Factors Predicting Treatment of World Trade Center-Related Lung Injury: A Longitudinal Cohort Study.

The factors that predict treatment of lung injury in occupational cohorts are poorly defined. We aimed to identify patient characteristics associated with initiation of treatment with inhaled corticosteroid/long-acting beta-agonist (ICS/LABA) >2 years among World Trade Center (WTC)-exposed firefighters. The study population included 8530 WTC-exposed firefighters. Multivariable logistic regression assessed the association of patient characteristics with ICS/LABA treatment for >2 years over two-year intervals from 11 September 2001-10 September 2017. Cox proportional hazards models measured the …

Double Jeopardy: Hearing Loss and Tinnitus Among Noise-Exposed Workers.

The purpose of this study was to determine the prevalence and characteristics of tinnitus and assess the relationship between tinnitus and hearing loss among firefighters and operating engineers, who are exposed to noise on-the-job. The study analyzed existing data from two different populations (154 firefighters and 769 operating engineers) who completed a survey and audiometric tests as part of a hearing loss prevention intervention study. Approximately 40% of both groups reported tinnitus; 34% of firefighters and 59% of operating engineers showed hearing loss at noise-sensitive frequencies …

Evaluation of the Implementation of the NFFF Stress First Aid Intervention in Career Fire Departments: A Cluster Randomized Controlled Trial.

Firefighting is inherently dangerous, though recently concerns have shifted from traditional fireground injuries (burns and asphyxiation) to a focus on mental and behavioral health. Although firefighters are remarkably resilient, research suggests many suffer negative psychological consequences from repeated exposures to trauma. While the Stress First Aid (SFA) model has gained increased attention and adoption among fire departments as a model for behavioral health training, it has not been formally evaluated. This cluster randomized controlled trial used a crossover design comparing the immed…

Cancer morbidity of professional emergency responders in Korea.

BACKGROUND: Many professional emergency responders (ERs) who belong to the Korean National Emergency Management Agency (NEMA) have been cross-trained and serve multiple roles. As such, firefighters and other ERs in Korea are exposed to similar occupational hazards. This study was conducted to estimate cancer morbidity in male ERs and compare that with Korean men. METHODS: The cohort was comprised of 33,416 male ERs working between 1980 and 2007, who were alive on December 31, 1995. Work histories were merged with the Korea National Central Cancer Registry (KNCCR) to assess cancer morbidity be…

Performance requirements of physically strenuous occupations: validating minimum standards for muscular strength and endurance.

Employment rights legislative mandates passed in the USA over the last three decades emphasize the importance of validating performance standards for physically strenuous occupations. This study validated minimally acceptable standards for the muscular strength and endurance necessary to perform fire suppression activities. Incumbent firefighters (n=153) selected for key demographic characteristics completed a simulated set of firefighting tasks (Fire Suppression Evolution) and then a Predictor Test Battery of physical abilities tests. Regression analysis revealed that three predictor test ite…

Maladaptive self-appraisals before trauma exposure predict posttraumatic stress disorder.

This study tested the proposal that negative appraisals represent a risk factor for developing posttraumatic stress disorder (PTSD) after trauma. Trainee firefighters (N = 68) were assessed during training (before trauma exposure) for PTSD, history of traumatic events, and tendency to engage in negative appraisals. Firefighters were reassessed 4 years later (N = 52), after commencing firefighter duty (after trauma exposure), for PTSD and depression using the Posttraumatic Stress Disorder Scale (E. B. Foa, L. Cashman, L. Jaycox, & K. Perry, 1997) and the Beck Depression Inventory (Version 2; A.…

Exposure to metals and semivolatile organic compounds in Australian fire stations.

Firefighting is an occupation with exposure to a wide range of chemicals by means of inhalation, ingestion or dermal contact. Although advan … fer of chemicals from fire suppression operations. Due to this elevated concentration of chemicals, firefighters may face increased exposure, and in turn increased risk of adverse health effects. Data suggest that exposure may be mitigated by means of increased laundering frequency and increased decontamination at the scene of the fire.

Pushing and pulling: personal mechanics influence spine loads.

This study assessed several mechanical issues related to low back loading during pushing and/or pulling tasks. Nine male participants performed two-handed pushing and pulling tasks at two handle heights with three loads, using a cable pulley system. Four of these men were professional firefighters trained in performing pushing and pulling tasks while the other five were graduate students who lacked manual work experience. The more experienced firefighters produced less spinal compression and shearing forces when compared to the less experienced students under the same conditions. The firefight…

Use of respiratory protection among responders at the World Trade Center site--New York City, September 2001.

The terrorist attacks on the World Trade Center (WTC) on September 11,2001, created an occupational health and safety challenge for New York City (NYC) firefighters and rescue workers responding to the disaster. Immediate respiratory hazards included explosions, fire, falling debris, and dust clouds containing particulate matter comprised of pulverized building materials. Ongoing risks included lingering particulate matter in the air and intermittent combustion products from initial and persistent fires beneath the rubble pile. Because the nature and extent of exposures in disaster situations …

Overexertion and heat stress in the fire service: a new conceptual framework.

From the year 2000, the United States Fire Administration (USFA) has been recording all line-of-duty deaths in the fire service. Stress or overexertion caused 1096 out of 2598 total line-of-duty deaths in the United States from 2000 to 2021. Those deaths due to stress or overexertion were further classified as myocardial infarction (90%), cerebrovascular accident (6.8%), other (2.6%), and heat exhaustion (0.6%). Environmental heat exposure is a concern in firefighting, as firefighters work in extreme conditions, such as high ambient temperatures, while wearing protective clothing. Heat stress …

Effects of using exposure footwear for firemen and fitness training shoes on lower limb biomechanics during walking.

Exposure footwear for firemen (EFF) is a crucial component of firefighters' personal protective equipment, designed to protect against hazards, such as heat, punctures, impact, and electrocution. However, its effect on gait biomechanics remains unclear. This study was aimed to investigate the influence of EFF on firefighters' lower limb biomechanical performance and the underlying mechanisms. Twenty healthy male firefighters (mean height: 175.41 ± 4.39 cm, mean weight: 65.74 ± 5.81 kg, mean BMI: 21.37 ± 2.17 kg/m2) were randomly assigned to wear EFF and fitness training shoes for comparison du…
