Peroneal Tendon Subluxation and Dislocation in Calcaneus Fractures.

The present study investigated the relationship between type of calcaneal fractures and subluxation or dislocation of peroneal tendon. Also, we investigated clinical outcomes of patients with both calcaneal fractures and dislocations or subluxations of peroneal tendons in early surgical treatments (at the time of surgery for calcaneal fractures) and delayed surgical treatment (at the time of surgery for calcaneal plate removal) for dislocations or subluxations of peroneal tendons. We included 151 patients with calcaneal fractures who were followed for ≥2 years after surgery. Among them, 21 cas

Complex Fracture-Dislocations of the Elbow in the Pediatric Population.

BACKGROUND: Complex fracture-dislocations of the elbow, including terrible triad of the elbow, are serious injuries with guarded outcomes in adults. Although described extensively in adults, little is known about similar complex fracture-dislocations of the elbow in the pediatric population. The purpose of this study was to describe patterns of elbow dislocations with associated fractures in children and report the outcomes of these injuries. METHODS: This was a retrospective review of patients who presented to a level I trauma center from 2007 to 2019 with an elbow dislocation and at least 2

Acute Fractures and Dislocations of the Ankle and Foot in Children.

Distinct biologic and mechanical attributes of the pediatric skeleton translate into fracture patterns, complications, and treatment dilemmas that differ from those of adults. In children, increasing participation in competitive sports activities has led to an increased incidence of acute injuries that affect the foot and ankle. These injuries represent approximately 13% of all pediatric osseous injuries. Important posttraumatic complications include premature physeal arrest, three-dimensional deformities and consequent articular incongruity, compartment syndrome, and infection. The authors de

Management of Proximal Interphalangeal Joint Fracture Dislocations.

Fracture dislocations of the proximal interphalangeal (PIP) joint of the finger are often caused by axial load applied to a flexed joint. The most common injury pattern is a dorsal fracture dislocation with a volar lip fracture of the middle phalanx. Damage to the soft-tissue stabilizers of the PIP joint contributes to the deformity seen with these fracture patterns. Unfortunately, these injuries are commonly written off and left untreated. A late-presenting PIP joint fracture dislocation has a poor chance of regaining normal range of motion. The provider must be suspicious of these injuries. …

Adult Monteggia and Olecranon Fracture Dislocations of the Elbow.

Monteggia fractures and olecranon fracture dislocations represent complex injuries with distinct patterns of bony and soft tissue involvement. Fractures of the proximal ulna and olecranon process may lead to disruption of the proximal radioulnar joint and/or ulnohumeral joint. The keys to treatment are recognition of the pattern of injury and formation of an algorithmic surgical plan to address all components of the injury process. Complications are common and may be related to the injury spectrum itself and/or inadequate fracture alignment or fixation.

[Reconstructions after inveterated fractures and dislocations of the foot].

Inveterated fractures or dislocations of the foot which are defined as being more than 6-12 weeks old can be secondarily reconstructed anatomically in only very rare cases. All other cases need reorientation of all axes of the foot requiring single or combined joint fusion. In cases of significant posttraumatic bone necrosis the dead bone has to be radically resected and substituted by an autogenous bone graft from the iliac crest. Old cases of dislocation, for example after subtalar dislocation without anatomical reduction, need reorientation of the foot axes by performing arthrodesis. Sublux

3-D analysis of dislocation in zygoma fractures.

OBJECTIVE: When fractured, zygomas rotate and dislocate. The present study quantitatively elucidates the pattern of the rotation. METHODS: 50 patients with tri-pod-type zygoma fractures were involved in this study. After defining a 3-dimensional coordinate system--consisting of the M-L axis (the axis directed from the medial to lateral side of the skull), I-S axis (directed from the inferior to superior side), and P-A axis (directed from the posterior to anterior side), the degree with which the fractured zygomas rotated around each of these axes was measured using 3-dimensional graphic softw

Fractures and dislocations of the midfoot: Lisfranc and Chopart injuries.

The midfoot is a complex association of five bones and many articulations between the forefoot metatarsals and the talus and calcaneus, which make up the hindfoot. These anatomic relationships are connected and restrained by an even more complex network of ligaments, capsules, and fascia, which must function as a unit to provide normal and painless locomotion. The common eponyms of Lisfranc and Chopart refer to the distal and proximal joint relationships of the midfoot, respectively. Midfoot injuries range from single ligament strains to complicated fracture-dislocations involving multiple bon

Diabetic Lisfranc fracture-dislocations and Charcot neuroarthropathy.

The goal with Lisfranc fracture-dislocations is to regain joint congruity and reestablish midfoot stability to avoid debilitating posttraumatic arthrosis and chronic pain in the sensate patient. In the diabetic population, dense peripheral neuropathy and/or vascular disease are equally important and may alter the surgical approach to traumatic tarsometatarsal injuries. The initial diagnosis in the diabetic population may be delayed due to subtle radiographic findings and/or patient unawareness of trauma in the insensate foot. Failure to initiate treatment in the early stages of acute diabetic

Open ankle dislocations without associated fracture.

BACKGROUND: This study aimed to clarify the intermediate followup results of reduction without soft tissue repair in open ankle dislocations without fracture. METHODS: We retrospectively reviewed cases of open isolated ankle dislocations without associated fracture. Five male patients treated between January 2005 and July 2009 were enrolled in this study. Debridement and reduction with primary wound closure of the skin were performed without suture of the capsule or repair of the ligaments. The outcome was evaluated by the American Orthopaedic Foot & Ankle Society score (AOFAS). The time peri

Fixation of the coronoid process in elbow fracture-dislocations.

BACKGROUND: Terrible triad injuries consist of a posterior dislocation of the elbow, a coronoid fracture, and a radial head fracture. The coronoid plays a pivotal role as an anterior buttress, yet the optimal management of the coronoid fracture remains unknown. We hypothesize that suture lasso fixation of the coronoid fracture leads to fewer complications and improved outcomes compared with screw or suture anchor fixation techniques. METHODS: A retrospective chart review performed at three tertiary care centers identified forty consecutive patients treated for terrible triad injuries of the e

Surgical dislocation of the hip for fractures of the femoral head.

Traumatic dislocations of the hip are high-energy injuries that frequently occur with fractures of the femoral head. Controversy exists regarding many aspects of the treatment of these fractures, but following reduction, large displaced head fragments require open reduction and internal fixation. Traditionally, an anterior approach to the hip joint has been used for surgical access to the femoral head, but this incision often results in limited visualization and subsequent difficulty with anatomic reduction. Recently, a surgical hip dislocation technique has been described for acetabular fract

[Surgical treatment of dislocated transverse fractures of the sacrum].

PURPOSE OF THE STUDY: Transverse fractures of sacrum are quite rare. Only about 70 cases have been reported in the relevant literature. Low frequency and rather difficult fracture imaging lead to therapeutical uncertainty and are a cause of a delay of urgent surgery. MATERIAL AND METHODS: A group of three male patients aged 17.43 and 31 years was studied. Two patients sustained the injury at a paragliding accident, one by fall from height. One patient underwent only a simple spinal canal decompression 24 days after the injury. The second patient underwent surgery on the fourth day and in the

Management of proximal interphalangeal joint fractures and dislocations.

Proximal interphalangeal joint (PIP) injuries are among the most common in the hand and their severity is often underestimated. These injuries often lead to prolonged disability, pain, and stiffness. Appropriate treatment includes a thorough assessment, physical examination, and directed imaging. Such an approach should lead to a rational treatment plan that focuses on the rehabilitation of all damaged components, including osseous, articular, and soft tissue structures. This article reviews all elements in the management of PIP injuries and introduces an assessment method for PIP injuries bas

[Special positioning aids in fractures and dislocations of the spine].

Early recognition of spine injuries is of outmost importance. In the case of suspected fractures and in all severe skull injuries, a careful approach and careful positioning of the injured patient is mandatory. Immobilization of the spine is the prime concern during transportation of trauma patients to prevent neurologic compromise. We prefer the supine position on a stable support. In the case of cervical spine injury, immobilisation with a cervical collar in extension is necessary. Definitive transportation should be performed on a vacuum mattress supported by a scoop-stretcher. In fractures

Isolated fracture-dislocations of the first tarsometatarsal joint.

Eight cases of isolated fracture-dislocation of the first tarsometatarsal joint are presented. This is an uncommon injury and a distinct entity from other tarsometatarsal joint injuries. Reduction of both the fracture and the dislocation is easy but is unstable. Kirschner wire and screw fixation adequately stabilizes the reduction and gives good results. Results of conservatively treated cases are usually poor, since redisplacement in plaster is quite common.

[Epiphyseal fractures-dislocations of the lower extremity of the tibia].

Ninety six distal tibial epiphyseal fractures were identified and treated in our institution from 1976 to 1988. The average age was twelve years and eight months (range two to seventeen years), but seventy-one were between eleven and fourteen years old. Using the Salter-Harris classification we have found twelve type 1 tibial fractures, fourty-two type 2, thirty type 3 and twelve type 4. Four were triplane fractures and seven were Tillaux fractures. Twenty-six had injuries in the medial corner of the ankle mortise (Mac-Farland). Fifty patients were treated non-operatively with closed reduction

Complex ankle fracture dislocations with syndesmotic diastasis.

Ankle fracture dislocations with fibular "diastasis" require anatomic reduction and usually internal fixation for acceptable clinical results. This review discusses anatomic, radiologic, and clinical aspects in the treatment of these injuries. Clinical examples are given.

The conservative treatment of the fractures and dislocations of the extremities in children.

Both authors have dedicated most of their time since 1948 to the treatment of fractures in children. On the basis of their experience they are therefore submitting to the medical public those methods and results of the conservative treatment of fractures and dislocations in children which stood the test of time. The experiences of both authors as well as successful therapeutic methods of other surgeons are discussed in this book. Differences are stated between fractures in adults and in children which may be attributed to the growth factor of children's bones and their enormous biological driv
