Sacral nerve stimulation for fecal incontinence following surgery for rectal prolapse repair: a multicenter study.

PURPOSE: A proportion of patients have fecal incontinence secondary to a full-thickness rectal prolapse that fails to resolve following prolapse repair. This multicenter, prospective study assessed the use of sacral nerve stimulation for this indication. METHODS: Patients had to have more than or equal to four days with fecal incontinence per 21-day period more than one year after surgery. They had to have failed conservative treatment and have an intact external anal sphincter. RESULTS: Four female patients aged 42, 54, 68, and 65 years met the inclusion criteria. Three of the four patients…

Randomized, controlled trial of topical phenylephrine for fecal incontinence in patients after ileoanal pouch construction.

INTRODUCTION: Fecal incontinence is experienced by some patients with an ileoanal reservoir pouch. The alpha1-adrenergic agonist phenylephrine raises resting anal sphincter pressure in healthy volunteers and may be of value in these patients. METHODS: Twelve patients (7 female), median age 44 (range, 29-67) years were studied. All had fecal incontinence despite a noninflamed pouch of normal size and ultrasonographically structurally normal anal sphincter muscles. Patients were treated with topical 10 percent phenylephrine and placebo gels, allocated in random order in a double-blind, crossove…

EVALUATION OF CLINICAL ASPECTS OF PATIENTS UNDERGOING SACRAL NEUROMODULATION FOR FECAL INCONTINENCE: A RETROSPECTIVE MULTICENTER STUDY.

BACKGROUND: Fecal incontinence affects approximately 5%-10% of non-institutionalized individuals and is associated with socioeconomic burden. A meaningful proportion of patients fail conservative management and may require interventional therapies. Sacral neuromodulation has emerged as a less invasive option with favorable outcomes in clinical practice. OBJECTIVE: To describe clinical and demographic characteristics and to assess fecal incontinence severity and fecal urgency before and after sacral neuromodulation implantation in Brazilian centers. METHODS: Observational study including pati…

Sensation of Incomplete Evacuation in Patients With Fecal Incontinence: A Cross-Sectional Study of a Tertiary Referral Cohort.

INTRODUCTION: Many patients with fecal incontinence (FI) report a sensation of incomplete evacuation (SIE), a symptom that may have clinical relevance but remains insufficiently characterized. This study compared symptoms, categorized high-resolution anorectal manometry (HRAM), and health-related quality of life (HRQL) in patients with FI alone and those with FI and SIE. METHODS: In this cross-sectional study, 777 adults with FI referred to a tertiary pelvic floor unit (2015-2022) were included. All participants underwent HRAM and completed questionnaires assessing gastrointestinal symptoms a…

Functional results after ventral mesh rectopexy in 50 patients with obstructed defecation and/or fecal incontinence.

INTRODUCTION AND AIM: Ventral mesh rectopexy (VMR) for correcting rectal prolapse, rectocele, and enterocele improves obstructed defecation syndrome (ODS) and fecal incontinence (FI). This procedure is popular due to its minimally invasive approach and favorable clinical outcomes. Our aim was to evaluate the clinical response of patients that underwent VMR, focusing on changes in the ODS and FI scores. MATERIAL AND METHODS: A retrospective, observational, cohort study was conducted on patients that underwent VMR within the time frame of May 2019 and May 2024 at a high-volume hospital. Fifty c…

A case of ischemic stroke in the basal ganglia presenting with fecal and urinary incontinence as initial clinical manifestations.

Limb disorders and slurred speech are common clinical symptoms associated with acute ischemic stroke. Although urinary incontinence is a known symptom in many cases of acute ischemic stroke, the simultaneous impairment of both bowel and bladder function is relatively rare. The occurrence of fecal and urinary incontinence as the primary clinical manifestation in minor acute ischemic stroke is especially uncommon. In this study, we present the case of a 67-year-old male patient who experienced a minor acute ischemic stroke in the basal ganglia region, notably presenting with both fecal and urina

Comparison of the Fecal Incontinence Severity Scores between Self-administration by Patients and an Oral Interview by a Physician.

OBJECTIVES: To compare patients' self-administered responses to the Fecal Incontinence Severity Index (FISI) questionnaire (A1) with their responses to physician's oral interview (A3). METHODS: Patients (n=100: mean age: 72 years; 66 women) with FI completed the FISI and the modified FISI (with written explanations) questionnaires, followed by a physician interview. To identify a threshold for the rating gap between A1 and A3, we calculated each patient's mean difference in the FISI scores. RESULTS: There was no significant difference in the FISI scores between A1 and A3. A rating gap existe…

Preclinical testing of an anal bulking agent coated with a zwitterionic polymer in a fecal incontinence rat model.

Fecal incontinence is a disabling condition in which the passage of fecal material cannot be controlled. Although the condition is not life-threatening, it can seriously reduce the quality of life of a patient by isolating them from others. Though various surgical treatments are available for moderate to severe symptoms, a bulking agent is a minimally invasive technique that has attracted attention because of its safety and simple treatment process. However, the biocompatibility of bulking agent materials remains a central issue, with their durability questioned because immune responses and/or…

The Efficacy of Biofeedback Therapy for the Treatment of Fecal Incontinence After Soave Procedure in Children for Hirschsprung's Disease.

Introduction: Hirschsprung's disease is a common digestive tract malformation in children, and the Soave procedure is one of the classic surgical methods for Hirschsprung's disease (HD). Fecal incontinence is one of the most common postoperative complications that can cause significant distress to the patients and their family, the incidence of which is 20% in a recent series. Biofeedback therapy (BFT) can be an effective treatment for managing anorectal disorders, but there has been little report of the efficacy of BFT for the treatment of fecal incontinence after the Soave procedure, and the…

Advanced Management Protocol of Transanal Irrigation in Order to Improve the Outcome of Pediatric Patients with Fecal Incontinence.

BACKGROUND: Transanal irrigation (TAI) is employed for children with fecal incontinence, but it can present several problems which require a study of their outcomes among different pathologies and without a tailored work up. The aim of our study was to evaluate the effectiveness of an advanced protocol in order to tailor TAI, prevent complications, and evaluate outcomes. METHODS: We included 70 patients (14 anorectal malformation, 12 Hirschsprung's disease, 24 neurological impairment, 20 functional incontinence) submitted to a comprehensive protocol with Peristeen®: fecal score, volumetric en…
