[Metastasizing pancreatic vipoma. Its diagnosis and therapy with the somatostatin analog octreotide].

HISTORY: An 82-year-old woman was hospitalized for anaemia of 4.8 g/dl after having suffered for about one year from watery treatment-resistant diarrhoea, causing a weight loss of ca. 10 kg. FINDINGS: Computed tomography, magnetic resonance imaging and endosonography revealed a 2.5 x 2.0 cm space-occupying mass in the body of the pancreas. Taking into account electrolyte abnormalities (potassium 2.7 mmol/l), marked metabolic acidosis (pH 7.16, base excess -20.3 mmol/l) and achlorhydria, an increased serum concentration of vasoactive peptide (VIP) of 548.5 pmol/l confirmed a VIPoma. Somatostat…

[Rectal Cancer: Optimal Preoperative Diagnostics].

Preoperative diagnostics for rectal cancer aim to determine the extent of local and systemic spread. Local staging includes rectoscopy with accurate height localisation, histological confirmation, MRI of the pelvis and, particularly in the case of localised tumours, endosonography. In addition to tumour height and possible infiltration of adjacent organs, MRI findings should include minimum tumour distance from the mesorectal fascia and MR morphological criteria for extramural vascular invasion. In the case of lower rectal cancer, the relationship to the various components of the sphincter mus…

Resection of an anal fistula with endoscopic submucosal dissection.

A 34-year-old man presented with paroxysmal hypogastralgia during defecation for 2 weeks. Physical and laboratory examination findings were unremarkable, other than a depression located 1 cm above the dentate line, accompanied by mild tenderness and a clubbed induration extending to the rectum. Colonoscopy showed a 2.0×0.8 cm longitudinal, protruding mass in the posterior wall of the lower rectum. Endosonography revealed a mixed echogenic mass originating from the rectal submucosa, with no sign of muscular wall disruption. There was no evidence of Crohn's or other diseases. Following anorectal…

A "Back Light System" for Identification of Sites for Endoscopic Ultrasound-Guided Fine-Needle Aspiration in Solid Pancreatic Masses: A Prospective, Randomized Study with a Crossover Design.

BACKGROUND/AIMS: We applied a back light system (BLS) with a magnifying glass to improve the ability to assess the adequacy of specimen sampling using endosonography. We conducted this study to evaluate the efficacy of the BLS in sampling of specimens by endoscopic ultrasound-guided fine needle aspiration of solid pancreatic masses. METHODS: This was a prospective, randomized, crossover, single-center clinical trial. An endosonographer evaluated adequacy on gross visual inspection and identified whitish specimen sampling sites with and without the BLS according to a randomization sequence in …

A Classification of Procedural Complexity in Therapeutic Endoscopic Ultrasound: Rationale and Implications for Practice and Training.

BACKGROUND: Therapeutic endoscopic ultrasound (TEUS) has expanded into a broad procedural platform including access, delivery, measurement, ablation, drainage, and anastomoses. Nevertheless, TEUS lacks a shared procedural complexity classification. METHODS: Within an International initiative of the Next Generation EUS group, a dedicated task force developed a four-level classification, subsequently evaluated through Delphi methodology. The classification considered target stability and size, accessory exchange, adverse event profile, technical standardization, and prerequisite skills. To supp

Endoscopic ultrasound-guided choledochoduodenostomy (EUS-CDS) as first-line treatment for malignant distal biliary obstruction.

In patients with malignant distal biliary obstruction, drainage via Endoscopic Ultrasound-guided Choledochoduodenostomy (EUS-CDS) is indicated in clinical guidelines following a failed Endoscopic Retrograde Cholangiopancreatography (ERCP). However, current interest lies in evaluating its role as a first-line treatment. Through this letter, we present a case report that represents this clinical scenario, as well as a brief summary of the most recent literature supporting the use of EUS-CDS as a first-line treatment for malignant distal biliary obstruction.

Current update on the role of endoanal ultrasound: a primer for radiologists.

Endoanal ultrasound (EAUS) is a valuable imaging modality for the evaluation of anal and perianal pathologies. It provides detailed information about the anatomy and physiology of the anorectal region and has been used in pre-and post-operative settings of anorectal pathologies. EAUS is not only useful in the evaluation of benign pathologies but also in loco-regional staging of anal and rectal tumors. EAUS has several advantages over MRI, including reduced cost, better patient tolerance, and improved scope of application in patients with contraindications to MRI. Despite its benefits, EAUS is

Comparison of Three Methods of Gallbladder Drainage for Patients with Acute Cholecystitis Who Are at High Surgical Risk: A Network Meta-Analysis and Systematic Review.

Background: Percutaneous gallbladder drainage (PTGBD), endoscopic ultrasound-guided gallbladder drainage (EUSGBD), and endoscopic transpapillary gallbladder drainage (ETGBD) are used for the treatment of patients with acute cholecystitis who are at high surgical risk. However, it is unclear which procedure is associated with the best outcomes. Methods: We systematically searched records in PubMed, Embase, Web of Science, the Cochrane Central Register of Controlled Trials, and ClinicalTrials.gov up to March 1, 2020. Studies that compared at least two of PTGBD, ETGBD, and EUSGBD were included. R

Current Evidence on Endoscopic Biliary Drainage in the Era of Surgically Altered Anatomy: A Narrative Review.

Endoscopic retrograde cholangiopancreatography (ERCP) remains the reference standard for biliary drainage, yet it fails in up to 15% of cases, most notably when a surgically altered anatomy (SAA) is present. The expanding use of bariatric and oncologic gastrointestinal surgery has increased the number of patients in whom the papilla is displaced, unreachable, or replaced by a biliodigestive anastomosis, making conventional access difficult or impossible. This narrative review appraises the current evidence on endoscopic biliary drainage in SAA, focusing on procedural efficacy, anatomical pecul

International Consensus Statements on Endoscopic Ultrasound-Guided Biliary Drainage: Results From the 1st JGES International in 2025.

UNLABELLED: EUS-guided biliary drainage (EUS-BD) has evolved as an alternative to ERCP-BD for biliary obstruction, but its role as a primary drainage and its terminology have not been standardized. AIMS: To establish an international consensus on the indications, techniques, and standardized terminology of EUS-BD. METHODS: To address these, an international consensus meeting was held during the 1st JGES International. Fourteen expert endoscopists from six countries participated in a structured Delphi process, reviewing evidence and voting on clinical questions related to EUS-BD indications,

Reply to "Matters Arising: a simple method for diagnosing gallbladder malignant tumors with subserosa invasion by endoscopic ultrasonography".

The reader noted that a deep hypoechoic area is the main echoic indication of subserosa (SS) invasion of malignant gallbladder tumors. In this study, we evaluated an irregular lateral high-echoic layer (LHEL) as the main indication of SS invasion. If the presence of a deep hypoechoic area combined with an irregular LHEL is considered, the diagnosis of SS invasion may be improved.

Endoscopic Ultrasound-Guided Versus Transjugular Portal Pressure Measurements: Systematic Review and Meta-Analysis.

PURPOSE: Published reviews of endoscopic ultrasound-guided portal pressure gradient (EUS-PPG) have emphasized feasibility and safety. We performed a systematic review and meta-analysis specifically to evaluate how closely EUS-based portal pressure measurements track invasive comparator measurements in prospective paired studies and to summarize agreement, technical success, and adverse events. METHODS: We searched major databases through January 2026 for prospective cohorts reporting same-patient EUS-based portal pressure measurement and invasive hemodynamic measurements. Correlations were po

Comparing Endoscopic Ultrasound (EUS) vs. Magnetic Resonance Cholangiopancreatography (MRCP) in the Etiological Evaluation of Idiopathic Acute Pancreatitis (IAP): A Systematic Review and Meta-Analysis.

INTRODUCTION: Idiopathic acute pancreatitis (IAP) accounts for up to 20% of acute pancreatitis cases despite thorough initial evaluation. Endoscopic ultrasound (EUS) and magnetic resonance cholangiopancreatography (MRCP) are commonly used as second-line imaging modalities when IAP etiologies are unclear, yet their comparative diagnostic performance remains uncertain. We conducted a systematic review and meta-analysis to assess the diagnostic yield of EUS versus MRCP in identifying etiologies of IAP. METHODS: A systematic search of PubMed, EMBASE, and Google Scholar (January 2000-April 2025) i
