Symptom-related damage patterns in mesial temporal lobe epilepsy with hippocampal sclerosis.

OBJECTIVE: This study was undertaken to elucidate the neuroanatomical and network substrates underlying diverse seizure symptoms in mesial temporal lobe epilepsy with hippocampal sclerosis (mTLE-HS) by applying multivariate support vector regression (SVR) to individualized structural symptom mapping and atrophy network mapping (ANM). METHODS: We retrospectively enrolled 216 patients with mTLE-HS and 460 healthy controls. Six non-mutually exclusive seizure symptoms were assessed. We performed left-right flipping of all images of right-sided mTLE-HS patients along the midline before processing

The usefulness of intraoperative electrocorticography (iECoG) in pediatric temporal lobe epilepsy surgery.

OBJECTIVE: This study aimed to evaluate the usefulness of intraoperative electrocorticography (iECoG) in providing a more accurate surgical strategy, thereby yielding seizure freedom following resective surgery in children with temporal lobe epilepsy (TLE). METHODS: The authors conducted a retrospective review of pediatric patients with drug-resistant TLE due to various etiologies, within a relatively long follow-up (range 8.5-11.5 years). Patients were divided into two groups based on whether they were operated on using iECoG or not, which was employed in cases of uncertain delineation of th

White matter resection and verbal memory deficits after temporal lobe epilepsy surgery.

Temporal lobe resection for focal, drug-resistant temporal lobe epilepsy (TLE) causes verbal memory deficits in 30% of left hemisphere-operated patients. Structural, functional and computational modelling have shown a widespread structural and functional memory network with hubs in critical brain regions including the hippocampus, subcortical and neocortical regions. We hypothesized that damage to white matter pathways forming a network involving cortical and subcortical regions may be responsible for postoperative memory problems. In this study, we measured verbal memory encoding (immediate r

Thalamic glutamatergic neurons regulate seizure onset and generalization in temporal lobe epilepsy.

Temporal lobe epilepsy (TLE) is a common form of epilepsy in adults. The anterior nucleus of the thalamus (ANT) is a potential therapeutic target due to its role in epileptic networks. Using calcium imaging, we show that glutamatergic neurons in the anterodorsal thalamic nucleus (AD), a subarea of the ANT, are activated during hippocampal seizures. Chemogenetic inhibition of AD glutamatergic neurons suppresses seizure onset and generalization in TLE models, whereas activation promotes seizures. Trans-monosynaptic tracing identifies the postsubiculum (PoSub) as an important downstream target of

Sleep and circadian rhythm disruptions in animal models of temporal lobe epilepsy.

Temporal lobe epilepsy (TLE) is frequently accompanied by disruptions to sleep and circadian rhythms, which substantially contribute to disease burden. Human studies are often confounded by antiseizure medications, limiting insight into underlying mechanisms. Animal models therefore provide critical opportunities to examine causal interactions, yet their translational validity has not been systematically evaluated. In this review, we first outline the relevance of rodent models for studying epilepsy- and sleep-related processes. We then examine current evidence for sleep and circadian disturba

Circuit Reorganization of Subicular Cell-Type-Specific Interneurons in Temporal Lobe Epilepsy.

The subiculum represents a crucial brain pivot in regulating seizure generalization in temporal lobe epilepsy (TLE), primarily through a synergy of local GABAergic and long-projecting glutamatergic signaling. However, little is known about how subicular GABAergic interneurons are involved in a cell-type-specific way. Here, employing Ca2+ fiber photometry, retrograde monosynaptic viral tracing, and chemogenetics in epilepsy models of both male and female mice, we elucidate circuit reorganization patterns mediated by subicular cell-type-specific interneurons and delineate their functional dispar

Linking Memory Impairment to Structural Connectivity in Extrahippocampal Temporal Lobe Epilepsy Surgery.

OBJECTIVE: Temporal lobe epilepsy (TLE) constitutes the most common drug-refractory epilepsy syndrome. Tailored approaches are required, as TLE originates from extrahippocampal lesions in about one-quarter of surgical candidates. Despite high success rates in seizure control, concern persists regarding postoperative memory decline after lesionectomy. We investigated the associations between structural connectivity and postoperative memory performance in extrahippocampal TLE surgery. METHODS: In total, 55 patients (25 females, 30 males; mean age 29.8 ± 14.5 years; epilepsy duration 7.9 ± 10.5

Neuroinflammation leads to pharmacoresistance in temporal lobe epilepsy via promoting spermine degradation.

Pharmacoresistance remains intractable in epilepsy, necessitating in-depth mechanism investigations. Cumulative data have pointed to active neuroinflammation in pharmacoresistant epilepsy, but the process between neuroinflammation and pharmacoresistance remains unknown. In this study we investigated how severe neuroinflammation altered anti-seizure drugs (ASMs) pharmacology. Hippocampal kindling or kainic acid-induced temporal lobe epilepsy (TLE) models were established in mice that had received intra-hippocampal LPS injection. Acute hippocampal slices were prepared; current-clamp recording wa

N6-methyladenosine (m6A) dysregulation contributes to network excitability in temporal lobe epilepsy.

Analogous to DNA methylation and protein phosphorylation, it is now well understood that RNA is also subject to extensive processing and modification. N6-methyladenosine (m6A) is the most abundant internal RNA modification and regulates RNA fate in several ways, including stability and translational efficiency. The role of m6A in both experimental and human epilepsy remains unknown. Here, we used transcriptome-wide m6A arrays to obtain a detailed analysis of the hippocampal m6A-ome from both mouse and human epilepsy samples. We combined this with human proteomic analyses and show that epilepti

Effective Connectivity Predicts Surgical Outcomes in Temporal Lobe Epilepsy: A SEEG Study.

INTRODUCTION: Temporal lobe epilepsy (TLE), the most common type of drug-resistant epilepsy (DRE), has a postoperative seizure-free rate of ~70%. Furthermore, precisely localizing the epileptogenic zone and determining the surgical resection area have been established as the key factors influencing surgical outcomes. Herein, we innovatively coupled the surgical resection area with characteristics of effective connectivity via intracranial electroencephalography (iEEG) to predict patients' surgical prognosis. METHODS: This study involved 56 patients who underwent TLE surgery and were followed

Longitudinal changes in hippocampal morphology before and after temporal lobe epilepsy surgery.

Temporal lobe epilepsy (TLE) is associated with progressive neocortical thinning that may be prevented by resective surgery, but little knowledge exists on dynamic changes in the hippocampus. Here, we assessed progressive morphological changes of the hippocampus before and after TLE surgery. In this longitudinal case-control neuroimaging study, we included patients with unilateral drug-resistant TLE before (main cohort n = 23; replication cohort n = 81) or after (n = 54) anterior temporal lobe resection and healthy volunteers (n = 120) matched for age and sex. We compared hippocampal volumes a

The structure-function dissociation in mesial temporal lobe epilepsy with temporopolar blurring.

OBJECTIVES: While ipsilateral temporopolar blurring (TPB) has been associated with white matter degeneration in temporal lobe epilepsy with hippocampal sclerosis (TLE-HS), its influence within the large-scale neural networks remains unclear. This study investigates the network-level topological alterations associated with TPB through multilayer network analysis. METHODS: This retrospective study investigated the impact of TPB on topological architecture of structural (SCN) and functional connectivity networks (FCN) in patients with TLE-HS using graph theory analysis. We further calculated the

E4BP4-Driven Circadian SLC6A1 Expression Governs Tiagabine Chronoefficacy in Temporal Lobe Epilepsy.

Temporal lobe epilepsy (TLE), the most prevalent form of focal epilepsy and a leading cause of surgically managed intractable cases, is characterized by rhythmic spontaneous seizures that exhibit underutilized therapeutic potential. Here, we aim to uncover the impact of dosing time on the anticonvulsant effect of tiagabine and to elucidate the underlying mechanisms involved. Tiagabine markedly attenuated seizure severity and progression in both acute and chronic models of pilocarpine-induced TLE. Pharmacological effects of tiagabine were found to vary according to dosing time, demonstrating gr

Gray matter abnormalities and memory impairment in left mesial temporal lobe epilepsy.

OBJECTIVES: Mesial temporal lobe epilepsy (MTLE) is a common neurological disorder, with memory impairment being one of its typical symptoms. Most previous studies have focused on assessing declarative memory directly related to hippocampal functions, but emerging data suggest a decline in the efficiency of other types of memory as well. The aim of this study was to comprehensively assess various types of memory and analyze the relationship between memory performance and the volume of selected gray matter structures. METHODS: In total, 21 patients with left-MTLE and 28 age- and education-matc

High energy consumption characterizes abnormal brain state transitions in temporal lobe epilepsy.

Temporal lobe epilepsy (TLE) patients experience shifts between non-seizing and seizing brain states, but the structural networks underlying these transitions remain undefined and poorly characterized. We detected dynamic brain states in resting-state fMRI and constructed linked structural networks utilizing multi-shell diffusion-weighted MR data. Leveraging network control theory, we interrogated the structural data for all possible brain state transitions, identifying those requiring abnormal levels of transition energy (low or high) in TLE compared to matched healthy participants (n's = 25)
