CT-based assessment of bear-inflicted maxillofacial injuries: evaluation using the facial injury severity scale and its association with hospitalization duration.

PURPOSE: Bear-inflicted injuries are often associated with severe maxillofacial injury. This study aimed to evaluate CT findings of bear-inflicted injuries using the Facial Injury Severity Scale (FISS) and other trauma grading systems, with a particular focus on the relationship between FISS scores and hospitalization duration as well as the characteristics of maxillofacial injuries. MATERIALS AND METHODS: This retrospective study included 31 patients with bear-inflicted injuries who underwent whole-body CT. Maxillofacial injury severity was assessed using FISS, while head and thoracic injuri…

The role of multislice spiral computed tomography in the diagnosis and management of acute facial trauma in patients with multiple injuries.

Using of mutislice spiral CT as first line examination for the diagnosis of Acute Facial trauma in the setting of Polytrauma reduces both: valuable time and cost of patient treatment. After a brief clinical examination, MDCT was performed depending on the area of injury, using a slice thickness of 0.65 mm. The obtained data were analyzed using 3D, MIP and Standard axial with Bone reconstruction protocols. 64 polytrauma patients were evaluated with both Anterior and Lateral craniography (plain skull X ray: AP and Lateral) and Multi Slice CT. Craniography detected only 18 cases of traumatic inju

Assessment of facial nerve injury with "House and Brackmann facial nerve grading system" in patients of temporomandibular joint ankylosis operated using deep subfascial approach.

BACKGROUND: Preservation of the functional integrity of the facial nerve (FN) is a critical measure of success in temporomandibular joint (TMJ) surgery. In spite of the development of a myriad of surgical approaches to the TMJ, FN remains at risk. The deep subfascial approach provides an additional layer of protection (the deep layer of the temporalis fascia and the superficial temporal fat pad) to the temporal and zygomatic branches of the FN and thus, is the safest method to avoid FN injury. OBJECTIVES: To assess FN injury following TMJ surgery using deep subfascial approach and measuring i

CD4(+) T cell-mediated facial motoneuron survival after injury: Distribution pattern of cell death and rescue throughout the extent of the facial motor nucleus.

We have previously demonstrated that CD4(+) T cells transiently rescue facial motoneurons (FMN) from axotomy-induced death in immunodeficient mice. Three subpopulations of motoneurons have been observed within the facial motor nucleus following axotomy: one that always survives axotomy (50%), one that is amenable to rescue from axotomy-induced death through the addition of neurotrophic factors or CD4(+) T cells (30-40%), and one that always dies after axotomy (10-15%). The objective of this study was to anatomically map the extent of axotomy-induced cell death and immune cell rescue in the fac

Effect of Epineurial Neurorrhaphy on Restoration of Facial Nerve Injuries with Different Levels of Neurotmesis in a Rat Model: A Pilot Study.

BACKGROUND: Different degrees of neurotmesis of the peripheral facial nerve frequently are encountered in clinic, for which the epineurial neurorrhaphy is the preferred technique. However, because of the capability of self-restoration of nerves and the side effects of surgery, neurorrhaphy may not be an optimal choice for various degrees of neurotmesis. In this study, we explored the necessity of epineurial neurorrhaphy for different degrees of neurotmesis, in addition to investigating factors that impact neural functional recovery. METHODS: Rat models were divided into 6 groups: intact, noni

Blunt-mechanism facial fracture patterns associated with internal carotid artery injuries: recommendations for additional screening criteria based on analysis of 4,398 patients.

PURPOSE: Blunt internal carotid artery injuries (BCAIs) can result from craniofacial trauma, yet the association between craniofacial fractures and BCAIs is poorly understood. MATERIALS AND METHODS: A retrospective cohort study of patients with blunt-mechanism facial fracture(s) presenting to a large trauma center was undertaken to identify facial fracture patterns predictive of BCAIs. Predictor variables included specific facial fracture patterns. Additional variables included demographic, injury mechanism, and associated injury classifications. Outcome variables included the presence or abs

Advanced trauma life support (ATLS) and facial trauma: can one size fit all? Part 2: ATLS, maxillofacial injuries and airway management dilemmas.

Maxillofacial trauma poses an obvious threat to the patient's airway, which may not be immediately evident. In the multiply injured patient, the co-existence of actual or potential injuries elsewhere may complicate airway management, notably in the presence of full spinal immobilization. Following high-velocity trauma, injuries to the cervical spine must be assumed to be present. They also need to be ruled out in an appropriate and timely manner, as patients may wish to sit up. Assessment and management of the airway in maxillofacial trauma can be difficult, requiring a senior anaesthetist or

Use of the Facial Artery for Free Functioning Muscle Transfers: An Alternative Pedicle for Salvage in Brachial Plexus Lesions with Vascular Injuries.

Free functional muscle transfer (FFMT) is a salvage procedure recommended in cases of brachial plexus injury with late presentations or failures of primary nerve reconstruction. The workhorse for most authors is the gracilis, and the most common indication is the restoration of elbow flexion. For successful revascularization of the muscle, donor vessels must be in proximity of the site of the muscle fixation and allow direct coaptation to a donor nerve, ideally without the use of nerve grafts. A major problem occurs when patients have sustained concomitant vascular injuries to the subclavian a

Don't Blow It! Extensive Subcutaneous Emphysema of the Neck Caused by Isolated Facial Injuries: A Case Report and Review of the Literature.

BACKGROUND: Although subcutaneous emphysema resulting from maxillofacial surgery is well described in the literature, the association with maxillofacial trauma is less firm. Clinically evident subcutaneous emphysema from facial injury is uncommon and extension into the cervical and mediastinal tissues is exceedingly rare, with few reported cases. CASE REPORT: An unusual case of extensive subcutaneous emphysema after facial trauma is presented. The case posed a diagnostic dilemma in our emergency department. WHY SHOULD EMERGENCY PHYSICIANS BE AWARE OF THIS?: Facial fractures are a rare but imp

Facial and dental injuries due to dog bite in a 15-month-old child with sequelae in permanent teeth: a case report.

This article reports a longitudinal follow-up of a 15-month-old child with dental trauma resulting from an attack by a dog. The injury consisted of laceration of the facial tissues and loss of the upper central deciduous incisors, in addition to loss of bone tissue in the same area. A malformation of the crown of the right central permanent incisor and complete change of the shape of the left central permanent incisor were observed. The etiological factors of childhood injuries as well as the importance of dental emergency care are discussed and the 14-year clinical and radiographic follow up

[Injuries to the facial portion of the skull and their late effects in children and juveniles - a 10-year analysis (author's transl)].

A 10-year analysis comprising a total of 534 patients was used to study the histories of 192 puerile and juvenile patients with maxillofacial injuries. The proportion of children up to ten and fifteen years of age was 11.4 percent and 16.1 percent, respectively. Clinical and roentgenological reexaminations could be made of 139 patients. Typical fracture localizations and the frequency of late effects observed in the facial skeleton and dental system are discussed. It was possible for late damage to former tooth germs in the cleft of fracture to be observed in every second patient. Disturbances…

Scar Remodeling With Adipose-derived Stem Cell Exosomes After Facial Deep Dermal Injury.

Adipose-derived stem cell exosomes (ASC-exosomes) are emerging as a highly effective regenerative therapy for scar management. We present a case of a 30-year-old female patient who developed severe facial dermal injury and scar contracture following complications from a skin booster injection. Despite initial epithelialization, poor scar quality persisted. Treatment consisted of topical application of ASC-exosome with adjunctive microneedling to enhance intradermal delivery. During a 6-month period, therapy resulted in significant dermal healing, minimal scarring, and improved scar texture and

Expression Patterns of SMAD1-8 in the Peripheral Facial Nerve Following Compressive Nerve Injury or Axotomy.

Facial nerve injury can lead to significant functional impairment, emotional impacts, and difficulties in social and economic activities. Although peripheral nerves have the potential for recovery, incomplete regeneration can pose challenges. Suppressor of Mothers Against Decapentaplegic Homolog (SMAD) proteins are crucial in the nerve-regeneration process. The study aimed to investigate the changes in SMAD protein expression involved in peripheral nerve regeneration following facial nerve injury induced by compression or axotomy in a pre-clinical study conducted on Sprague Dawley rats. Facial

Unexpected consequences: Post-traumatic parotid sialocele following facial stab injury: A case report.

Parotid sialocele is characterized by a collection of saliva in the soft tissue surrounding the parotid gland. Etiology may be traumatic, iatrogenic, or laceration to the salivary duct or the gland parenchyma itself. We here report the case of a 40-year-old male patient who presented with a primary complaint of swelling on the right side of his face. The patient had a history of facial trauma, having been stabbed in the face, and the swelling developed several months after receiving medical treatment for the injury. Ultrasonography and sialography were used in order to obtain an adequate diagn

Beneficial Effect of Bee Venom and Its Major Components on Facial Nerve Injury Induced in Mice.

Peripheral nerve injury (PNI) is a health problem that affects many people worldwide. This study is the first to evaluate the potential effect of bee venom (BV) and its major components in a model of PNI in the mouse. For that, the BV used in this study was analyzed using UHPLC. All animals underwent a distal section-suture of facial nerve branches, and they were randomly divided into five groups. Group 1: injured facial nerve branches without any treatment. Group 2: the facial nerve branches were injured, and the normal saline was injected similarly as in the BV-treated group. Group 3: injure
