Robot-assisted pancreatoduodenectomy with preservation of the vascular supply for autologous islet cell isolation and transplantation: a case report.

INTRODUCTION: For patients with chronic pancreatitis presenting with medically intractable abdominal pain, surgical intervention may be the only treatment option. However, extensive pancreatic resections are typically performed open and are associated with a substantial amount of postoperative pain, wound complications and long recovery time. Minimally invasive surgery offers an avenue to improve results; however, current limitations of laparoscopic surgery render its application in the setting of chronic pancreatitis technically demanding. Additionally, pancreatic resections are associated wi

Partial pancreatic resection along the embryological fusion plane - no longer a fantasy.

BACKGROUND: The embryological connection between the dorsal and ventral pancreatic regions divides the pancreas into two segments. This anatomical dependence allows segmental pancreatic resection through the embryological fusion plane (EFP). The advantages of limited pancreatic resection are the preservation of the natural continuity and function of the gastrointestinal tract and the avoidance of the metabolic and endocrine consequences of total resection of the pancreas and the duodenum. MATERIALS AND METHODS: Two patients are described who underwent anatomicalsegmentectomyof the pancreatic

Successful pregnancy after mucinous cystic neoplasm with invasive carcinoma of the pancreas in a patient with polycystic ovarian syndrome: a case report.

BACKGROUND: The incidence of invasive cancer within a mucinous cystic neoplasm of the pancreas varies between 6 and 36%. Polycystic ovarian syndrome is a disorder characterized by hyperandrogenism and anovulatory infertility. One surgical treatment that can restore endocrine balance and ovulation in polycystic ovarian syndrome is partial ovarian destruction. Successful pregnancies following preconception pancreaticoduodenectomies (Whipple procedures) and chemoradiation to treat pancreatic neoplasms have been reported rarely but none were diagnosed with pre-cancer polycystic ovarian syndrome-as

Surgery for chronic pancreatitis--extended pancreaticojejunostomy.

BACKGROUND/AIMS: The role of Frey's operation as extended pancreatic duct drainage operation was evaluated in patients with chronic pancreatitis. METHODOLOGY: 206 patients with chronic pancreatitis were surgically treated, evaluated and followed for a minimum period of 6 months, in order to ascertain the status of pancreatic function and pain relief for both pancreatic duct drainage operations, including Frey's operation, and pancreatectomy. RESULTS: Pain relief was established in 62 out of 86 patients (72.1%) undergoing pancreas duct drainage operation, in 51 out of 65 patients (78.5%) unde

The role of the phrenic nerves in stress response in upper abdominal surgery.

Previous studies have failed to demonstrate a block of the endocrine response to upper abdominal surgery by thoracic epidural analgesia. To clarify the bases for this failure, we compared the effects of epidural analgesia of different dermatome levels up to C8-T2 or C3-4. The patients who received general anesthesia alone showed significant increases of adrenocorticotropic hormone (ACTH) and arginine vasopressin (AVP) immediately after skin incision. The patients with C8-T2 blocked developed significant increases in these hormones, not after the skin incision, but after the intraabdominal proc

Pancreatic fistula after pancreaticoduodenectomy: the conservative treatment of choice.

BACKGROUND: A pancreatic fistula (PF) is the most common complication after pancreaticoduodenectomy (PD), and its reported incidence varies from 2% to 28%. The aim of the present study was to analyse the treatment of a complicated PF comparing the surgical approach with conservative techniques. METHODS: From January 2000 through to August 2006, 121 patients were submitted for PD. The study consisted of 70 men and 47 women, with a median age of 60 years (SD +/- 12). The main indications for PD were pancreatic duct carcinoma in 52 patients (44.5%), ampullary carcinoma or adenoma in 18 (15.4%) a

Three-Port Transoral Robotic Thyroidectomy: Technical Description and Early Outcomes.

AIM: Transoral robotic thyroidectomy has emerged as a scarless alternative to conventional and remote-access approaches; however, technical standardization and outcome data for truly scarless three-port configurations remain limited. The aim of this study was to describe the surgical technique and early clinical outcomes of three-port transoral robotic thyroidectomy (TORT) as a truly scarless remote-access approach to the thyroid gland. METHODS: This technical note reports a consecutive series of 25 patients who underwent three-port transoral robotic thyroidectomy between June 2017 and May 20

Surgical Approaches to the Hypothalamus: A Systematic Review of Anatomical Corridors and Postoperative Outcomes.

BACKGROUND: The hypothalamus is a vital neuroanatomical structure involved in endocrine, autonomic, and homeostatic regulation. Its deep location near critical neural and vascular structures makes surgical access complex. This review aims to evaluate and compare these surgical approaches, focusing on anatomical access and their clinical and functional outcomes. METHODS: This review was conducted following Preferred Reporting Items for Systematic Reviews and Meta-Analyses 2020 guidelines. A comprehensive search of PubMed and Scopus was performed for studies published between January 2000 and J

A pancreas-preserving technique for the management of symptomatic pancreatic anastomotic insufficiency refractory to conservative treatment after pancreas head resection.

BACKGROUND AND AIM: Management of symptomatic pancreatic anastomotic insufficiency after pancreas head resection remains controversial. Completion pancreatectomy as one frequently performed option is associated with poor prognosis. PATIENTS AND METHODS: During a 4-year period, a two-step strategy was applied in four consecutive patients suffering from pancreatic anastomotic insufficiency refractory to conservative management after a pancreas head resection. In the first step, sepsis was overbridged by meticulous debridement and resection of the pancreaticojejunostomy, leaving the biliary anas

Somatostatinoma: a case report and review of the literature.

Somatostatinomas are rare endocrine tumors that were first described in 1977. In addition to the present case report, there have been 31 cases reported in the literature. We have reviewed the literature to integrate the symptoms, physical findings, diagnostic tests, treatment, and length of survival of these patients. Although the symptoms that occurred in the majority of cases were those that are seen in most patients with intra-abdominal neoplasms, symptoms relating to the presence of excess circulating somatostatin--diabetes, maldigestion, and cholelithiasis--were frequently seen. Physical

From 1971 to 1994, 16 cases of total colon Hirschsprung's disease were treated at the University Hospital in Rennes. Diagnosis have been at 2 days to 3 months. Two children had a family history of Hirschsprung disease among which one associated megacolon and multiple endocrine neoplasia. This family had a mutation of the RET proto oncogene. Six children died before complete surgical cure, among whom 4 before total parenteral nutrition. Six were treated according to Lester Martin, 3 according to Duhamel, and 1 to Swenson. Diarrhea and occlusions happened during the first postoperative years. No

Parathyroid surgery in children.

Parathyroid surgery in children is uncommon. Spontaneously occurring cases of hyperparathyroidism are almost always due to single-gland disease: however, on exploration all four parathyroid glands should be identified. Most of the other instances in which the surgeon needs to perform a parathyroidectomy on an infant or a child will be situations were multiple-gland disease is the rule rather than the exception. Therefore, the surgeon must have in his mind a well developed logical approach to the management of children with parathyroid disorders on the basis of multiple glandular disease. We be

The role of ultrasonography in treating polycystic ovary syndrome.

Repeated ultrasound imaging was performed in 23 cases of polycystic ovary syndrome (PCO). Diagnosis was established on the basis of the clinical picture, the endocrine parameters and on that of ultrasonographic findings. Almost all patients presented with oligomenorrhoea and also hirsutism, and obesity appeared in more than half of the cases. The hormonal picture was characterized by elevated LH and testosterone, by a higher than the upper limit of the normal range of oestradiol and low progesterone levels. In 18 cases, ultrasonography documented enlarged bilateral ovaries containing several s

Pulmonary complications associated with the treatment of patients with congenital cardiac disease: consensus definitions from the Multi-Societal Database Committee for Pediatric and Congenital Heart Disease.

A complication is an event or occurrence that is associated with a disease or a healthcare intervention, is a departure from the desired course of events, and may cause, or be associated with, suboptimal outcome. A complication does not necessarily represent a breech in the standard of care that constitutes medical negligence or medical malpractice. An operative or procedural complication is any complication, regardless of cause, occurring (1) within 30 days after surgery or intervention in or out of the hospital, or (2) after 30 days during the same hospitalization subsequent to the operation

Intramuscular Transplantation of Human Pluripotent Stem Cell-derived Pancreatic Endocrine Cells in Mice.

Human pluripotent stem cell (hPSC)-derived pancreatic endocrine cells constitute a promising source for diabetes cell therapy and disease modeling. Although several differentiation protocols have been established, in vivo implantation is commonly required to study maturation and function within a physiological context. Most transplantation approaches rely on sites such as the kidney capsule or subcutaneous space, which may pose technical or physiological limitations. Here, we present a detailed protocol for the intramuscular transplantation of hPSC-derived pancreatic SC-islets into immunodefic

Controversies in endoscopic ultrasonography-guided management of walled-off necrosis.

Walled-off necrosis (WON) develops as local complications after acute necrotizing pancreatitis. Although less invasive interventions such as endoscopic ultrasonography (EUS)-guided drainage and endoscopic necrosectomy are selected over surgical interventions, delayed and step-up interventions are still preferred to avoid procedure-related adverse events. However, there is a controversy about the appropriate timing of drainage and subsequent necrosectomy. The advent of large-caliber lumen-apposing metal stents has also brought about potential advantages of proactive interventions, which still n

Anesthetic Management for Kidney Transplant in a Young Patient With Mitral Regurgitation: Case Report.

Patients with chronic kidney disease who are candidates for transplant may experience changes in capillary permeability, coagulation, and the endocrine system; alterations in the pulmonary vasculature; and cardiac structural and functional changes. Patients with renal replacement by hemodialysis have a mortality rate 30 times higher than those who do not have uremia. According to the onset, duration, and severity of chronic kidney disease, cardiovascular disease will be reflected in baseline function and response to stress. For this reason, it is important to establish the functional condition
