Venous thromboembolic diseases associated with uterine myomas diagnosed before hysterectomy: a report of two cases.

Large uterine myomas (compressed veins in the pelvis), and the resulting impaired blood flow have been associated with deep vein thrombosis and pulmonary embolism. Careful detection of the preoperatively formed thrombi and strict perioperative management, such as in the following cases we report, may reduce lethal pulmonary embolism and may positively affect patient prognosis.

Case report 748: Chondroblastoma of the femur with an aneurysmal bone cyst.

A case of chondroblastoma associated with an aneurysmal bone cyst has been described. The radiographic appearance of the lesion understandably reflects the combined features of both tumors. Up to one-half of all cases of aneurysmal bone cysts are found in association with other tumors, benign or malignant, and up to 15% of chondroblastomas are combined with an aneurysmal bone cyst (4).

A case of osteoblastoma associated with severe systemic toxicity.

A case of an osteoblastoma of the proximal femur with a unique local, massive reactive periostitis mimicking osteosarcoma or osteomyelitis and unique systemic manifestations is reported. The severe toxic manifestations included: massive weight loss, chronic fever, anemia, systemic periostosis, and other signs. Due to confusion as to diagnosis, lack of response to numerous antibiotic regimens, and severe cachexia with clinical signs of impending death, an amputation was performed. Pathologic study revealed an osteoblastoma. A thorough review of the case suggests that the signs and symptoms were

Bisphosphonate-related atypical femoral fracture with bone metastasis of breast cancer: case report and review.

BACKGROUND: Intravenous bisphosphonates (BPs) have been used to reduce the frequency of skeletal-related events due to bone metastases of several kinds of cancers. Although many studies on BP-related atypical fractures (BRAFs) due to the use of BP for osteoporosis treatment have been reported, few reports on BRAFs arising as a complication of long-term BP use for bone metastasis of cancer are available. CASE REPORT: A 62-year-old woman with a history of breast cancer presented with right thigh pain after she had a fall. Radiographs indicated a transverse fracture in the shaft of the right fem

Uterine intravenous leiomyomatosis with femoral vein, intracaval, intracardiac and pulmonary artery extension.

We report a 54-year-old female with recently diagnosed multiple cardiac thrombi and pulmonary embolism that was treated with thrombolytics and anticoagulants. She presented again with worsening dyspnea and was found to have persistent large cardiac thrombi on echocardiogram. Surgical findings revealed a single right atrial mass originating from inferior vena cava and extending into the pulmonary artery. The mass was successfully removed. Final pathology revealed a benign smooth muscle and vascular mass with estrogen and progesterone receptor positivity favoring uterine intravenous leiomyoma. S

Robot-Assisted Radical Prostatectomy in a Patient with Pre-existing Femoral Crossover Bypass Graft: Multidisciplinary Management and Outcomes.

Robot-assisted radical prostatectomy (RARP) has become the standard surgical treatment for localized clinically significant prostate cancer, yet in patients with pre-existing pelvic vascular grafts, such as the iliofemoral graft, there is a significant risk of iatrogenic injury during abdominal access and pelvic dissection. This report describes a 69-year-old male with D'Amico intermediate-risk prostate cancer and a right iliofemoral polytetrafluoroethylene (PTFE) crossover bypass graft implanted for peripheral arterial disease. Major comorbidities included obesity (BMI 32 kg/m2), ASA III stat

Salvage of Distal Femoral Replacement Loosening with Massive Osteolysis Using Impaction Grafting: A Report of 2 Cases.

CASE: Salvage of 2 cases of distal femoral replacement loosening with massive osteolysis using impaction grafting are presented with 9- and 11-year follow-ups. CONCLUSION: Surgeons should keep impaction grafting in their armamentarium for cases of failed DFR with severe osteolysis. Doing so may allow for preservation of the native hip and deferment of more radical procedures (i.e. total femur replacement) that have high rates of complication and poor survivorship.

The limitations of today's clinical guidance: Atypical femoral fracture and long-term bone-modifying agents in the oncology setting.

While bone-modifying agents such as bisphosphonates and denosumab are crucial to preventing skeletal-related events in patients with bone metastases, the optimal duration remains undefined. Extended duration may be associated with adverse effects such as osteonecrosis of the jaw and atypical femoral fracture. Although uncommon, atypical femoral fracture represents a serious consequence of prolonged bone-modifying agent use and are characterized by a prodrome and distinct radiographic findings. The oncology setting encompasses a unique set of atypical femoral fracture risk factors and considera

Exploration of the Optimal Shape for Bone Tumour Biopsy.

BACKGROUND/AIM: Biopsy hole for bone tumour biopsy may cause pathological fractures. This study aimed to identify the optimal shape of bone tumour biopsy hole using the rabbit femoral head compression test to avoid pathological fracture. MATERIALS AND METHODS: A compression test with no defect was performed to identify bone fracture location. Three shapes of biopsy holes (same size) were made artificially. Sixty rabbit femurs were randomly divided (n=15 each) into control (no defect), Shape 1 (round), Shape 2 (square), and Shape 3 (rectangular) groups. RESULTS: Twelve out of fifteen femurs w

Pathologic fractures of the distal femur: Current concepts and treatment options.

Pathologic fractures of the distal femur caused by bone metastases are not as common as those in the proximal femur but provide great difficulty to adequately treat. This systematic review shows that insufficient literature exists to draw clinically relevant conclusions for essential questions, such as "what factors indicate an endoprosthetic reconstruction for distal femur pathologic fractures?" Due to paucity of literature in the systematic review, a current concepts review (including treatment flowchart), based on instructional reviews and experience, was also performed.

Trauma-Induced Resolution of Solitary Osteochondroma of the Distal Femur.

Osteochondromas are benign lesions manifested as bony protrusions capped by cartilage. The exact cause of these growths is not known, and there is no treatment other than surgical excision if the lesion becomes symptomatic. Spontaneous resolution is an uncommon phenomenon that is not completely understood. A 12-year-old girl presented with a mass behind the left knee diagnosed as an osteochondroma. She was followed with serial radiographs because the lesion was minimally symptomatic. At 2.5 years after presentation, the patient reported feeling a "pop" with knee hyperflexion, and radiographic

Rare massive osteolipoma in the upper part of the knee in a young adult.

Lipoma is a common benign soft tissue tumor. This article describes a massive osteolipoma, an unusual lipoma that is fixed to the femoral periosteum. A 21-year-old man presented with a subcutaneous mass at the knee region, which had been present for more than 36 months, and a slight limitation of joint flexibility. On physical examination, a mass approximately 12×6×2 cm(3) in dimension was palpable beneath the skin at the knee proximal medial area when the knee was in flexion. The mass was ovoid, hard, nontender, well demarcated, large, subcutaneous, and relatively fixed to the femur. Medical

[Pathological proximal femur fracture: consider also primary bone tumour].

Two male and one female patient, aged 64, 70 and 51 respectively, were surgically treated for pathological fracture of the proximal femur without preoperative biopsy. In contrast to their benign radiological diagnosis, all three patients were finally diagnosed as having a malignant primary bone tumour. The proximal femur is the primary location of pathological fractures in the appendicular skeleton. Metastases to bone are the most common cause of a destructive lesion of the skeleton in an adult. Although rare, a primary bone tumour must be included in differential diagnosis of a pathological f

Osteosarcoma occurring in osteogenesis imperfecta tarda.

The combination of osteogenesis imperfecta (OI) and osteosarcoma is rare, as reflected by the fact that there are only eight proven cases in the published English literature. We present here another case, one that was treated by modern oncological techniques and major limb salvage surgery in the form of distal femur endoprosthesis initially and later a total femur replacement. The patient is the only one of the series to have had major limb salvage and is the only--and longest--surviving patient (more than 6 years), thus disproving the probability that this combination of disorders is particul

[Embolism and intramedullary femoral surgery].

All intramedullary femoral surgery entails embolic phenomena which explain peroperative collapses formally known as bone cement implantation syndrome, as well as perioperative fat embolism syndromes. Locally, the bigger the cavity is, the higher the number of accidents: 2.5-5 per cent for GUEPAR hinged-knee prosthesis, 1.75 per cent for total hip arthroplasty with long stem, and 0.1 per cent during classic THA with cement limited to the metaphysis. Anomalies in bone vascularization also increase risk: 10.5-13 per cent during prophylactic nailing for shaft metastases, 1-11.5 per cent during hem
