Meniere's disease is the classic triad of fluctuating hearing loss, episodic vertigo, and severe tinnitus. It usually occurs unilaterally, although about 15% to 20% of patients may have bilateral disease. The disease involves a series of acute exacerbations followed by a period of remission; it progresses until the ear has essentially "burned itself out," causing symptoms to subside. Treatment of Meniere's disease should be tailored to the severity of symptoms and the patient's life-style. Initial treatment involves conservative medical management and psychologic support. If this fails, endoly

The etiopathogenesis of Meniere's disease has remained controversial since the early 1900s. Many investigators have studied the molecular and cellular pathology of the inner ear in patients with this disorder. Three basic pathologic mechanisms have emerged; fibrosis of the endolymphatic sac and vestibular epithelia, altered glycoprotein metabolism, and immune-mediated inner ear disease. This article reviews the current understanding of these three basic pathologic processes.

Proper development of the inner ear, including specification and differentiation of progenitors for the endolymphatic sac, is crucial for hearing and balance. In a mouse model of the SOX9Y440X/+ mutation that causes campomelic dysplasia and deafness, impaired development of the endolymphatic system at mid-gestation was associated with SOX10 downregulation. The underlying molecular causes and significance are unclear. Here, we found in Sox9Y440X/+ mice, Sox10 expression was downregulated, prior to overt differentiation, in the dorsomedial aspect of the E10.5 otic vesicle that normally becomes t…

It has been postulated that otosclerosis may produce vertigo by several mechanisms. One mechanism is by causing endolymphatic hydrops. We present six temporal bones in which otosclerosis and endolymphatic hydrops coexist. We consider that there is a spectrum-like interrelationship between these two entities. At one end of the spectrum the relationship is coincidental, while at the other end of the spectrum we consider the massive amount of active otosclerosis to be a causative factor in the development of the endolymphatic hydrops.

The purpose of this study was to clarify the underlying mechanism of vertiginous attacks in Ménière's disease (MD) while obtaining insight into water homeostasis in the inner ear using a new animal model. We conducted both histopathological and functional assessment of the vestibular system in the guinea-pig. In the first experiment, all animals were maintained 1 or 4 weeks after electrocauterization of the endolymphatic sac of the left ear and were given either saline or desmopressin (vasopressin type 2 receptor agonist). The temporal bones from both ears were harvested and the extent of endo…

Enlargement of the endolymphatic sac, duct, and vestibular aqueduct (EVA) is the most common inner ear malformation identified in patients with sensorineural hearing loss. EVA is associated with pathogenic variants in SLC26A4. However, in European-Caucasian populations, about 50% of patients with EVA carry no pathogenic alleles of SLC26A4. We tested for the presence of variants in CHD7, a gene known to be associated with CHARGE syndrome, Kallmann syndrome, and hypogonadotropic hypogonadism, in a cohort of 34 families with EVA subjects without pathogenic alleles of SLC26A4. In two families, NM_…

To investigate the postoperative anxiety and its relationship with life quality in patients with Ménière's diseases. Methods: A total of 68 patients with Ménière's disease, who received the treatment of endolymphatic sac decompression from 2010 to 2016, were enrolled. They finished two scales (the self-rating anxiety scale and the quality of life questionnaire for endolymphatic sac decompression of Ménière's disease) by snail mail. The patients were divided into different groups based on their preoperative course, postoperative follow-up time, clinic stage and quality of life after endolympha…

Saccotomy is an established surgical treatment for Meniere's disease that involves exposing and opening the endolymphatic sac and draining the endolymph. However, in patients experiencing recurrent vertigo after saccotomy, it is questionable whether revision saccotomy should be recommended as opposed to more invasive procedures, such as neurectomy of the vestibular nerve. To determine the utility of revision saccotomy, we conducted a retrospective study of 29 patients undergoing this procedure for recurrent vertigo. Patient charts were reviewed for the re-occurrence of any vertiginous symptoms…

Nystagmus, head-deviation, and hearing loss were provoked by Type I allergy induced in guinea pigs with passive sensitization and specific antigen challenge. Mast cell degranulation, edema of connective tissue of the endolymphatic sac and endolymphatic hydrops were histologically observed. These findings indicated that Type I allergy can be induced in the inner ear, and that symptoms analogous to Meniere's disease may be induced by this response.

Recently several investigations have been reported suggesting that the cause of endolymphatic hydrops might be an immunologic disorder of the endolymphatic space, including the endolymphatic sac. As the first choice in a conservative treatment by medication, the authors have used a combination of prednisolone and furosemide per os, which is rather safe and effective for hydrops patients in a subacute stage. However, some patients do not respond to this treatment or gradually become immune to this medication. With these patients, we have tried an intravenous administration of immunoglobulin G w…

A study was made of the anatomical structure and functional significance of the ultriculo-endolymphatic (UE) valve in man. The material cons … ly is to permit the occasional egress of excessive accumulation of endolymph to be processed in the endolymphatic sac and to accomplish this while preserving the normal endolymph volume and membrane anatomy of the utricle and canals.

In an attempt to develop an animal model of immune mediated Meniere's disease, we have injected lipopolysaccharide (LPS) directly into scala media of guinea pigs and monitored functional and morphological changes over a period of 6 weeks. Depending on the concentration of LPS, changes ranged from moderate-to-severe hearing loss and endolymphatic hydrops with minimal cellular infiltrate or fibrosis, to dense cellular infiltration that filled the scalae. Interestingly, higher concentrations of LPS not only induced severe cellular infiltration, hydrops, and hearing loss, but also a substantial en

The paravestibular canaliculus was studied by light microscopy in serial sections of the temporal bones from otosclerotic patients who underwent fenestration or stapes surgery. In all examined 23 specimens (13 cases), the endolymphatic duct and sac were observed to be normally developed with no pathological findings. The paravestibular canaliculus was found in 14 of the specimens (60.9%). Its course was traced from the proximal to the distal area in 12 specimens, and it appeared to merge into the distal portion of the endolymphatic sac in 7 of them. Twelve of the paravestibular canaliculi cont…

A critical review of the results of treatment of 125 patients with Meniere's disease by the endolymphatic subarachnoid shunt is presented. Of the patients, 61% experienced improvement in vertigo. Hearing stabilized or improved in the majority of cases. Detailed analysis of preoperative factors did not enable prediction of the likelihood of success in an individual case. The controversy regarding surgical therapy of Meniere's disease is discussed, and the conclusion that there is a role for endolymphatic sac surgery in the treatment of Meniere's disease is presented.
