OBJECTIVES: Despite the importance of disclosing a pathogenic/likely pathogenic (P/LP) variant related to hereditary cancer, fewer than half of index cases share their genetic test results with at-risk, biological relatives. Given this missed opportunity for cancer prevention, this study explored the barriers to family communication in Korea, from the perspective of women with hereditary breast and ovarian cancer syndrome.
METHODS: In-depth interviews were conducted with 22 women (17 affected, 5 unaffected) carrying P/LP variants in one the BRCA genes in Korea. Individual face-to-face and sma

BackgroundLimited research has examined neurocognitive decline in family caregivers of persons with Alzheimer's disease and Alzheimer's disease-related dementias (AD/ADRD), despite most caregivers being over 65 and more susceptible to age-related declines due to increased stressors. The lack of insight into how psychosocial factors interact with caregivers' neurocognitive functioning limits our ability to identify targets for reducing neurocognitive risk.ObjectiveInvestigate the effects of psychosocial factors on neurocognitive functioning of AD/ADRD caregivers.MethodsThe caregiver sample (n =

BACKGROUND: Hemiplegic migraine (HM) is a rare migraine subtype with motor aura. Familial hemiplegic migraine type 2 (FHM2), caused by ATP1A2 mutations, often mimics acute stroke, making early neuroimaging differentiation critical. While dynamic perfusion changes are documented during attacks, the dissociated neurovascular dynamics with reduced macrovascular flow signal and presumed microvascular hyperemia has not been well characterized.
CASE PRESENTATION: A 37-year-old man presented with acute severe headache and global aphasia. Initial diffusion-weighted MRI was unremarkable, but time-of-f

Intolerance of uncertainty (IU) is an individual difference dimension reflecting difficulty in tolerating aversive responses triggered by uncertainty. IU is an important etiological and maintaining mechanism of social anxiety disorder (SAD). However, IU's role as a familial risk factor for SAD remains unresolved. In the present study, we sought to 1) confirm that SAD runs in families (as per prior studies), 2) test if IU also runs in families, and 3) examine if IU is a familial risk factor for SAD. We also explored other related personality factors that could also be familial risk factors for

The Rab3 protein family is composed of a series of small GTP-binding proteins, including Rab3a, Rab3b, Rab3c, and Rab3d, termed Rab3s. They play crucial roles in health, including in brain function, such as through the regulation of synaptic transmission and neuronal activities. In the high-energy-demanding and high-traffic neurons, the Rab3s regulate essential cellular processes, including trafficking of synaptic vesicles and lysosomal positioning, which are pivotal for the maintenance of synaptic integrity and neuronal physiology. Emerging findings suggest that alterations in Rab3s expressio

HNRNPU-related neurodevelopmental disorder (HNRNPU-NDD) is caused by pathogenic and likely pathogenic variants in HNRNPU. With increasing accessibility to advanced genetic investigations, children presenting with developmental delay and intellectual disability will often undergo genomic testing; hence, the number of patients found to be affected by HNRNPU-NDD is increasing. We document a cohort of 17 previously unpublished patients with HNRNPU variants, including the first familial case, building on those previously published by our group. A comprehensive literature review was performed, ident

Heterozygous variants in the ACAN gene are a recognized cause of short stature. We aimed to evaluate the response to growth hormone (GH) treatment in members of an extended family carrying an ACAN variant. Clinical and laboratory data of five related children were retrospectively collected. GH therapy was initiated at a mean age of 3.2 ± 2.1 years and treated for 6.4 ± 5.0 years. The height standard deviation scores (SDS) increased from -3.3 ± 1.0 to -2.0 ± 1.1 (p = 0.001), with a mean gain of 1.3 ± 0.3 SDS. The most notable increase in growth velocity SDS occurred during the first year of tre

Microcephaly-related global developmental delay (GDD) and intellectual disability (ID) are characterized by a broad spectrum of neurodevelopmental impairments and encompass a multitude of causal factors. METTL5, a critical component involved in 18S rRNA methylation, has garnered considerable attention owing to its pivotal role in the pathogenesis of GDD and ID associated with microcephaly. A comprehensive physical examination and developmental assessment were performed for a 2-year-old girl presenting with symptoms of GDD and primary microcephaly. Whole-exome sequencing (WES) was performed to

Background: Neurofibromatosis type 2 (NF2), currently more accurately named NF2-related schwannomatosis (NF2-SWN), is classified as a multiple tumor syndrome, caused by impaired expression of the merlin protein. Approximately 50% of affected individuals inherit a germline mutation from their parents, while reports on the somatic mutation landscape of other genes are infrequent. Aim: To further explore the somatic mutations of NF2-SWN and provide a theoretical basis for the treatment of NF2-SWN. Design: A retrospective study was conducted to follow up on NF2-SWN patients who underwent surgical

Inherited thrombocytopenia is a rare phenomenon. MYH9-related disorder (MYH9-RD) is one such pathology characterized by thrombocytopenia and giant platelets with the presence of cytoplasmic inclusion bodies in the granulocytes. The condition is often misdiagnosed as immune thrombocytopenia (ITP) due to its similarities in clinical phenotype and often no associated secondary causes. Ensuing treatments, frequently unnecessary, may predispose to adverse outcomes or perceived a lack of improvement. We report a young lady in her 20s who was eventually found to have MYH9-RD after her second pregnanc

BACKGROUND AND OBJECTIVES: Tibial muscular dystrophy (TMD) is an autosomal dominant, slowly progressive late-onset distal myopathy. TMD was first described in 1991 by Udd et al. in Finnish patients, who were later found to harbor a heterozygous unique 11-bp insertion/deletion in the last exon of the TTN gene-the Finnish founder variant (FINmaj). In homozygous state or compound heterozygosity with a truncating variant, the FINmaj causes early-onset recessive titin-related limb-girdle muscular dystrophy type 10 (LGMD R10). So far, the FINmaj variant has not been detected outside the Finnish popu

BACKGROUND: Compared to other occupational groups, first responders (FR) experience worse mental health outcomes due to duty-related trauma and occupational stressors. Despite their best efforts, they bring this stress home to friends and family. Consequently, FR and their supporters suffer from increased psychosocial difficulties and experience stigma and other barriers to help-seeking. Prior work offers little opportunity for open dialogue and shared understanding of the repercussions of this occupation for all members of the first responder community. In this qualitative study, we aimed to:

BACKGROUND: Kidney transplantation is the treatment of choice for pediatric end-stage renal disease. Transplant recipients often have better neurocognitive, academic, and health-related outcomes. While there is some evidence that health-related quality of life (HRQOL) improves following kidney transplant, other studies have found adolescent transplant recipients report lower overall HRQOL than healthy peers. Current research has focused on individual-level factors affecting adjustment to organ transplant, warranting examination of HRQOL from a systems perspective. Family environment and social

Fibroblast growth factor (FGF) signaling encompasses a multitude of functions, including regulation of cell proliferation, differentiation, morphogenesis, and patterning. FGFs and their receptors (FGFR) are crucial for adult tissue repair processes. Aberrant FGF signal transduction is associated with various pathological conditions such as cartilage damage, bone loss, muscle reduction, and other core pathological changes observed in orthopedic degenerative diseases like osteoarthritis (OA), intervertebral disc degeneration (IVDD), osteoporosis (OP), and sarcopenia. In OA and IVDD pathologies s

Developmental and epileptic encephalopathies (DEEs) refer to a group of severe epileptic syndromes characterized by seizures as well as a developmental delay which can be a consequence of the underlying etiology and/or the epileptic encephalopathy. The genes responsible for DEEs are numerous and their number is increasing since the availability of Next-Generation Sequencing. Pathogenic variants in GRM7, encoding the metabotropic glutamate receptor 7, were recently shown as a cause of a severe DEE with autosomal recessive inheritance. To date, only ten patients have been reported in the literat

BACKGROUND: The role of proprotein convertase subtilisin/kexin type 9 (PCSK9) in dyslipidemia may go beyond its immediate effects on low-density lipoprotein receptor (LDL-R) activity.
OBJECTIVE: This study aimed to assess PCSK9-derived alterations of high-density lipoprotein (HDL) physiology, which bear a potential to contribute to cardiovascular risk profile.
METHODS: HDL was isolated from 33 patients with familial autosomal dominant hypercholesterolemia (FH), including those carrying PCSK9 gain-of-function (GOF) genetic variants (FH-PCSK9, n = 11), together with two groups of dyslipidemic
