Sudden unexpected death in epilepsy (SUDEP) is probably caused by periictal cardiorespiratory alterations such as central apnea, bradyarrhythmia, and neurogenic pulmonary edema. These alterations may occur in people with epilepsy and vary in duration and severity. Seizure-related ventricular tachyarrhythmias have also been hypothesized to be involved in SUDEP, but compelling evidence of these, or of predisposition to these, is lacking. Ventricular tachyarrhythmias are facilitated by pathologic cardiac repolarization. Electrocardiography (ECG) indicators of pathologic cardiac repolarization, su

Understanding of the causes of pulmonary oedema must be based on knowledge of the mechanism responsible for fluid exchange between the several compartments of the normal lung. Recent physiological studies have clarified the main features of these mechanisms. However in three areas knowledge is still incomplete--the magnitude of the hydrostatic and oncotic forces responsible for fluid movement within the lung, the means by which protein leaks across the wall of small pulmonary vessels and the routes by which fluid and protein pass between the interstitial tissues of the lung and the alveolar sp

We describe a case of a 32-year-old man who died due to bilateral re-expansion pulmonary edema (RPE) following the insertion a chest tube for unilateral spontaneous pneumothorax. Fifteen minutes after inserting the chest tube, the patient with right spontaneous pneumothorax was diagnosed with right re-expansion edema by chest radiograph. Although multiple treatments were administered, the patient died. However, the findings from autopsy showed bilateral RPE existed in the decedent but not unilateral RPE. Autopsy, microscopic examination, and clinical records concluded that the cause of death w

Renal vascular hypertension (RVHT) is one of the most common secondary forms of hypertension. It is estimated that 1% to 5% of all cases of hypertension can be attributed to RVHT. RVHT is generally caused by progressive stenosis of the renal artery most often due to atherosclerosis, and less often caused by fibromuscular dysplasia. Atherosclerotic renal artery stenosis (ARAS) can lead to the development of resistant hypertension and can also cause progressive impairment of renal function. ARAS can also result in serious cardiac complications, such as flash pulmonary edema or congestive heart f

Steroids are used for specific indications in the perioperative period to reduce laryngeal or spinal cord edema, or for prophylaxis and treatment of postoperative nausea and vomiting. Given the other potential causes for hemodynamic alterations in the perioperative setting, it is important for physicians to be aware of cardiovascular side effects of short term steroids. Changes in blood pressure and heart rate, cardiac dysrhythmias, and even death have been described in patients receiving short term intravenous steroids. Bradycardia has been reported following short term methylprednisolone and

Myocardial infarction (MI) is a life-threatening disease with high morbidity and mortality, closely associated with immune-inflammatory responses. As essential pathways for immune cell clearance and interstitial fluid drainage, lymphatic vessels are critical in regulating tissue fluid homeostasis and systemic immune surveillance. Cardiac lymphatics interact with immune cells, directly and indirectly, to mediate post-MI inflammation, participate in the clearance of necrotic tissue, and contribute to cardiac remodeling. Studies indicate that after MI, promoting cardiac lymphangiogenesis can acce

We present a case of a 48-year-old female who developed myocarditis and near fatal arrhythmias during high dose Il-2 therapy for metastatic renal cancer. On day 5 of therapy, the patient developed sudden onset chest pain, elevated cardiac enzymes and ST segment changes on EKG. Coronary angiogram was normal, however echocardiogram showed reduced ejection fraction and hemodynamic measurements showed elevated bilateral elevated filling pressures. The patient then developed episodes of recurrent ventricular arrhythmia, precipitated by bradycardia and PVC, requiring defibrillation and temporary pac

BACKGROUND: Swimming-induced pulmonary edema (SIPE) is an underrecognized cause of acute dyspnea in healthy athletes. Risk factors include older age, female sex, asthma, and underlying cardiopulmonary disease.
CASE SUMMARY: A 49-year-old man developed acute dyspnea during the swimming portion of a triathlon. On presentation, he exhibited hypoxemia, crackles, and radiographic pulmonary edema. Troponin elevation prompted further cardiac imaging, revealing preserved left ventricular function and focal acute myocardial edema. He was diagnosed with SIPE with concomitant myocardial injury; his cond

Radiology plays an important role in veterinary cardiology, along with other methods, such as electrocardiography, echocardiography, and biomarkers, in the diagnosis of cardiac diseases. Particularly, thoracic radiography is often used to assess heart size and for confirmation of left-sided congestive heart failure by the presence of pulmonary edema. Many cardiac diseases in dogs are associated with a heart size increase, especially in more advanced stages, and cardiac size assessment has been of great interest for a long time for both clinicians and researchers. A substantial number of method

Body fluid volume regulation by the kidney relies upon the complex interaction of numerous factors. However, in edematous disorders, extrarenal factors can override the 'innate wisdom' of the kidney. For example, in patients with cardiac failure or liver disease and in pregnant women, the normal kidney continues to retain sodium and water despite expanded blood, plasma and extracellular fluid volumes. Such fluid retention can ultimately lead to pulmonary congestion, ascites or peripheral edema. Understanding the kidney's modulation of total body sodium and water in these patients has been perp

We report the case of a 32-year-old male with Chlamydia trachomatis infection and admitted with chest pain, signs of myocardial damage and coronary arteries free from significant atherosclerotic disease. Cardiac magnetic resonance imaging (MRI) documented imaging patterns of myocardial involvement suggestive of acute myocarditis, and repeated cardiac MRI examinations were used to define appropriate clinical management of the patient. In particular, the decision to submit the patient to an additional antibiotic course was based on evidence of persisting myocardial edema, while no further treatm

Generalized myoclonus status is common in comatose patients after cardiac resuscitation, but its prognostic value is uncertain. We studied the clinical, radiologic, and pathologic findings in 107 consecutive patients who remained comatose after cardiac resuscitation. Myoclonus status was present in 40 patients (37%). Features more prevalent in patients with myoclonus status were burst suppression on electroencephalograms, cerebral edema or cerebral infarcts on computed tomography scans, and acute ischemic neuronal change in all cortical laminae. All patients with myoclonus status died. Of 67 p

A rare accumulation of serous ascites following traumatic cardiac rupture is reported for a 21-year-old man transferred to the emergency center of our hospital from a community hospital suffering from severe shock due to a motorcycle accident. Computed tomography scan showed moderate pericardial and intrapelvic fluid accumulation strongly suggesting cardiac and visceral injuries. An emergency sternotomy disclosed a rupture of the right atrial appendage, successfully closed with primary sutures. A laparotomy was done to aspirate moderate serous fluid, which was clear and not bloody. Edema of th

BACKGROUND: Metastatic cardiac lymphoma is rare and may present as a large mass with a high risk of hemodynamic compromise and poor outcomes.
CASE SUMMARY: A 79-year-old woman who had a history of thyroid mucosa-associated lymphoid tissue (MALT) lymphoma and achieved remission after radiotherapy presented with dyspnea and edema. Transthoracic echocardiography and computed tomography revealed a large mass occupying the right atrium and ventricle. Biopsy confirmed diffuse large B-cell lymphoma, suggesting a late transformation of MALT lymphoma with cardiac involvement. Low-dose chemotherapy was

BACKGROUND: Since constrictive pericarditis is most often idiopathic and the pathophysiology remains largely unknown, both the diagnosis and the treatment can be challenging. However, by definition, inflammatory processes are central to this disease process. Amniotic membrane patches have been shown to possess anti-inflammatory properties and are believed to be immune privileged. Due to these properties, amniotic membrane patches were applied intraoperatively in a complicated patient presenting with constrictive pericarditis.

INTRODUCTION: Cor triatriatum is a congenital cardiac anomaly in which the left (sinister) or right (dexter) atrium is divided into two compartments by residual embryonic tissue, resulting in a tri-atrial heart. As cor triatriatum dextrum can present clinically in various ways and have multiple associated cardiac anomalies, this report attempts to contribute to what is known about this exceedingly rare disorder.

Diabetic ketoacidosis in children is associated with a wide gamut of complications, which can be either disease- or therapy-related. Common complications in children with DKA include cerebral edema, acute kidney injury, hypokalemia, hypoglycemia, etc. As opposed to asymptomatic hypophosphatemia, which is common during the management of DKA, severe symptomatic hypophosphatemia leading to life-threatening complications like rhabdomyolysis, hemolysis, respiratory failure secondary to muscle weakness, and cardiac complications are rare. The authors present a case of a newly diagnosed T1DM patient

Sodium and water retention in cardiac failure and cirrhosis is pivotal in the morbidity and mortality of patients with these disorders. Moreover, the pathophysiology of these edematous disorders is quite similar. Both disorders have activation of the renin-angiotensin-aldosterone system, increased sympathetic activity, and nonosmotic stimulation of arginine vasopressin, which is initiated by unloading of the arterial baroreceptors; this occurs secondary to diminished cardiac output with heart failure and primary systemic arterial vasodilation with cirrhosis. With this common pathophysiology ca

A model of heterotopic cardiac transplantation for diagnosis of rejection is described. Heterotopic cardiac transplantations were performed in the thorax using the left innominate artery as an arterial supply with venous return into the superior vena cava. Six pairs of mongrel dogs underwent cardiac transplantation using this technique. Two dogs died postoperatively on the 2nd and 3rd day due to respiratory failure. Another four donor hearts arrested their beats in 6 to 8 postoperative days (mean 6.3 days) resulting from acute cardiac rejection. Serial echocardiographic recordings were found t

Peripheral vasodilators nitroglycerin and sodium nitroprusside were given to 29 patients in the treatment of acute circulatory insufficiency (10 of these patients had pulmonary edema) which in most of them was caused by acute myocardial infarction. Diastolic pressure in the pulmonary artery, which was measured by means of a Swan-Ganz balloon catheter, was above 18 mm Hg (18--34 mm Hg) in all patients. Sodium nitroprusside and nitroglycerin given sublingually caused rapid normalization of diastolic pressure in the pulmonary artery and reduction of arterial pressure with no essential changes in
