Comparison of endoscopic tympanoplasty, cartilage vs. temporalis fascia in chronic suppurative otitis media with inactive mucosal disease: A prospective randomised study.

BACKGROUND: The introduction of endoscopy into middle ear has open up new opportunities for minimal invasive temporal bone surgery. The present study was planned to compare anatomical and functional outcome in patients who underwent endoscope assisted tympanoplasty using cartilage and temporalis fascia graft. METHODS: The present prospective observational randomized study was carried out in the Department of Otorhinolaryngology of a tertiary care teaching hospital of Rajasthan, India from September 2020 to July 2021. Fifty patients of age group 18-60 years, with diagnosis of chronic otitis me

Cartilage-sparing complete otoplasty technique: a 10-year experience in 100 patients.

Otoplasty is one of the most frequently performed esthetic surgical procedures in children and adolescents. Several techniques can give satisfactory results, but few address all the components of the prominent ear deformity. The author reports on the evolution and application of a cartilage-sparing otoplasty technique that addresses all the components of the prominent ear deformity. One hundred patients (200 ears) were treated over a 10-year period. An 8% revision rate and minimal complications were encountered.

Trauma Management of the Auricle.

Smaller injuries of the auricle, such as lacerations without tissue loss, have more or less standardized treatment protocols that require thorough wound closure of each affected layer. Even extended lacerations of larger parts of the ear quite often heal with only minor irregularities. New in vivo diagnostic tools have aided the understanding of this outstanding "skin flap behavior." At the other end of the trauma severity spectrum are partial or complete amputations of the ear. Here, the debate has become more intense over the last decade. There were numerous reports of successful microvascul

Microtia reconstruction: does the cartilage framework grow?

The use of free rib cartilage ear frameworks in unilateral microtia reconstruction has prompted much discussion about their potential for growth. The senior author has reconstructed ear frameworks in 132 microtia patients, most of whom were under 3 years of age when surgery was initiated. Of this group, 29 were assessed for ear growth through comparison of the lead-plate model of the original normal ear to the normal ear growth and the reconstructed ear framework after a period of at least 2 years. Similarly, 14 reconstructed ears were compared to 14 normal ears at least 2 years after reconstr

The anatomy and application of the postauricular fascia flap in auricular reconstruction for congenital microtia.

BACKGROUND: The various layers of the scalp are widely used in reconstructive procedures. The technique used for soft tissue coverage seems to influence the framework extrusion rate in auricular reconstruction. We have investigated the role of the postauricular fascial flap in providing ample, thin neurovascular cover to facilitate ear reconstruction in congenital microtia. METHODS: Following postauricular cutaneous tissue expansion, the postauricular neurovascular fascia flap was raised to partially cover the projected three-dimensional autogenous cartilage framework. A suction drain was pla

Pyrophosphate release by osteoarthritis cartilage incubates.

Incubates of articular cartilage from young but not mature rabbits, as well as growth plate cartilage, elaborated PPi into basel Eagle's medium during a 4-hour period. Control rabbit synovial membrane and ear cartilage elaborated negligible amounts of PPi. The PPi was shown to be nondialyzable but could be dissociated from the alkaline phosphatase by ultracentrifugation. In 16 patients with osteoarthritis a substantial output of PPi by samples of articular cartilage from the knee was demonstrated. The present authors believe that either rapid cell division and matrix synthesis found in the bas…

Otologic aspects of ear burns.

Otologic experiences in a large burn center are discussed and the following areas are emphasized. First, the otologic effects of hyperbaric oxygen therapy and the prevention and treatment of barotrauma that is occasionally secondary to this therapy. Second, the diagnosis and treatment as well as the prevention of perichondritis in severely burned ears will be discussed. A total of thirty-five cases of perichondritis will be reviewed. An encouraging aspect of the method of treatment has evolved over the past five years: there has been a minimum of deformity in the healed ear, with no subsequent

Modification of the stages in total reconstruction of the auricle: Part I. Grafting the three-dimensional costal cartilage framework for lobule-type microtia.

Total auricular reconstruction for microtia is dependent on the fabricated three-dimensional costal cartilage framework, with all the auricular features and the skin flaps formed and utilized to cover the three-dimensional framework, in which extreme caution is required. Furthermore, other factors, such as contraction of the skin graft, use of the remnant ear cartilage in construction of the tragus, and the number of surgeries required, all affect the final results of total auricular reconstruction for microtia, especially with reference to the contour of the constructed ear, morphologic locat…

[Relapsing polychondritis].

For three months a 17-year-old boy had been suffering from laryngitis, pharyngitis and conjunctivitis, weight loss, nocturnal sweating and signs of changing cranial-nerve involvement (dizziness, nausea, nystagmus). The diagnosis of recurrent polychondritis was made only when, in addition to a definite inspiratory stridor there also developed a painful swelling of the left ear cartilage and a saddle nose due to loss of the cartilaginous portion of the nasal skeleton. Histological examination of the inflamed ear cartilage confirmed the diagnosis. After treatment with prednisone (100 mg daily, gr…

Tenosynovial Giant Cell Tumor in the Dermis of the External Auditory Meatus.

A 26-year-old African American woman with a history of a recurring "oozing papule" in the right ear presented to the emergency department in July 2010 with a 2-month history of an enlarging, painful growth in the right ear canal. Physical examination revealed a 1-cm round cystic lesion along the right, anterior external auditory canal wall, just medial to the tragus. Initial diagnosis was an infected cyst of the external ear canal. The patient was instructed to follow-up with an ear, nose, and throat (ENT) office. Two months following the emergency department visit, inspection by ENT revealed

[Materials for reconstruction of the middle ear].

To rehabilitate most cases of conductive hearing loss closure of ear drum perforations and rebuilding of the ossicular chain can be performed. Due to the great number of biocompatible bone substitute materials available it is occasionally difficult for the surgeon to choose the most favorable substitute. Autogenous structures (ossicles, cortical bone, cartilage) and allogenous tissues (ossicles, cortical bone, cartilage, dentin) are possible bone replacement materials. Xenogenic tissue is currently not used in middle ear surgery. Ionomer cement is a hybrid material for replacement of bone but

Cartilage tympanoplasty for management of tympanic membrane atelectasis: is ventilatory tube necessary?

OBJECTIVES: Cartilage/perichondrium composite graft with concomitant placement of a ventilation tube is a common practice among otologists to reverse atelectasis and to repneumatize the middle ear. We conducted this study to investigate the necessity of a ventilation tube primarily incorporated into the perichondrium/cartilage graft for reconstruction of the atelectatic tympanic membrane (TM). STUDY DESIGN: Prospective clinical trial. METHODS: Forty-six patients with TM atelectasis and intact ossicular chain were randomized to 2 groups. In Group I, 23 patients underwent reconstruction of the

The surgical correction of protuberant ears.

While prominent ears are considered a sign of good fortune in the Far East, Western society looks upon prominent ears in a far less positive manner. Children with prominent ears are often the subjects of verbal and at times physical abuse by their peers, resulting in adverse psychological effects. Advances in otoplasty have made it possible not only to pin back the ears, but also to reshape them, reduce their size, or make them more symmetrical. For a dermatologic surgeon, an otoplasty may be an unfamiliar surgical procedure, however, the surgery itself does not significantly differ from ear w

Bilateral microtia reconstruction.

BACKGROUND: Ear reconstruction for congenital microtia is a challenge for the plastic and reconstructive surgeon. Ten percent of microtia cases are bilateral. However, the published literature contains relatively little information about auricular reconstruction in bilateral microtia. Some authorities prefer to reconstruct each side at different stages. In this article, we introduce an operative method to reconstruct both sides simultaneously. This is completely feasible, and saves time and cost. Furthermore, this method allows comparison between sides during surgery, and facilitates carving o

Eustachian tube patency and function in tympanoplasty with cartilage palisades or fascia after cholesteatoma surgery.

OBJECTIVE: To compare the Eustachian tube patency and function after tympanoplasty with either cartilage palisades or fascia grafting after one-stage surgery in children with tensa cholesteatoma. STUDY DESIGN: In children operated for tensa cholesteatoma, cartilage palisade tympanoplasty was performed in 32 ears and fascia tympanoplasty in 29 ears. The patency of the Eustachian tube was evaluated by the Valsalva maneuver before the operation, 1 to 3 months after the operation, and at a follow-up examination 46(1/2) months later (median). Eustachian tube function was evaluated by the nine-step

Nuances of otoplasty: a comprehensive review of the past 20 years.

Otoplasty for the correction of the prominent ear is a heavily debated topic in Facial Plastic Surgery. This article presents the past 20 years of literature on the topic in a concise and organized manner. The greatest area of focus is on the finer nuances between cartilage-sparing and cartilage-incising techniques. In addition, some of the latest research on anesthesia techniques, nonoperative approaches, and social issues are discussed.

Complete Ear Reconstruction With Pre-laminated Radial Artery-Based Free Flap in Old Traumatic Amputation.

Total ear reconstruction in old avulsion injuries can be extremely challenging for reconstructive surgeons due to regional scarring. This rules out the use of local flaps. In this case report, a young woman presented to us with a seven-year-old total amputation of the left ear, which was initially reconstructed with a Medpor implant and occipito-parietal fascial flap. The implant had to be later explanted due to infection. The authors decided to use a pre-laminated radial free flap with a costal cartilage graft framework for ear reconstruction.

Petrified ears in a patient with Keutel syndrome: temporal bone CT findings.

We present unusual imaging findings of petrified ears in a 9-year-old girl with Keutel syndrome. The patient presented for a temporal bone study for hearing loss. CT scan showed middle and inner ear abnormalities along with extensive and unsuspected calcification of the external ears and ossicular ligaments. On further investigation, the patient was found to have diffuse cartilage calcification in the larynx and tracheobronchial tree, brachytelephalangism and peripheral pulmonary stenosis suggestive of Keutel syndrome. Confirmation was obtained by mutation analysis.
