The foot drawing method: reliability of measuring foot length and outward rotation in children with clubfoot.

BACKGROUND: The Ponseti method is the gold standard for clubfoot treatment. However, relapse and residual gait deviations are common, and follow-up until 7 years of age is recommended. We evaluated the reliability of the foot drawing method, a new instrument for the follow-up of clubfoot. The method uses drawings of the foot in the neutral position and external rotation to measure foot length and outward rotation. METHODS: Nineteen children aged 2.5-7 years who were treated with the Ponseti method for congenital clubfoot were included. Two raters made the drawings twice (D1 and D2). Each rate…

The role of foot pressure measurement in the prediction and prevention of diabetic foot ulceration-A comprehensive review.

The predominant risk factor of diabetic foot ulcers (DFU), peripheral neuropathy, results in loss of protective sensation and is associated with abnormally high plantar pressures. DFU prevention strategies strive to reduce these high plantar pressures. Nevertheless, several constraints should be acknowledged regarding the research supporting the link between plantar pressure and DFUs, which may explain the low prediction ability reported in prospective studies. The majority of studies assess vertical, rather than shear, barefoot plantar pressure in laboratory-based environments, rather than du…

Evaluation of the paediatric foot using footprints and foot posture index: A cross-sectional study.

AIM: Footprints have long been used as proxy measures of foot morphology, yet there is little consensus regarding footprints versus measures of foot posture, which address foot anatomy directly. Foot posture in children can be a confusing clinical presentation, with previous studies both supporting and refuting the relationship between childhood obesity and flat feet. The aim of this study was to determine the relationship between footprints and foot posture in children. METHODS: A total of 316 school children (153 boys, 163 girls) from Spain, aged 6-9 years, were assessed for both footprint …

A Forefoot Strike Requires the Highest Forces Applied to the Foot Among Foot Strike Patterns.

Ground reaction force is often used to predict the potential risk of injuries but may not coincide with the forces applied to commonly injured regions of the foot. This study examined the forces applied to the foot, and the associated moment arms made by three foot strike patterns. 10 male runners ran barefoot along a runway at 3.3 m/s using forefoot, midfoot, and rearfoot strikes. The Achilles tendon and ground reaction force moment arms represented the shortest distance between the ankle joint axis and the line of action of each force. The Achilles tendon and joint reaction forces were calcu…

A prospective study of risk factors for foot ulceration: the West of Ireland Diabetes Foot Study.

BACKGROUND: This is the first study to examine risk factors for diabetic foot ulceration in Irish general practice. AIM: To determine the prevalence of established risk factors for foot ulceration in a community-based cohort, and to explore the potential for estimated glomerular filtration rate (eGFR) to act as a novel risk factor. DESIGN: A prospective observational study. METHODS: Patients with diabetes attending 12 (of 17) invited general practices were invited for foot screening. Validated clinical tests were carried out at baseline to assess for vascular and sensory impairment and foot…

A new protocol for 3D assessment of foot during gait: application on patients with equinovarus foot.

BACKGROUND: The aim of this study is to assess the clinical value of a recently introduced original protocol for full three dimensional analysis of ankle rotations in patients with equinovarus foot. METHODS: A preliminary study merging the Total3Dgait protocol and the conventional Vicon® Plug-in-Gait marker-sets on five patients with foot deformity was performed to compare the output exactly over the same gait cycles. In the second study, 15 patients with equinus varus foot were assessed retrospectively by means of the Total3Dgait protocol before and after surgery. Data on ankle kinematics we…

Magnetic resonance imaging characteristics of the foot in horses with palmar foot pain and control horses.

Palmar foot pain is a common cause of lameness. Magnetic resonance imaging (MRI) has the potential to detect damage in all tissues of the equine foot, but an understanding of the differences in magnetic resonance (MR) images between feet from horses with and without palmar foot pain is required. This study aimed to describe MR findings in feet from horses with no history of foot-related lameness, and to compare these with MR findings in horses with lameness improved by palmar digital local analgesia. Thirty-four limbs from horses euthanized with a clinical diagnosis of navicular syndrome (amen…

Approach to Partial Foot Amputations.

Diabetes mellitus is a global health concern, with complications such as peripheral neuropathy and peripheral arterial disease (PAD) leading to diabetic foot ulcers, infections, and amputations. Chronic limb-threatening ischemia, a severe form of PAD, further impairs healing and increases amputation risk. Limb-sparing procedures like partial foot amputations aim to preserve mobility and improve outcomes. Neuropathy contributes to unnoticed injuries and poor skin integrity, while abnormal foot biomechanics from deformities increase pressure points, promoting ulcer formation. Early diagnosis, pr…

Foot Care for Muslim Patients.

Islam is the fastest growing religion in the United States and the world, and the number of Muslim patients who foot and ankle specialists will encounter is expected to increase as a result. Therefore, it is important to understand the Muslim patients' emphasis on modesty to ensure comfort during the patient encounter. The provider should understand the permissibility of animal-derived products, especially as it relates to lower-extremity wound care and surgical products. Specific rituals that are pillars of Islam, such as prayer, fasting in Ramadan, and Hajj pilgrimage, all have direct relati…

Fibroblasts in Diabetic Foot Ulcers.

Fibroblasts are stromal cells ubiquitously distributed in the body of nearly every organ tissue. These cells were previously considered to b … litus (DM), which represent the current major causes of morbidity and mortality worldwide. Diabetic foot ulcer (DFU), one of the most severe complications of DM affects 40 to 60 million people. Chronic non-healing DFU wounds expose patients to substantial sequelae including infections, gangrene, amputation, and death. A complete understanding of the pa …

Surgical Management of Foot Drop.

A one-and-a-half-year-old male Wire Fox Terrier weighing 3 kg presented with continuous drooling and vomiting. Its body condition score was 3/9, and severe atrophy of the temporal/masticatory muscle, trismus, and enophthalmos was observed on physical examination. The radiographic examination and fluoroscopic oesophagography revealed a type 1 hiatal hernia with gastroesophageal reflux. The serology tests revealed increased muscle enzyme activities. The antibody tests for acetylcholine receptor, type 2M fibre, and infectious agents were negative. A conventional surgical treatment was performed,

Diabetic foot problem in Nepal.

INTRODUCTION: Nepal is a developing country where diabetes is becoming a major health challenge due to its high prevalence of 8.5% affecting around 2 million people. Due to limited resources, there are many barriers to providing affordable and convenient diabetes care or regular screening for complications. There is no reliable data on incidence, prevalence, and complications of diabetic foot problems in Nepal. METHODS: We conducted an online survey amongst senior physicians, who were members of 'Diabetes & Endocrine Association of Nepal' to assess their perception of diabetic foot problems i…

Diabetic Foot Ulcers: A Review.

IMPORTANCE: Approximately 18.6 million people worldwide are affected by a diabetic foot ulcer each year, including 1.6 million people in the United States. These ulcers precede 80% of lower extremity amputations among people diagnosed with diabetes and are associated with an increased risk of death. OBSERVATIONS: Neurological, vascular, and biomechanical factors contribute to diabetic foot ulceration. Approximately 50% to 60% of ulcers become infected, and about 20% of moderate to severe infections lead to lower extremity amputations. The 5-year mortality rate for individuals with a diabetic …

Diabetic foot ulcer: treatment challenges.

Wound care professionals face complex challenges in their attempts to achieve healing in patients with a diabetic foot ulcer (DFU).1 Peripheral neuropathy limits sensation, which necessitates consistent offloading of the DFU site. Peripheral arterial, venous, or lymphatic circulation disorders must be diagnosed and addressed. Early diagnosis and management of soft tissue or bone infection is required. Chronic kidney disease, glycemic control, and nutritional needs must be addressed along with counterproductive behavior patterns.2 The use of topical hydrocolloid-based hydrogel dressings3 or hon…

Debridement of Diabetic Foot Ulcers.

Diabetic foot ulcerations have devastating complications, including amputations, poor quality of life, and life-threatening infections. Diabetic wounds can be protracted, take significant time to heal, and can recur after healing. They are costly consuming health care resources. These consequences have serious public health and clinical implications. Debridement is often used as a standard of care. Debridement consists of both nonmechanical (autolytic, enzymatic) and mechanical methods (sharp/surgical, wet to dry debridement, aqueous high-pressure lavage, ultrasound, and biosurgery/maggot debr…
