[Reconstruction using non-vascularized fibula of an aneurysmal bone cyst of the humerus in a child].

Aneurysmal bone cyst is a pseudotumoral bone dystrophy which accounts for 1.5 to 4% of benign bone tumours. We report the case of a bulky aneurismal cyst of the left superior humeral metaphysis in a seven year old child revealed by a repeated pathologic fracture. Exeresis of the cyst was performed with reconstruction by a non vascularized fibula. After a follow up of one year the patient recovered safely with normal range of motion and anatomically. The cystic exeresis sparing the growth cartilage with fibula graft is indicated in the management of active and aggressive aneurismal cyst of long

Combined surgery with 3-in-1 osteosynthesis in congenital pseudarthrosis of the tibia with intact fibula.

BACKGROUND: Re-fracture is the most serious complication in congenital pseudarthrosis of the tibia (CPT). There are reports that children with small cross-sectional areas in the sections of the pseudarthrosis are more prone to re-fracture. Presently, preventing complications is a challenge. Increasing the cross-sectional area in healed segments may reduce the incidence of re-fracture. PURPOSE: To elucidate the indications, surgical technique, and outcomes of combined surgery and 3-in-1 osteosynthesis in CPT with intact fibula. METHODS: We retrospectively assessed 17 patients with Crawford Ty

Surgical treatment of tibial plafond fractures.

Intra-articular fractures of the tibial plafond are typically the result of rotational or axial loading forces, and both mechanisms of injuries can result in an associated fibula fracture. Rotational distal tibial plafond fractures are typically of lower energy and are associated with less articular injury and chondral impaction, whereas axial load injuries of the distal tibial plafond are associated with a higher incidence of intra-articular and soft tissue injury. The goal of this article is to review the mechanisms of injury, fracture patterns, and potential complications associated with th

Contraindications to the repositioning of fractured or dislocated limbs in the fields.

Supracondylar fractures of the femor or humerus and fractures of the radius, ulna, tibia or fibula, plus crush injuries of the limbs, most likely should not be manipulated in the field because of the potential for traumatizing adjacent nerves, veins and arteries. An emergency splinting system that allows immobilization without repositioning has been developed. Furthermore, fractures in or about the elbow, wrist, knee, or ankle should not be subjected to reduction in the field, unless there are compelling reasons to justify the risk of producing significant additional trauma. Conservative immob

[Fractures of the anterolateral tibial rim : The fourth malleolus].

The anterolateral tibial rim with the anterior tibial tubercle (Tubercule de Tillaux-Chaput) serves as an insertion site of the anterior inferior tibiofibular ligament (AITFL). It can also be termed the anterior malleolus or fourth malleolus. Fractures of the anterolateral tibial rim typically result from an external rotation or abduction mechanism of the talus within the ankle mortise. They are frequently overlooked in plain radiographs. Computed tomography (CT) is needed for an exact visualization of the fracture anatomy and treatment planning. A total of three main types can be differentiat

[Reconstruction of the long bones of the legs in children via free vascularized transplant of the fibula after resection for sarcoma].

Nineteen children were operated on between 1985 and 1994. All the patients presented a sarcoma of long bones: osteosarcoma: 12 and Ewing's sarcoma: 7. They were operated on: resection and reconstruction of the long bones with a free vascularised fibula. Pre and post-operative chemotherapy was used. The average follow-up was 3 years (between 10 years and 1 year). The mean bone defect was 20 cm (between 32 and 11 cm). Boys were more frequently encountered [12]. Average age was 9.5 years. The pathological bone was: femur: 14, tibia: 4, humerus 1. The approach needs usually two incisions for femor

Minimally Invasive Treatment for Tibial Malrotation after Locked Intramedullary Nailing.

Rotational malreduction is a potential complication of intramedullary nailing for tibial shaft fractures. We experienced a symptomatic case of a 24° externally rotated malunion that we treated with minimally invasive corrective osteotomy. A 49-year-old man sustained a tibial shaft spiral fracture with a fibula fracture. He had been initially treated elsewhere with a reamed statically locked intramedullary nail. Bone union had been obtained, but he complained of asymmetry of his legs, difficulty walking and running, and the inability to ride a bicycle. We decided to perform corrective osteotomy

Double segmental tibial fractures--an unusual fracture pattern.

A case of a 50-year-old pedestrian who was hit by a bike and suffered fractures of both bones of his right leg was presented. Complete clinical and radiographic assessment showed double segmental fractures of the tibia and multisegmental fractures of the fibula. Review of the literature revealed that this fracture pattern was unique and only a single case was reported so far. Moreover, we discussed the possible mechanisms which can lead to such an injury. We also discussed the management of segmental tibial fracture and the difficulties encountered with them. This case was managed by modern os

Management of Infected Gap Non-union of Distal Femur with Induced Membrane Technique using Limb Reconstruction System Followed by Augmentation Plating and Bone Grafting: A Case Report.

INTRODUCTION: Distal femur fractures are high-velocity injuries which accounts for 7-10% of all femoral fractures of which 5-10% are open fractures. Despite advances in techniques and implants, treatment remains a challenge, in many situations due to increased risk of infection, non-union and loss of range of motion. Surgical management of such complex injury includes radical debridement with stabilization followed by management of gap non-union with appropriate techniques and restores the range of motion. CASE REPORT: We present a case of 20-year-old man who had suffered multiple orthopedic

An Injured Psittacosaurus (Dinosauria: Ceratopsia) From the Yixian Formation (Liaoning, China): Implications for Psittacosaurus Biology.

We describe a Psittacosaurus specimen from the Lujiatun beds of the Yixian Formation in Liaoning, China with an abnormality on its left fibula. Although a large number of Psittacosaurus specimens are known, only a single example of a pathologic Psittacosaurus has been previously noted. The specific pathology in the current specimen is believed to be a healed fibular fracture as assessed through a combination of gross morphology, microcomputed tomography (microCT), and histology data. The fracture can be identified using microCT, but the degree of remodeling and the stage of fracture repair are

Fixation of anterolateral distal tibial fractures: the anterior malleolus.

OBJECTIVE: The anterior tibial rim with the anterolateral tibial tubercle provides attachment to the anterior tibiofibular syndesmosis. It may be considered an anterior or "fourth" malleolus. Fixation of a displaced anterior tibial fragment in the treatment of malleolar fractures aims at providing a bone-to-bone fixation of the anterior tibiofibular ligament and restoring the integrity of the tibial incisura. INDICATIONS: Displaced intra-articular fragments of the anterior tibia; fractures involving the tibial incisura; fractures with intercalary fragments; impaction of the anterior tibial pl

Predictors of a persistent dislocation after reduction of syndesmotic injuries detected with intraoperative three-dimensional imaging.

BACKGROUND: In about 25% of cases, reduction of acute unstable syndesmotic injuries and stabilization with syndesmotic screws leads to an inadequate reduction. Conventional fluoroscopy does not provide reliable information about the reduction outcome. However, use of intraoperative 3D imaging can be more accurate. The purpose of this study was to identify predictors of inadequate reduction so that the need for intra- or postoperative 3D imaging could be assessed. Our hypothesis was that complex injuries of the syndesmosis present a higher risk of malreduction than simpler ankle fractures. MET

An Uncommon Complication of Ankle Varus During Tibial Transport in an Ilizarov Frame: A Case Report.

CASE: We treated a 22-year-old man with an infected fracture of the tibia by resection of necrosed bone and bone transport with an Ilizarov device. Four weeks after initiation of transport, an ankle varus deformity from distal migration of the fibula was noticed. A wire that had been incorrectly placed through both the fibula and the transport fragment of the tibia was identified as responsible for this complication. It was changed after an acute reversal of the transport. CONCLUSION: Errors in the surgical technique during the insertion of Ilizarov wires can result in unusual complications.
