Anatomic, Functional, and Mechanical Risk Factors for Elbow Injury in the Throwing Athlete: A Prospective Cohort Study of 128 High School Baseball Players.

BACKGROUND: Among high school baseball players, elbow injury is a significant career-threatening problem. Although structural abnormalities of the elbow joint, poor physical function, and faulty pitching mechanics have been identified as potential risk factors for this disorder, their relationship has not been fully elucidated. PURPOSE: To determine the interaction among these risk factors and, in particular, to examine whether structural abnormalities of the elbow joint are an important risk factor for elbow injury. STUDY DESIGN: Cohort study; Level of evidence, 2. METHODS: A total of 128

Management of a Rare Complex Triad Injury of the Elbow Joint: Radial Head Fracture, Triceps Tendon Avulsion, and Rupture of the Medial Collateral Ligament.

BACKGROUND: Isolated triceps tendon injuries are infrequent, and their combination with a medial ligament avulsion is even rarer. Eccentric force on the triceps tendon following a fall on the outstretched hand, combined with a valgus torque at the elbow joint, exposes the medial side to stretching forces, potentially disrupting the ulnar collateral ligament (UCL). Furthermore, compressive forces on the lateral side could result in injury to the radial head. Currently, there is no standardized surgical approach for managing such combined injuries. INDICATIONS: The complex triad injury presente

Do technical components of microanastomoses influence the functional outcome of free gracilis muscle transfer for elbow flexion in traumatic brachial plexus injury?

INTRODUCTION: The traumatic lesions of the brachial plexus in adults are devastating injuries causing continuous severe functional impairment for both work and daily living activities. The restoration of elbow flexion is one of the most important movements for patient recovery to previous activities. Free gracilis muscle transfer has good outcomes for cases with late presentation or as a rescue surgery to regain elbow flexion, however, bad results are present in all cohorts with insufficient recovery of muscle strength for elbow flexion. A number of hypotheses can be postulate to explain the f

A comparison of intercostal and partial ulnar nerve transfers in restoring elbow flexion following upper brachial plexus injury (C5-C6+/-C7).

PURPOSE: Restoring active elbow flexion is essential in the surgical management of C5-C6 +/- C7 brachial plexus palsies. This study compares the clinical results of 2 techniques to restore elbow flexion: the partial ulnar nerve transfer and the intercostal nerve transfer. METHODS: Partial ulnar nerve transfer was performed in 23 patients, and intercostal nerve transfer was performed in 17 patients. For both techniques, the transfer to the musculocutaneous nerve was made at the same anatomical point. Age and preoperative delay were comparable between groups of patients. RESULTS: Biceps reinne

An unprecedented pan-brachial injury: A case report of ipsilateral floating shoulder, floating elbow, and acetabular fracture following a psychosis-induced fall.

INTRODUCTION: High-energy trauma can result in complex orthopedic injury patterns. The "floating shoulder" and "floating elbow" are distinct, severe injuries involving serial fractures of the upper extremity, disrupting adjacent joint function. The simultaneous ipsilateral occurrence of both, further complicated by a pelvic fracture, represents an exceptionally rare and challenging clinical scenario. CASE PRESENTATION: We present the case of a 45-year-old male with a history of schizophrenia who sustained multiple fractures after a fall from a height during a psychotic episode. He was diagnos

Bilateral simultaneous distal radius fractures with elbow dislocations following a fall from height: A rare orthopedic quadruple injury with review of the literature.

INTRODUCTION: Patellar tendon ruptures are a relatively common injury encountered by orthopedic surgeons and typically only occur unilaterally. However, there are rare reports of bilateral patellar tendon ruptures occurring simultaneously, in patients with underlying systemic disorders, higher energy mechanisms, or injury or overuse in high-level athletes. When patellar tendon ruptures occur, and the extensor mechanism is disrupted, patellar tendon repair versus reconstruction is warranted to restore functionality. The use of dermal allografts for the reconstruction of chronic patellar tendon

Anterolateral Thigh Flap for Low-Voltage Fourth-Degree Electrical Burn Injury With Immediate Radial Nerve Palsy at the Elbow: A Case Report.

While high-voltage electrical injuries usually cause severe burn wounds and axonal polyneuropathy, low-voltage electrical injuries cause limited cutaneous wounds and demyelinating mononeuropathy, of which the median and ulnar nerves are the most commonly involved. We present the case of a 42-year-old man who suffered a 480-voltage electrical injury at his right elbow, resulting in a 24 × 10 cm fourth-degree burn wound and immediate radial nerve palsy. The burn wound was debrided with confirmation and preservation of radial nerve continuity. The wound was covered with a free anterolateral thigh

Factors Affecting Outcomes After Free Functional Gracilis Muscle Transfer for Elbow Flexion in Brachial Plexus Injury: A Systematic Review and Meta-Analysis.

PURPOSE: Free functional gracilis transfer (FFGT) is a useful option for reconstruction of elbow flexion following brachial plexus injury presenting late or with poor outcomes from previous nerve surgery. In this systematic review and meta-analysis, we aimed to investigate variables associated with superior outcomes. The efficacy of single versus double FFGT, where the first FFGT is performed to restore elbow flexion, and the choice of donor nerve for neurotization were evaluated. METHODS: A meta-analysis was conducted, including studies that provided postoperative Medical Research Council (M

Effect of shorter nerve graft and selective motor branch of recipient nerve on nerve transfer surgery for elbow flexion in patients with brachial plexus injury.

OBJECTIVE: The spinal accessory nerve (SAN) is commonly used as a donor nerve for reinnervation of elbow flexors in brachial plexus injury (BPI) reconstruction. However, no study has compared the postoperative outcomes between SAN-to-musculocutaneous nerve (MCN) transfer and SAN-to-nerve to biceps (NTB) transfer. Thus, this study aimed to compare the postoperative time to recovery of elbow flexors between the two groups. METHODS: A total of 748 patients who underwent surgical treatment for BPI between 1999 and 2017 were retrospectively reviewed. Among them, 233 patients were treated with nerv

Radial head arthroplasty vs. open reduction and internal fixation for the treatment of terrible triad injury of the elbow: A systematic review and meta-analysis update.

Terrible triad injury of the elbow (TTIE) is a severe high-energy injury to the elbow, mainly including elbow dislocation, coronoid fracture and radial head fracture. It is difficult to maintain the stability of the elbow joint using traditional conservative treatment, and there is a high risk of redislocation and various complications. Therefore, surgical treatment is currently advocated, mainly for repairing damaged ligaments and reconstructing bony structures, but there is still controversy about the treatment plan for the radial head. The current meta-analysis was conducted to compare the

[Type V floating elbow with type III A3 exposure and nerve injury. Initial and definitive treatment with a minimally invasive technique. A case report].

BACKGROUND: The increased number of high energy accidents has led to the occurrence of several injuries in a single extremity, particularly among youths. Stanitski and Micheli defined the floating elbow as a forearm fracture plus an ipsilateral supracondylar fracture. Its incidence ranges from 3% to 13%, predominant ages are 7 to 11 years, with a 2:2 male:female ratio. The mechanism of injury is as follows: fall from a height with elbow hyperextension and wrist dorsiflexion and pronation of the forearm. OBJECTIVE: To present the case of a child with a type V left open floating elbow injury, s

Open Reduction and Internal Fixation Versus Resection of the Radial Head to Treat Terrible Triad Injury of the Elbow: What if Radial Head Replacement is not Possible?

BACKGROUND: Terrible triad of elbow injury (TTI) has great potential for joint instability with a relatively poor prognosis. Different surgical methods have been introduced for radial head fractures. In this study, we aimed to evaluate the results of treatment of radial head fractures with open reduction and internal fixation (ORIF) and resection in TTI patients when radius head replacement was not attainable. MATERIALS AND METHODS: In this retrospective study, 48 patients with TTI who underwent radial head fracture surgery with ORIF (n = 33) or resection (n = 15) were included. During follow

Work Disabling Nerve Injury at Both Elbows Due to Laptop Use at Flexible Workplaces inside an Office: Case-Report of a Bilateral Ulnar Neuropathy.

BACKGROUND: This case report describes whether a female civil servant who developed bilateral ulnar neuropathy can be classified as having an occupational disease. METHODS: The Dutch six-step protocol for the assessment and prevention of occupational diseases is used. RESULTS: Based on the six-step protocol, we propose that pressure on the ulnar nerve in the elbow region precipitated the neuropathy for this employee while working prolonged periods in elbow flexion with a laptop. CONCLUSION: Despite the low incidence laptop use might be a risk factor for the occurrence of ulnar neuropathy du

Concurrent musculoskeletal complaints in elbows, shoulders, and necks after common hand and forearm injuries or conditions: A cross-sectional study among 600 patients.

STUDY DESIGN: This is a cross-sectional study among 600 patients. INTRODUCTION: Isolated hand and forearm injuries or conditions are common in the emergency and orthopedic departments. So far, little is known about whether these patients suffer from concurrent musculoskeletal complaints (MSCs) besides their hand and forearm complaints. Neglecting concurrent MSCs in the upper limbs and necks could hamper rehabilitation and prolong the time taken to return to daily and work-related activities. PURPOSE OF THE STUDY: The purpose of this study was to investigate the prevalence of concurrent MSCs
