Results of a German national survey to assess early mobilization versus bed rest in the postoperative management of incidental durotomy during lumbar surgery and the need for a randomized clinical trial.

OBJECTIVE: Incidental durotomy during degenerative lumbar spine surgeries poses a complex medical challenge. Despite its relatively common occurrence, consensus on its management remains elusive due to the absence of robust, high-level evidence, resulting in significant variations in practice across medical institutions. The aim of this survey was to assess the current practices in Germany regarding early mobilization versus bed rest in the postoperative management of incidental durotomy during lumbar surgery to estimate the need for a randomized clinical trial. METHODS: A digital survey titl

The grade of instability in fragility fractures of the pelvis correlates with impaired early mobilization.

PURPOSE: This study aimed to investigate whether gait patterns of patients with fragility fractures of the pelvis (FFP) comply with the grade of fracture instability, defined by radiological patterns. PATIENTS AND METHODS: This prospective, single-center, observational study included 39 patients with an FFP. Gait analysis was performed with a wearable insole force sensor (Loadsol® by Novel, Munich, Germany) 4-7 days after admission. Patients were divided in two groups: Group A included FFP type 1 fractures, which affect the anterior pelvic ring only, Group B contained FFP type 2-4 fractures w

Morphomechanical alterations in the medial gastrocnemius muscle in patients with a repaired Achilles tendon: Associations with outcome measures.

BACKGROUND: Functional deficits are found in ankles that have sustained an Achilles rupture. This study sought to evaluate and compare the morphomechanical characteristics of the medial gastrocnemius muscle in the legs of participants within six months of a unilateral Achilles repair to determine any correlations between those characteristics and objective outcomes and self-reported functional levels. METHODS: Fifteen participants were assessed via measurements of muscle morphologies (fascicle length, pennation angle, and muscle thickness) in a resting state, the mechanical properties of the

[Stroke therapy. What is established, what is new?].

Early treatment of a stroke, for example, on a stroke unit reduces neurological deficits and decisively improves survival. Of initial importance is an optimal supply of oxygen. Fever should be rapidly reduced, and blood sugar within the normal range also improves the prognosis. Only when blood pressure is very high is--moderate--lowering recommended. On the contrary, raising the blood pressure and cardiac output may have a positive effect on cerebral bloodflow with fewer neurological deficits resulting. In the case of immobile patients or patients with higher-grade paralysis, antithrombotic me

Epidural anaesthesia and analgesia - effects on surgical stress responses and implications for postoperative nutrition.

BACKGROUND: Surgical injury leads to an endocrine-metabolic and inflammatory response with protein catabolism, increased cardiovascular demands, impaired pulmonary function and paralytic ileus, the most important release mechanisms being afferent neural stimuli and inflammatory mediators. RESULTS: Epidural local anaesthetic blockade of afferent stimuli reduces endocrine metabolic responses, and improve postoperative catabolism. Furthermore, dynamic pain relief is achieved with improved pulmonary function and a pronounced reduction of postoperative ileus, thereby providing optimal conditions f

Ilizarov treatment of pilon fractures.

The authors retrospectively reviewed 13 pilon fractures in 12 patients treated with a diaphyseal-epiphyseal technique of Ilizarov external fixation. Clinical followup averaged 16 months (range, 12-25 months). Eighty-four percent of the fractures had healed within 16 weeks after surgery. Average range of motion of the ankle was 12 degrees dorsiflexion and 25 degrees plantar flexion. One delayed union was treated with bone grafting and eventually healed; 1 nonunion was treated with ankle arthrodesis. Most patients (91%) experienced mild or no pain. There were no deep infections. This technique i

A prospective investigation of the healing of grafted pretibial wounds with early and late mobilisation.

The aims of this study were to determine whether early mobilisation delays wound healing after meshed split skin graft repair of pretibial wounds and to identify other factors delaying the healing of these wounds. A prospective randomised controlled trial was conducted to compare the time to complete healing of patients mobilised early (the first postoperative day) against those who mobilised late (the tenth postoperative day). There was no difference in time to complete healing. Patients with a history of oral steroid therapy had a mean delay in wound healing of approximately 8 days (p < 0.02

Experience in posterior lumbar interbody fusion: unicortical versus bicortical autologous grafts.

Over the past seven years, 96 PLIF operations were performed with the autologous bone graft method. In 44 cases, unicortical bone grafts were obtained from the posterior iliac crest area. In the remaining 52 cases, bicortical bone grafts were obtained from the anterior iliac crest. The failure rate in the patients treated by interbody fusion with the unicortical posterior iliac crest graft was 11.8%. The failure rate in patients treated by interbody fusion with bicortical bone grafts was only 1.9%. The patients were ambulatory three to four days after the operation. In cases of spondylolysis a

An update on the early management of traumatic paraplegia (nonoperative and operative management).

Both the spinal cord physician and the spinal surgeon must be fully cognizant of the alterations of functions in multiple systems of the body caused by a spinal cord injury. Complications can easily arise, particularly within the respiratory, urinary, and integumentary systems. Most are preventable if the medical and nursing staff are sufficiently knowledgeable to anticipate them and if the necessary equipment and facilities are available. Regionalization of care with early referral to a spinal cord injury center has become a cost-effective way to manage these patients. Only if their medical n

Statistical Genetics of DMD Gene Mutations in a Kazakhstan Cohort: MLPA/NGS Variant Validation and Genotype-Phenotype Modelling.

Background: Duchenne muscular dystrophy (DMD) results from pathogenic variants in the DMD gene, one of the most significant and most mutation-prone genes in the human genome. Although global mutation registries are well developed, genetic data from Central Asian populations remain extremely limited, leaving essential gaps in regional epidemiology and in the understanding of genotype-phenotype patterns. Methods: We conducted a retrospective analysis of patients with genetically confirmed dystrophinopathy in Kazakhstan. Variants were identified using multiplex ligation-dependent probe amplificat

Dry Beriberi During Pregnancy After Hyperemesis Gravidarum: A Case Report of Axonal Sensorimotor Polyneuropathy.

Thiamine (vitamin B1) deficiency is a preventable complication of prolonged hyperemesis gravidarum, and the resulting malnutrition may lead to disabling peripheral neuropathy. We report the case of a 25-year-old primigravida who presented at 20 weeks of gestation with rapidly progressive distal weakness and glove-and-stocking paresthesias. Electroneuromyogram testing revealed an axonal sensorimotor polyneuropathy, and the serum thiamine level was low, supporting the diagnosis of thiamine deficiency-related neuropathy (dry beriberi). High-dose intravenous thiamine resulted in partial neurologic

Zickel nail fixation of bilateral subtrochanteric fractures.

The introduction of the Zickel nail has provided a significant advance in the treatment of subtrochanteric fractures. The device allows early mobilization and a high rate of union. In a 24-year-old man, the device was used bilaterally for subtrochanteric fractures, thus obviating the need for prolonged hospitalization and promoting early ambulation, and return to work. It must be emphasized that a bilateral operation for subtrochanteric fracture is a formidable procedure but can be accomplished in an otherwise healthy individual.

Safety recommendations on laboratory and blood count values for early mobilization in critically ill adults: an expert consensus.

Although early mobilization is a cornerstone of modern critical care for reducing intensive care unit-acquired weakness, the duration of mechanical ventilation, and length of stay, its implementation remains highly variable due to the absence of standardized laboratory criteria to guide clinical decision-making. Therefore, this study aimed to generate safety recommendations regarding laboratory and blood count values to be considered prior to initiating early mobilization in critically ill adults, both with and without invasive supports, across different clinical conditions. To address this ev

Outcomes of an accelerated nonsurgical management protocol for hip fractures in the elderly.

The majority of hip fractures in elderly patients are managed surgically with superior outcomes. However, for patients who refuse surgery or are deemed medically unfit, traction used to be the mainstay of nonsurgical treatment, which is associated with prolonged hospitalization and inpatient complications from immobility. This study, therefore, aims to evaluate the outcomes of an early wheelchair mobilization protocol as an alternative nonsurgical treatment option. This is a retrospective study of 87 elderly patients who were managed nonsurgically for their hip fractures over a 1-year period.
