Abnormal functional connectivity patterns in temporal lobe epilepsy-An international ENIGMA-epilepsy study.

OBJECTIVES: Temporal lobe epilepsy (TLE) impacts multiple brain networks. Aberrant functional connectivity has been demonstrated in resting-state networks (RSNs) that mediate higher brain functions in TLE. This study aimed to identify the reproducible patterns of altered functional connectivity in TLE in a large, international cohort through ENIGMA-Epilepsy. METHODS: Resting-state functional MRI datasets from nine centers across North America, South America, Europe and South Africa, including 442 people with TLE and 387 healthy adults, were analyzed. We examined group differences in whole-bra…

Clinical Characteristics, Treatment Patterns, and Drug-Resistant Epilepsy Among Emirati Adults with Epilepsy.

INTRODUCTION: Detailed clinical descriptions of treated adult epilepsy in the Arab region remain scarce. We examined epilepsy type, antiseizure medication (ASM) use, comorbidities, and drug-resistant epilepsy (DRE) in Emirati adults with epilepsy in the United Arab Emirates (UAE). METHODS: We conducted a retrospective cohort study of Emirati adults aged 18-65 years attending neurology clinics at a tertiary center in Al Ain between January 2019 and January 2024. Patients were screened from neurology clinic visits, and those with at least one G40.909 coded encounter underwent chart review. Epil…

Somatic DNA Variants in Epilepsy Surgery Brain Samples from Patients with Lesional Epilepsy.

Epilepsy affects 50 million people worldwide and is drug-resistant in approximately one-third of cases. Even when a structural lesion is identified as the epileptogenic focus, understanding the underlying genetic causes is crucial to guide both counseling and treatment decisions. Both somatic and germline DNA variants may contribute to the lesion itself and/or influence the severity of symptoms. We therefore used whole exome sequencing (WES) to search for potentially pathogenic somatic DNA variants in brain samples from children with lesional epilepsy who underwent epilepsy surgery. WES was pe…

Topographic differences in EEG microstates: distinguishing juvenile myoclonic epilepsy from frontal lobe epilepsy.

UNLABELLED: Pseudomonas aeruginosa (P. aeruginosa) is a critical healthcare challenge due to its ability to cause persistent infections and the acquisition of antibiotic resistance mechanisms. Lack of preventive vaccines and rampant drug resistance phenomenon has rendered patients vulnerable. As new antimicrobials are in the preclinical stages of development, mining for the unexploited drug targets is also crucial. In the present study, we designed a B- and T-cell multi-epitope vaccine against P. aeruginosa using a subtractive proteomics and immunoinformatics approach. A total of five proteins

MicroRNAs as potential biomarkers in temporal lobe epilepsy and mesial temporal lobe epilepsy.

Temporal lobe epilepsy is the most common form of focal epilepsy in adults, accounting for one third of all diagnosed epileptic patients, with seizures originating from or involving mesial temporal structures such as the hippocampus, and many of these patients being refractory to treatment with anti-epileptic drugs. Temporal lobe epilepsy is the most common childhood neurological disorder and, compared with adults, the symptoms are greatly affected by age and brain development. Diagnosis of temporal lobe epilepsy relies on clinical examination, patient history, electroencephalographic recordin…

Epilepsy in a melanocyte-lineage mTOR hyperactivation mouse model: A novel epilepsy model.

OBJECTIVE: To clarify the complex mechanism underlying epileptogeneis, a novel animal model was generated. METHODS: In our previous research, we have generated a melanocyte-lineage mTOR hyperactivation mouse model (Mitf-M-Cre Tsc2 KO mice; cKO mice) to investigate mTOR pathway in melanogenesis regulation, markedly reduced skin pigmentation was observed. Very unexpectedly, spontaneous recurrent epilepsy was also developed in this mouse model. RESULTS: Compared with control littermates, no change was found in either brain size or brain mass in cKO mice. Hematoxylin staining revealed no obvious…

White matter abnormalities across different epilepsy syndromes in adults: an ENIGMA-Epilepsy study.

The epilepsies are commonly accompanied by widespread abnormalities in cerebral white matter. ENIGMA-Epilepsy is a large quantitative brain imaging consortium, aggregating data to investigate patterns of neuroimaging abnormalities in common epilepsy syndromes, including temporal lobe epilepsy, extratemporal epilepsy, and genetic generalized epilepsy. Our goal was to rank the most robust white matter microstructural differences across and within syndromes in a multicentre sample of adult epilepsy patients. Diffusion-weighted MRI data were analysed from 1069 healthy controls and 1249 patients: t…

Successful epilepsy surgery in frontal lobe epilepsy with startle seizures: a SEEG study.

Pre-surgical assessment and surgical management of frontal epilepsy with normal MRI is often challenging. We present a case of a 33-year-old, right-handed, educated male. During childhood, his seizures presented with mandibular myoclonus and no particular trigger. As a young adult, he developed seizures with a startle component, triggered by unexpected noises. During his ictal episodes, he felt fear and grimaced with sudden head flexion and tonic axial posturing. Similar seizures also occurred without startle. Neuropsychological assessment showed executive dysfunction and verbal memory deficit…

Semiquantitative analysis of interictal glucose metabolism between generalized epilepsy and localization related epilepsy.

Positron emission tomography (PET) with [18F]fluoro-D-deoxyglucose (FDG) has been used to detect seizure foci and evaluate surgical resection with localization related epilepsies. However, few investigations have focused on generalized epilepsy in children. To reveal the pathophysiology of generalized epilepsy, we studied 11 patients with generalized epilepsy except West syndrome, and 11 patients with localization related epilepsy without organic disease. The FDG PET was performed by simultaneous emission and transmission scanning. We placed regions of interest (ROI) on bilateral frontal lobe,…

[Treatment protocol for long-term anti-epilepsy drugs in children with refractory epilepsy].

Long-term treatment of children with drug-resistant epilepsy has not been addressed in detail in the literature. Contraindications for surgery must be reassessed periodically. The epileptic nature and the focal characteristic of seizures must be verified, the absence of tumor, malformation or another indication for surgery (i.e. obstructed valve.), the choice of anti-epileptic drugs depending on the epilepsy syndrome, even following callosotomy, and the dose, that is often over-estimated, must also be re-evaluated.

Epilepsy surgery outcome in children with focal epilepsy due to tuberous sclerosis complex.

Tuberous sclerosis complex (TSC) is frequently associated with focal epilepsy due to cerebral tubers. Seizures are the first symptoms in most patients with brain involvement. These epilepsies are frequently severe, drug-resistant and may have a negative impact on the child's global development. Although most epilepsies are multicentric, these patients may be candidates for epilepsy surgery, if it is possible to determine a leading epileptogenic tuber. Nine patients with TSC were examined with long-term video-EEG monitoring, different neuroimaging techniques and neuropsychological tests. A main…

The basal ganglia and the epilepsies.

The basal ganglia are an evolutionarily conserved neural circuit best known for the selection and sequencing of voluntary movement. Increasing evidence, however, indicates that these structures also play an important role in regulating pathological brain states, including seizures. Early pharmacological and lesion studies demonstrated that manipulation of basal ganglia output nuclei, particularly the substantia nigra pars reticulata, profoundly alters seizure susceptibility across multiple experimental models. Subsequent work expanded this view, revealing that several basal ganglia structures,

Semiology of Opercular Epilepsies.

In this systematic review, we aimed to characterize the semiology of 'pure' opercular seizures by identifying cases in which the epileptogenic zone was restricted to the operculum. Seventy patients from 24 articles were included and categorized into four anatomic groups: 11 frontal, 27 central, 14 parietal, and 18 temporal operculum cases. Across these four anatomic regions, we identified the following semiological patterns: Frontal opercular seizures were characterized by a lower frequency of auras, most commonly nonpainful somatosensory sensations affecting the limbs or trunk and never exclu

Theory of mind in epilepsy.

Epilepsy is characterized by recurrent, chronic, and unprovoked seizures. Epilepsy has a significant negative impact on a patient's quality of life even if seizures are well controlled. In addition to the distress caused by seizures, patients with epilepsy (PwE) may suffer from cognitive impairment with serious social consequences such as poor interpersonal relationships, loss of employment, and reduced social networks. Pathological changes and functional connectivity abnormalities observed in PwE can disrupt the neural network responsible for the theory of mind. Theory of mind is the ability …

Epilepsy in children with leukodystrophies.

BACKGROUND: Epilepsy might be one of the manifestations in children with leukodystrophies, but the incidence of epilepsy in different types of leukodystrophies is unclear yet. METHODS: A retrospective observational cohort study was performed on children diagnosed with leukodystrophies in Peking University First Hospital from January 2004 to June 2019, and the patients were followed for 5.5 years (0.4-14.2 years) after the first visit. RESULTS: A total of 333 patients were included. The overall incidence of epilepsy was 30.6% (102/333). Alexander disease had the highest incidence (77.3%, 51/6…

Opportunities for Electroceuticals in Epilepsy.

Epilepsy is a neurological disorder that affects ∼1% of the world population. Nearly 30% of epilepsy patients suffer from pharmacoresistant epilepsy that cannot be treated with antiepileptic drugs. Depending on seizure type, a diverse range of therapies are available, including surgery, vagus nerve stimulation, and deep brain stimulation. We review the sensing and stimulation technologies most used in neurological disorders, and provide a vision of minimally invasive electroceuticals to enable accurate forecasting of epileptic seizures and therapy. The use of such systems could potentially hel…
