Mifamurtide is a conjugate of muramyl tripeptide linked to dipalmitoyl phosphatidyl ethanolamine; the phospholipid facilitates incorporation of the peptide into liposomes. The agent stimulates macrophages to seek out and destroy cancer cells. The compound was originated by Novartis (formerly CIBA-Geigy), and is being developed by IDM Pharma for osteosarcoma. Mifamurtide is being reviewed by regulatory authorities in the US and EU for this indication.CIBA-Geigy originally developed mifamurtide in the early 1980s and the agent was subsequently outlicensed to Jenner Biotherapies in the 1990s. IDM…

Auricular reattachment and reconstruction following traumatic ear avulsion is a challenging surgical problem. Suggested reconstruction methods include direct reattachment, composite grafting, pocket methods, coverage with periauricular flaps, and microsurgical repair. A published alternative is reattachment and burial of the amputated part under a postauricular flap. If circumstances delay the surgical intervention, what is a safe window to still consider this form of treatment? In the current report, we present a case of a complete partial ear avulsion, which was reattached in the ER as a fre…

With the aid of a mechanical middle ear model (MMM) the sound transmission properties of different middle ear implants were investigated. Input of the MMM involved a broad-band signal from 100 to 5000 Hz that was supplied by a miniaturized loudspeaker. Displacement of an artificial stapes footplate was measured by a fiberoptic probe. The transfer functions of four different total ossicular replacement prostheses (TORPs) of different materials and shapes were compared. Three of the devices revealed similar transfer functions which corresponded to the typical curve of the normal middle ear. One …

The aim of this paper is to report my experience of 352 ear reconstructions to correct microtia with Brent's technique (n = 184) and Nagata's technique (n = 144). The remaining 24 were atypical presentations reconstructed in a one-stage procedure. Brent's technique is a safe, step-by-step, four-stage reconstruction and Nagata's technique involves only two stages. Nagata's innovations include transposing the lobule of the auricle during the first stage, carving a framework that includes the tragus, and constructing the posterior wall of the concha during the second stage. Cases of typical micro…

The present study summarizes the experimental findings obtained on the pressure in the inner ear fluids and on the effects of pressure changes on cochlear function in the guinea pig. Two types of pressures have to be distinguished in the inner ear fluid compartments: (i) hydrostatic fluid pressure and (ii) superimposed hydrodynamic high frequency (> 100 Hz) sound pressure oscillations. Hydrostatic pressure in the inner ear fluids in guinea pigs is in the order of 200 Pa (2 cm H2O) and shows slow (< 5 Hz) respiratory and pulsatory oscillations as well as considerable physiological variations in…

The reconstruction of the destroyed ossicular chain has the purpose to re-establish an unhindered sound reception and transmission. With respect to the hydraulic factor, the reconstructed chain should be attached to a large effective area of the drum membrane, ideally to the malleus handle. The sensitivity is furthermore diminished in a linear relationship with increasing mass and stiffness. Therefore, cartilage cannot represent an acoustically ideal material for a drum membrane replacement. The reconstructed chain must vibrate without restraint for an unhindered transmission. Furthermore, the…

Bats host a diverse assemblage of ectoparasites that occupy specific anatomical microhabitats, yet the histological effects of these associations remain poorly documented. Here, we report a case of mite infestation of the external ear in the Neotropical bats species Carollia perspicillata and Carollia brevicauda, with a detailed histological characterisation of the auricle and inner ear of C. perspicillata. Bats were captured during biodiversity monitoring programmes in Antioquia, Colombia, and mites were collected and identified morphologically. Histological and histochemical analyses of ear …

The endolymphatic barrier, composed of tight junctions and ion transporters, regulates the ionic gradient between endolymph and perilymph and plays a pivotal role in maintaining inner ear function. Disruption of this biological barrier can result in ionic imbalance, causing both auditory and vestibular dysfunction. Medication needs to enter the perilymph instead of the endolymph by passing through the perilymphatic barrier to reach the hair cells. Targeted drug delivery has emerged as a promising therapeutic strategy for inner ear diseases, including intratympanic injection, intracochlear infu…

Cytarabine is an antimetabolite commonly used to treat hematological malignancies, especially acute myeloid leukemia (AML), acute lymphoblastic leukemia, and non-Hodgkin's lymphoma. Cytarabine-induced cutaneous adverse effects are common, usually manifesting as morbilliform eruptions predominantly on the acral site, intertriginous zone, and to a lesser extent on the elbows, knees, neck, and ears. The presentation on ears is usually called "Ara-C ears," which is considered as a rare subtype of acral erythema. We report a 53-year-old Thai woman with AML who developed cytarabine-induced rashes. T…

INTRODUCTION: Microsurgery of the ear requires complete evaluation of middle ear surgical anatomy, especially the posterior tympanic cavity anatomy. Preoperative assessment of the middle ear cavity is limited by the permeability of eardrum and temporal bone density. Therefore, middle ear exploration is an extremely useful method to identify structural abnormalities and anatomical variations.
OBJECTIVE: The aim of this study is to determine anatomic variations of the middle ear in an autopsy series.
METHODS: All evaluations were performed in the Forensic Medicine Institute Morgue Department. …

Admittance measurement is a promising tool for evaluating the status of the middle ear in newborns. However, the newborn ear is anatomically very different from the adult one, and the acoustic input admittance is different than in adults. To aid in understanding the differences, a finite-element model of the newborn ear canal and middle ear was developed and its behaviour was studied for frequencies up to 2000 Hz. Material properties were taken from previous measurements and estimates. The simulation results were within the range of clinical admittance measurements made in newborns. Sensitivit…
