The Role of Exercise Testing in Pulmonary Vascular Disease: Diagnosis and Management.

Exercise intolerance is the dominant symptom of pulmonary hypertension (PH). The gold standard for the estimation of exercise capacity is a cycle ergometer incremental cardiopulmonary exercise test (CPET). The main clinical variables generated by a CPET are peak oxygen uptake (Vo2peak), ventilatory equivalents for carbon dioxide (VE/Vco2), systolic blood pressure, oxygen (O2) pulse, and chronotropic responses. PH is associated with hyperventilation at rest and at exercise, and an increase in physiologic dead space. Maximal cardiac output depends on right ventricular function and critically det

Uncovering the mechanisms of exertional dyspnoea in combined pulmonary fibrosis and emphysema.

The prevailing view is that exertional dyspnoea in patients with combined idiopathic pulmonary fibrosis (IPF) and emphysema (CPFE) can be largely explained by severe hypoxaemia. However, there is little evidence to support these assumptions. We prospectively contrasted the sensory and physiological responses to exercise in 42 CPFE and 16 IPF patients matched by the severity of exertional hypoxaemia. Emphysema and pulmonary fibrosis were quantified using computed tomography. Inspiratory constraints were assessed in a constant work rate test: capillary blood gases were obtained in a subset of pat

1-Minute Sit-to-Stand Test: SYSTEMATIC REVIEW OF PROCEDURES, PERFORMANCE, AND CLINIMETRIC PROPERTIES.

PURPOSE: Tests for quantifying exercise capacity that are applicable in diverse settings are needed. The 1-min sit-to-stand test (1-MSTST) is such a test. This systematic review summarizes the literature addressing 1-MSTST procedures, performance, and clinimetric properties. METHODS: Three online databases, hand searches, and an expert consultant were used to identify literature relevant to the aims of this review. Inclusion required that studies addressed the 1-MSTST, focused on adults, and were written in English. RESULTS: Seventeen articles were identified that met the inclusion criteria.

Prevention with and without the use of medications for exercise-induced asthma.

Exercise-induced asthma or bronchospasm occurs in approximately 12% of the population. It can be prevented with proper techniques of exercise and environmental exposures and with medication. Exercise techniques include timing of exercise to less than 5-min interludes and continued repeat exercise every hour. Exercising to less than full tolerance may avoid the onset of exercise-induced asthma. In some individuals exercise in warm, humidified air is less asthmogenic than in dry, cold air. Medications that are effective and safe include bronchodilators such as theophylline and beta-agonists and

A new "complex exercise test" for children with cardiac diseases.

In view of the characteristics of children's play or exercise activity, we designed a new method of exercise testing for children, called the "Complex Exercise Test" (CET), which includes three kinds of exercise test: (1) a graded exercise test on the treadmill, (2) a simple bending and stretching test to evaluate momentary exercise tolerance and (3) a swimming test with the ECG in the water. The CET was applied to a total of 25 cases which included 10 cases of postoperative tetralogy of Fallot and 15 cases with ventricular premature contraction (VPC). A two-way exercise test, which included t…

Effect of tiotropium on neural respiratory drive during exercise in severe COPD.

BACKGROUND: Studies have shown that tiotropium once daily reduces lung hyperinflation and dyspnea during exercise and improves exercise tolerance in patients with COPD. Mechanisms underlying the effects of the muscarinic receptor antagonist tiotropium on COPD have not been fully understood. OBJECTIVE: In this study, we investigated whether improvement in neural respiratory drive is responsible for reducing dyspnea during exercise and improving exercise tolerance in COPD. METHODS: Twenty subjects with severe COPD were randomized into two groups: no treatment (Control, n = 10, 63.6 ± 4.6 years…

Absence of compensatory vasodilation with perfusion pressure challenge in exercise: evidence for and implications of the noncompensator phenotype.

Compromising oxygen delivery (O2D) during exercise requires compensatory vasodilatory and/or pressor responses to protect O2D:demand matchin … erences in the mechanisms governing O2D:demand matching should be considered as factors influencing exercise tolerance. NEW & NOTEWORTHY In healthy young individuals, compromising submaximally exercising muscle perfusion appears to evoke compensatory vasodilation to defend oxygen delivery. Here we report the absence of compensatory vasodilation in 8 of 21 such indiv …

Previous hyperthermic treatment increases mitochondria oxidative enzyme activity and exercise capacity in rats.

The study was designed to investigate the role of hyperthermia in the tolerance of exercise in rats and the possible mechanism was examined. The hyperthermic pretreatment was performed using an electric pad on the anesthesized rats 24 hours before exercise. Rats were exercised passively in a motor-controlled round treadmill in high temperature environment (36-37 degrees C) until exhaustion. The capacity of tolerance was calculated. Lymphocytes and gastrocnemius muscle were collected from both groups. The changes in muscular morphology, mitochondria oxidative enzyme activity and induction of Hs

Exercise capacity, muscle strength and objectively measured physical activity in patients after heart transplantation.

Maximal exercise capacity of patients after heart transplantation (HTX) remains limited, affecting their quality of life. Evidence on the evolution of muscle strength and physical activity (PA) post-HTX is lacking, but a prerequisite to tailor cardiac rehabilitation programmes. Forty-five consecutive patients were evaluated every 3 months during the first year post-HTX. Functional exercise capacity (Six minutes walking distance test (6MWD)), peripheral (Quadriceps strength (QF)) and respiratory (Maximal inspiratory strength (MIP)) muscle strength were evaluated. PA (number of steps (PAsteps),

Capturing the perspectives of women with coronary artery disease regarding interval training or continuous exercise in cardiac rehabilitation.

BACKGROUND: Motivators and barriers to exercise participation in women with coronary artery disease remain poorly understood. With evidence suggesting that women with coronary artery disease are less likely to adhere to exercise during cardiac rehabilitation and are more likely to drop out, it is important to understand these factors in order to optimize cardiac rehabilitation programs for women. METHODS: We contribute to the discussion by presenting findings from a qualitative study using two focus groups with nine women with coronary artery disease sharing their experiences with attending c

Current best practice in rehabilitation in interstitial lung disease.

Interstitial lung disease (ILD) is a group of chronic respiratory diseases characterized by dyspnoea on exertion and decline in health-related quality of life (HRQL). People with ILD experience significant exercise limitation with contributors that include ventilatory limitation, impaired gas exchange, decreased cardiac function and skeletal muscle dysfunction. Pulmonary rehabilitation (PR) is well established in patients with chronic obstructive pulmonary disease (COPD) as a means to overcome exercise limitation and improve activity-related dyspnoea. There is increasing evidence for similar e
