Behavior and bladder disturbance of enuretic children: a rational classification of a common disorder.

A group of 126 enuretic children were evaluated as to their psychiatric and neurological states, functional bladder volume and environmental circumstances. Few differences were found when the children were grouped according to presence or absence of social disadvantage, the presence of a family history of enuresis, or the nature of onset of enuresis. No classification differentially predicted response to treatment. However, children with an associated psychiatric disorder had significantly lower bladder volumes and had more developmental delays. It is concluded that the classifications of prim…

Bladder and bowel dysfunction in children with sleep bruxism.

OBJECTIVES: There is little evidence of a direct relationship between sleep bruxism and enuresis nocturna, and whether this enuresis is monosymptomatic or non-monosymptomatic nocturnal enuresis (MNE or NMNE) is unknown. The aim of the study is to investigate the type and frequency of nocturnal enuresis and other lower urinary tract symptoms (LUTS) in children with sleep bruxism. METHODS: The children who applied consecutively to the Department of Pediatric Dentistry were included in this study. Sleep bruxism was determined by history and confirmed by clinical examination by pedodontist. LUTS …

Neobladder overactivity; an equivalent to spontaneous rectal contraction.

A 76-year-old man had transitional cell carcinoma of the bladder and underwent orthotopic ileal neobladder, and developed severe nocturnal enuresis. We performed videomanometry in the ileal neobladder and the rectum of the patient. Slow filling of the ileal neobladder showed overactivity (the amplitudes up to 50 cmH2O), which was similar to that normally seen in the rectum (the amplitudes up to 20 cmH2O). In contrast to the normal rectal sensation, neobladder sensation was markedly decreased. During urination, the patient voided with strain and overactivity, both of which were similar to that …

Noncompliance with iron chelation therapy in patients with beta thalassaemia.

Twenty-three percent of patients with beta thalassaemia were found noncompliant to iron chelation. Excluding separation anxiety disorder and primary functional enuresis, the frequency of psychiatric disorders in the noncompliant and the compliant patients was 68% and 10%, respectively. Oppositional disorder was the most frequent psychiatric disorder; it was also associated with the most profound deviation from compliance. Separation anxiety disorder and primary functional enuresis were not associated with noncompliance. In one-third of the noncompliant patients no psychiatric disorder was iden…

[Treatment of children with an unstable bladder using the new anticholinergic drug Mictonorm (author's transl)].

10 children with an unstable bladder were treated with the new anticholinergic drug Mictonorm. With the exception of one boy, the bladder capacity and the volume of all the other children increased. Contractions with amplitudes of 1.5 or 3.0 kPa had been registered. The extent of the maximum amplitudes was reduced. Both the enuresis nocturna and the enuresis diurna were positively influenced, the former, however, more so.

The determination of six-year-old enuretic children's experiences using the dramatization method.

INTRODUCTION: Enuresis nocturna (NE) is a condition in which a child over the age of five wets the bed at least two nights a week for at least three months. Physical diseases, deep sleep, familial predisposition, psychological reasons, separation from parents, fear, traumatic experiences, sibling jealousy, inadequate-irregular toilet training are effective in the emergence of NE. OBJECTIVE: The aim of this study was to determine the prevalence of NE in 66-72-month-old children attending kindergarten, to examine the variables associated with NE, and to identify the feelings, thoughts and exper…

Sleep and night-time behaviour of enuretics and non-enuretics.

OBJECTIVE: To investigate connections between nocturnal enuresis and sleep factors such as the subjective depth of sleep and classical parasomnias. PATIENTS, SUBJECTS AND METHODS: One hundred school children aged 6-10 years answered a questionnaire, with their parents, and the same questions were asked of a group of 29 children of the same age suffering from severe nocturnal enuresis. RESULTS: There were significant differences in arousability, with the enuretic group being 'deep sleepers', and in the prevalence of onset insomnia, nightmares, interrupted sleep and bedtime struggles, which we…

Desmopressin resistant nocturnal polyuria secondary to increased nocturnal osmotic excretion.

PURPOSE: We investigated the role of increased solute excretion in children with desmopressin resistant nocturnal enuresis and nocturnal polyuria. MATERIALS AND METHODS: A total of 42 children with monosymptomatic nocturnal enuresis and significant nocturnal polyuria with high nocturnal urinary osmolality (more than 850 mmol/l) were not responding to desmopressin. A 24-hour urinary concentration profile was obtained with measurement of urine volume, osmolality, osmotic excretion and creatinine. The control group consisted of 100 children without enuresis. RESULTS: Based on osmotic excretion …

Translational Research for Pediatric Lower Urinary Tract Dysfunction.

This review provides a comprehensive view of translational research aimed at elucidating the pathophysiology of pediatric lower urinary trac … nence, urofacial (Ochoa) syndrome may be creating a prototype of a new research approach. Nocturnal enuresis has long been studied genetically, and several candidate loci have been reported. However, the pursuit for enuresis genes has been abandoned partly because genetic association and enuresis phenotype (bladder or renal type) could not be linked. Enure …

Two Japanese patients with gitelman syndrome.

Gitelman syndrome (GS) is a renal tubular disorder characterized by hypokalemia, hypomagnesemia, metabolic alkalosis and hypocalciuria due to defective tubular reabsorption of magnesium and potassium. This disease is caused by mutations of the thiazide-sensitive Na-Cl cotransporter (NCCT) gene, SLC12A3. Manifestations of GS are heterogeneous, from asymptomatic to mild symptoms of cramps and easy fatigue, to tetany and paralysis. Polydipsia, polyuria, and nocturia are also frequent in GS patients. Here we describe two Japanese patients with GS followed as nocturnal enuresis. In the first patien…

From Clinical Scenarios to the Management of Lower Urinary Tract Symptoms in Children: A Focus for the General Pediatrician.

Lower urinary tract symptoms (LUTS) are a relevant problem in the pediatric population, having a very high prevalence. Diurnal incontinence and nocturnal enuresis are surely the most frequent symptoms, presenting, respectively, in up to 30% of school-age children and up to 10% of children between 6 and 7 years. Stypsis is the most common comorbidity, and it must be considered in the management of LUTS; indeed, the treatment of constipation is curative in most cases for both incontinence and enuresis. The presence or absence of diurnal symptoms in nocturnal enuresis and urgency in diurnal incon…

The mechanisms of the action of electrical stimulation of muscles.

The mechanisms of the action of electrical stimulation of muscles are discussed. We believe that the principal cause for the good results obtained in cases of stress incontinence with electrical stimulation must be due to the formation of a better reflex in contraction of the muscles of the pelvic floor. This, however, envolves changes at the level of the spinal cord. By the help of stimulation the patient learns to use the urethral sphincter and secondary forces of retention in a better and a more proper way. The good results obtained in the treatment of some cases of nocturnal enuresis by an…

IL-12 overexpression in mice as a model for Sjögren lung disease.

The transition from day to night is associated with a pronounced decline in diuresis with reductions in the amount of excreted water, electrolytes, and other end products of our metabolism. Failure to do so leads to a large urine output at night, a condition known as nocturnal polyuria, encountered in a large proportion of children with nocturnal enuresis. The aim of this study was to clarify the mechanisms responsible for the nocturnal polyuria seen in enuretics with inadequate response to desmopressin (dDAVP). Forty-six enuretics (7-14 yr of age) and fifteen age-matched controls were admitte…

Modern approach in treatment of diabetes insipidus.

In this paper we have reviewed the position of desmopressin in the treatment of diabetes insipidus. Desmopressin is a synthetic analog of vasopressin, with more pronounced antidiuretic effect. It is treatment of choice in substitution therapy of diabetes insipidus. Its application before sleeping time can reduce nocturnal enuresis, so it has a place in the treatment of enuresis nocturna. Antidiuretic effect of desmopressin is the result of agonistic effect on V2 receptors in the renal tubules. The efficacy and safety of desmopressin in mentioned indications was confirmed in clinical studies.

[Urodynamic assessment of enuretic children (author's transl)].

Urodynamic assessment was carried out in 33 children with persistent enuresis in spite of previous therapy. According to these investigations it was possible to divide our patients into several groups. One group of children displayed a normal urodynamic pattern. Another group showed neurogenic non-inhibited detrusor contractions on cystometry. Obstruction was noted in a third group of patients by means of urodynamic parameters. A small group of children was urodynamically normal, but suffered from recurrent urinary infections. The therapeutic results and the aetiology of enuresis is discussed.

Urinary incontinence in children.

BACKGROUND: Urinary incontinence (bedwetting, enuresis) is the commonest urinary symptom in children and adolescents and can lead to major distress for the affected children and their parents. Physiological and non-physiological types of urinary incontinence are sometimes hard to tell apart in this age group. METHODS: This article is based on selected literature retrieved by a PubMed search and on an interdisciplinary expert consensus. RESULTS AND CONCLUSION: Nocturnal enuresis has a variety of causes. The main causative factors in monosymptomatic enuresis nocturna (MEN) are an impaired abil…

[Urodynamic studies in the child with urinary incontinence].

Voiding disorders in children are frequent. To establish an exact anamnesis, it has to be distinguished between enuresis and incontinence and this may consequently already have a therapeutical effect. Enuresis is defined as a normal void occurring at an inappropriate time or place. Incontinence is an involuntary loss of urine and a pathological voiding habit, often in combination with urinary infections. The causes of an enuresis are always functional, the causes for incontinence may be organic or functional. Organic causes are neurogenic bladder dysfunctions or structural anomalies of the low…
