Impaired fear inhibition is a biomarker of PTSD but not depression.

BACKGROUND: A central problem in posttraumatic stress disorder (PTSD) is a reduced capacity to suppress fear under safe conditions. Previously, we have shown that combat-related PTSD patients have impaired inhibition of fear-potentiated startle (FPS). Given the high comorbidity between PTSD and depression, our goal was to see whether this impairment is specific to PTSD, or a non-specific symptom associated with both disorders. METHODS: Fear-potentiated startle was assessed in 106 trauma-exposed individuals divided into four groups: (a) No diagnosis control, (b) PTSD only, (c) major depression

Prevalence and correlates of social fears in Hong Kong.

This study examined the 12-month prevalence of social fears and their correlates in Hong Kong. A random telephone survey of 3006 Chinese individuals aged 15-45 years was conducted using a questionnaire that covered sociodemographic information, 14 social fears, age of onset, duration of fear, DSM-IV diagnostic features of social anxiety disorder and other psychosocial features. The prevalence of any social fear was 28.7%. The most common fear was talking to a person of higher status. Respondents with social fears were more likely to be younger and female than those without. A two-step cluster

The effects of FG7142 on overexpectation of Pavlovian fear conditioning.

Six experiments studied the role of GABA-sub(A) receptor activation in expression of overexpectation of Pavlovian fear conditioning. After separate pairings of CSA and CSB with shock in Stage I, rats received pairings of the compound AB with shock in Stage II, producing overexpectation of fear. The expression of overexpectation was attenuated, in a dose-dependent manner, by the benzodiazepine partial inverse agonist FG7142. FG7142 had no effect on responding to a CS paired with a low magnitude US or a CS subjected to associative blocking. These results suggest that the negative prediction erro

Disgust, fear, and the anxiety disorders: a critical review.

Anxiety disorders have traditionally been conceptualized as reflecting the emotions of fear and anxiety. A developing program of research demonstrates a relation between disgust and three specific anxiety disorders: blood-injection-injury (BII) phobia, spider phobia, and contamination-related obsessive-compulsive disorder (OCD). This review serves three purposes. First, the authors review the response patterns predicted to be observed if the emotional response in these disorders involved disgust versus fear. The review suggests specific response patterns that characterize disgust and fear in t

The vicarious learning pathway to fear 40 years on.

Forty years on from the initial idea that fears could be learnt vicariously through observing other people's responses to a situation or stimulus, this review looks at the evidence for this theory as an explanatory model of clinical fear. First, we review early experimental evidence that fears can be learnt vicariously before turning to the evidence from both primate and human research that clinical fears can be acquired in this way. Finally, we review recent evidence from research on non-anxious children. Throughout the review we highlight problems and areas for future research. We conclude b

Spider fear and covariation bias in children and adolescents.

Covariation bias can be defined as phobic people's tendency to overestimate the association between fear-relevant stimuli and negative outcomes. The current article presents two studies that examined this type of cognitive bias in children and adolescents aged 8-16 years. Study 1 was concerned with a thought experiment during which youths (N=150) were asked to imagine that they participated in an experiment during which they had to view a series of pictures showing spiders, guns, and flowers, that were occasionally followed by a negative outcome (i.e., a mild electric shock). Participants were

The role of imagery in the maintenance and treatment of snake fear.

Two studies assessed the role of mental imagery in the maintenance of fear of common phobic stimuli. Study 1 asked participants who were afraid of a wide range of phobic stimuli to report their visual and somatic imagery. Blind ratings of the imagery on horror and vividness were positively correlated with participant's self-reported fear and avoidance. Study 2 tested the efficacy of modifying imagery using cognitive restructuring compared to in vivo exposure and a minimal exposure, relaxation control in snake fearful participants. Both active treatment groups improved significantly more than t

[Inhaled corticosteroids and the local effects--a justified fear?].

Inhaled corticosteroids continue to be hallmark players in asthma control. In time, they induced fear, hope, and created numerous discussions in specialty literature. Usually, the studies focus more on their beneficial effects and less on adverse effects. Surprisingly, lately more was written about systemic effects. A detailed review of some recent studies demonstrates that the most feared systemic effects (risk for osteoporosis due to calcium and phosphate metabolic changes, adrenal suppression, skin thinning, cataract, growth problems in children and teenagers, glaucoma) are very rare. The l

The impact of childhood sexual abuse on dental fear.

OBJECTIVE: Dental fear is a risk factor for poor oral health. Thus, treatment of dental fear is a challenge to dentists. The consequences of childhood sexual abuse (CSA) may include dental fear. A history of CSA complicates dental fear treatment, and it is often a secret. The aim of this study was to explore differences in subjective evaluations of use of dental services, experiences of dental treatment situations, dental appearance and dental problems in women who report both CSA and dental fear, and women who report dental fear only. METHODS: In an anonymous survey, 58 women with dental fea

Thought suppression in spider-fearful and nonfearful individuals.

The authors examined the suppression of spider-related thoughts in spider-fearful (n = 23) and nonfearful (n = 22) individuals. Participants were primed with vivid pictures of spiders and a story about spiders. Next, they were randomly assigned to 1 of 2 conditions: (a) suppression of thoughts associated with the previously presented spider-related stimuli or (b) free expression of any thoughts, including those related to the spider-related stimuli. All participants completed a subsequent free-expression exercise. Results indicated that spider-fearful individuals expressed thoughts about the s

The structure of feared situations in a nationally representative sample.

This study addressed the paucity of research on the covariation and hierarchic structure of fears in nationally representative or "real world" samples. Using data from the U.S. National Comorbidity Survey (N=8098), exploratory and confirmatory factor analyses were employed with split-half samples to delineate the multidimensional structure and hierarchic organization of 19 feared situations assessed by structured interview. On a first-order level, support was obtained for a five-factor model of fears: (1). agoraphobia, (2). speaking fears, (3). fears of being observed, (4). fears of heights or

Anxiety, fears, and phobias in persons with Williams syndrome.

Although much research has focused on the cognitive-linguistic profile associated with Williams syndrome, studies have yet to follow up on preliminary observations suggesting increased anxiety and fears in persons with this disorder. To this aim, Study 1 compared fears in 120 participants with Williams syndrome to 70 appropriately matched persons with mental retardation of mixed etiologies. Study 2 assessed differences in parent versus child reports of fears in 36 Williams syndrome and 24 comparison group parent-child dyads. In Study 3, rates of phobia and other anxiety disorders were assessed

Fear of bioterrorism and implications for public health preparedness.

After the human anthrax cases and exposures in 2001, the Illinois Department of Public Health received an increasing number of environmental and human samples (1,496 environmental submissions, all negative for Bacillus anthracis). These data demonstrate increased volume of submissions to a public health laboratory resulting from fear of bioterrorism.

Fear of storms and hurricanes in Antillean and Belgian children.

The present study assessed fear of hurricanes in children who had been confronted with this natural event (i.e., Antillean children, n=161). Their fear levels were compared to those of children who are unfamiliar with such an event (i.e., Belgian children, n=185). Antillean children reported significantly higher levels of fear of storms than Belgian children did, thus providing support for the notion that exposure to dangerous events promotes children's fears of those events. Surprisingly, however, Antillean children had lower scores on the 'Hurricanes' item than Belgian children. Plausibly, d

Recurrent attacks of fear and visual hallucinations in a child.

The case of a 7-year-old boy suffering from recurrent nocturnal and occasional daytime attacks with intense fear and complex visual hallucinations is presented. His state was otherwise normal, as were routine electroencephalographic (EEG) and magnetic resonance imaging (MRI) investigations in the past. Several differential diagnoses such as panic disorder, pavor nocturnus, and nightmares were considered but could not be definitely established or excluded. Since the attacks appeared after the divorce of his parents, an adjustment disorder was suspected, and the patient received psychotherapy fo

Dealing with the dangers of fear: the role of risk communication.

Among the many lessons of the homeland terrorist attacks of 2001 was that fear has powerful public health implications. People chose to drive instead of flying, thereby raising their risk of injury or death. Thousands took broad-spectrum antibiotics to prevent possible anthrax infections, thereby accelerating antimicrobial resistance. Such potentially harmful actions were taken by people seeking a sense of safety because they were afraid. This essay argues for greater emphasis on risk communication to help people keep their fears in perspective. Effective communication, not only through what t

The genetic epidemiology of irrational fears and phobias in men.

BACKGROUND: Much of our knowledge of the role of genetic factors in the etiology of phobias comes from one population-based sample of female twins. We examined the sources of individual differences in the risks for phobias and their associated irrational fears in male twins. METHODS: In personal interviews with both members of 1198 male-male twin pairs (707 monozygotic [MZ] and 491 dizygotic [DZ]) ascertained from a population-based registry, we assessed the lifetime history of agoraphobia and social, animal, situational, and blood/injury phobias as well as their associated irrational fears.
