure. Anticipated fear most reliably predicted body sw…

This study presents the neuro-otological findings of 523 patients attending a tertiary vestibular clinic with migraine- and nonmigraine-related dizziness. Subjects were categorized into one of 4 groups, definite migrainous vertigo, probable migrainous vertigo, vestibular disorder coexisting with migraine and nonmigraine-related dizziness. No notable relationship was found between the numbers of abnormal findings between the groups for the majority of the neuro-otological tests. However, there was a significant trend in emetic response to caloric testing. The definite migrainous vertigo group w…

In 1993 a multidisciplinary neurotology clinic was established at the Toronto Hospital, University of Toronto, where patients with symptoms of dizziness were assessed by both otolaryngologists and neurologists. The results from the first 400 patients seen in consultation are described. The disease pathologies identified in this patient population with dizziness showed some significant differences from other published series, which we believe reflects the specialized tertiary nature of referrals to this clinic. A model for the collaborative investigation of the dizzy patient is provided consist…

In the present study possible interactions between dizziness, or vertigo, and eventual malfunctioning of the Autonomic Nervous System were evaluated. Two groups of ten patients each were patients each were tested. All the patients had vertigo or dizziness of uncertain etiology and ten had arterious hypotension. A careful anamnestic research was carried out in order to individualize symptoms or elements which could induce dysautonomia. A series of 6 tests aimed at evaluating cardiovascular responses was performed in all the patients. Alterations were present in 65% of the test responses. In par…

We evaluated 102 patients with dizziness or vertigo who were 50 years of age and over, using magnetic resonance imaging (MRI). Slow blood flow (SBF) in the vertebrobasilar system was detected in 36 patients (35%). The patients with SBF experienced dizziness or vertigo for a longer period than those without SBF. The apogeotrophic type of direction-changing nystagmus was observed in 10 of 36 patients with SBF. No significant differences were found between patients with and without SBF with other neurotological tests. Because MRI can detect both infarcts in the hind-brain and SBF in the vertebral…

INTRODUCTION: Persistent postural-perceptual dizziness (PPPD) is a chronic functional vestibular disorder defined by persistent dizziness, unsteadiness, or non-spinning vertigo exacerbated by upright posture, active or passive motion, and visually complex environments. In clinical practice, however, the diagnostic label alone is often insufficient to guide management, because patients differ substantially in the relative prominence of visual dependence, motion-postural burden, multisensory provocation, affective-hypervigilant mechanisms, and migraine overlap.
OBJECTIVE: To propose a clinicall…

Earthquakes pose significant health challenges, including post-earthquake dizziness syndrome (PEDS), which is characterized by dizziness, vertigo, imbalance, nausea, and motion sensations. This review aimed to examine the intricate relationships between earthquake characteristics, building types, and dizziness, with a particular focus on the function of the autonomic nervous system in mediating these effects. The prevalence of PEDS varies based on earthquake magnitude, demographics, and environmental conditions, affecting 20-40 % of the population in regions such as Hokkaido, Kumamoto, Türkiye…
