The last 3 decades have seen a shift in the epidemiology of epiglottitis. Epiglottitis was once most commonly associated with Haemophilus influenzae type B. A 15-year-old boy presented with airway obstruction and a history of sore throat and progressive dyspnoea for 1 month. A lateral neck radiograph showed an enlarged epiglottis, and a neck computed tomography (CT) demonstrated a cyst attached to the lingual surface of the epiglottis. A large epiglottic cyst, 4 cm in length, was removed surgically. Epiglottic cysts have been reported to cause airway obstruction in neonates, infants and adults, but, to the best of our knowledge, it has rarely been reported in adolescents.

Epiglottitis has been mainly associated with childhood infection with Haemophilis influenzae type B but cases of adult epiglottitis are increasing. We report here a case of adult epiglottitis and present evidence that it was caused by Staphylococcus aureus. A 48-year old patient with clinical symptoms of epiglottitis grew Staphylococcus aureus in pure culture from an epiglottal swab. Staphylococcus aureus should be considered as a potential pathogen in adult epiglottitis.

This case report involves the anesthetic management of a rare case of adult epiglottitis. It is unique in that there are real photographs of the illness and a previously undocumented use of a specific airway adjunct in acute epiglottitis, the Bullard laryngoscope. Management of the case from start to finish is discussed, as well as the usefulness of the Bullard laryngoscope in a difficult intubation scenario.

A 53 year old man presented with the chief complaint of having a fish bone stuck in the throat for about 1 h. There was no dysphagia or respiratory symptoms. Plain lateral neck X-ray, direct laryngoscopy and oesophagogastroduodenoscopy showed a grossly swollen epiglottis with narrowing of the laryngeal lumen. No foreign body was found. His condition improved rapidly with intravenous antibiotic therapy. As acute epiglottitis may be a sudden life-threatening condition, a high index of suspicion should be maintained for patients who present with alleged foreign bodies in the throat.

A previous report reviewed the technique and indications for near-total laryngectomy with epiglottic reconstruction in the management of squamous cell carcinoma of the glottis. This approach permits removal of most of both vocal folds, with immediate reconstruction using the epiglottis without the need for stenting or multistage procedures.
