Giant cell arteritis (GCA) is a large-vessel vasculitis predominantly affecting individuals over 50 years, with a peak incidence between 70 and 80 years. Its pathogenesis involves complex immunological cascades triggered by vascular dendritic cell activation, recruitment of CD4⁺ T cells and monocytes, T helper lymphocyte polarization, and vascular remodeling driven by myofibroblasts and fibroblasts. Growing evidence implicates cellular senescence (particularly in vascular smooth muscle cells, fibroblasts, endothelial cells, and immune cells) in the pathophysiology of GCA. Senescent cells (SC)

INTRODUCTION: Giant cell arteritis (GCA) is the most common primary vasculitis and is associated with substantial morbidity. Glucocorticoids play a central role in GCA, resulting in cumulative glucocorticoid exposures that far exceed those used in other rheumatic conditions.
AREAS COVERED: This review summarizes current treatment strategies for GCA, with a particular focus on the Janus kinase (JAK) inhibitor upadacitinib. Evidence from randomized controlled trials, observational studies, and recent conference data is discussed, including efficacy, safety, and comparative considerations with e

OBJECTIVES: Cough is an underrecognized and atypical manifestation of giant cell arteritis (GCA), which may lead to diagnostic delay. Literature on cough in GCA is limited and lacks comprehensive data on clinical relevance. The aim of this study was to assess the frequency of cough at diagnosis in GCA patients and to compare clinical characteristics between those with and without cough.
METHOD: Consecutive patients diagnosed with GCA between 2000 and 2020 and followed for ≥ 12 months at the University Hospitals Leuven (Belgium) were included retrospectively.
RESULTS: We included 398 GCA pati

Pheochromocytomasand paragangliomas (PPGLs) are rare catecholamine-secreting tumors that can affect systemic physiology, including bone metabolism. Although they are typically associated with genetic syndromes such as MEN2 and von Hippel-Lindau disease, PPGLs are not commonly linked to primary bone tumors. However, recent findings suggest a novel cancer syndrome involving both PPGLs and giant cell tumor of bone (GCTB) through H3F3A mutations. We report a rare case of a 53-year-old woman with multicentric GCTB, initially diagnosed at age 24, who subsequently developed bilateral pheochromocytoma

PURPOSE OF REVIEW: Diffuse tenosynovial giant cell tumor (D-TGCT) is a benign neoplasm with locally aggressive potential of the synovium, bursae, and tendon sheaths. This review summarizes the current treatment landscape for D-TGCT, with a focus on systemic therapies.
RECENT FINDINGS: Surgery is the primary treatment option for tenosynovial giant cell tumor (TGCT), but there is a high risk of recurrence and associated morbidity, particularly for patients with advanced D-TGCT. Systemic therapies targeting the colony-stimulating factor 1 receptor (CSF1R) have resulted in positive tumor response

Granulomatosis with polyangiitis (GPA) is a rare auto-immune ANCA-associated small-vessel vasculitis characterized by necrotizing granulomatous inflammation, primarily affecting the sinuses, respiratory tract, and kidneys. Early diagnosis and treatment are crucial for improving patient outcomes and preventing rapidly progressive renal failure. We present a case of a 66-year-old woman presenting to the emergency department (ED) with a two-week history of productive cough, nausea, fevers, and headache. Observations and routine blood tests were unremarkable, so the patient was treated with antibi

Tenosynovial giant cell tumor (TGCT) is a rare inflammatory disorder affecting the joint synovium, bursae, and tendon sheaths that causes non-specific and often insidious joint discomfort. The application of systemic chemotherapy has been limited due to poor and unsustained disease responses. Surgery with or without adjuvant radiation is the primary treatment modality for TGCT. With its locally destructive nature and increased recurrence, multiple surgical interventions become necessary throughout the course of the disease, leading to disfigurement, decreased quality of life, and increased mor

Giant cell hepatitis is a rare infiltrative disease associated with several viruses, drugs, malignancies, and autoimmune conditions. To date, treatment aims at controlling the underlying etiology, and there are limited data on the clinical course and treatment of idiopathic cases. We present a case of idiopathic giant cell hepatitis in an otherwise healthy adult man and review the literature regarding treatment and outcomes in this population.

Giant cell arteritis (GCA), otherwise known as temporal arteritis, is a form of large vessel vasculitis. Patients may present with a variety of symptoms but the most common include headaches, jaw claudication, fatigue, night sweats and visual disturbance. Diagnosis is made using a combination of temporal artery ultrasound, clinical findings and blood tests and the diagnostic gold standard of a temporal artery biopsy and is sometimes needed for histopathological confirmation. Syphilis is a sexually transmitted disease caused by the spirochaete Treponema pallidum, which can infect the central ne

PURPOSE OF REVIEW: Diffuse-type tenosynovial giant cell tumor (dt-TGCT) is a benign clonal neoplastic proliferation arising from the synovium. Patients are often symptomatic, require multiple surgical procedures during their lifetime, and have reduced quality of life (QoL). Surgery is the main treatment with relapse rates ranging from 14 to 55%. The treatment strategy for patients with dt-TGCT is evolving. The purpose of this review is to describe current treatment options, and to highlight recent developments in the knowledge of the molecular pathogenesis of dt-TGCT as well as related therape

Giant cell arteritis (GCA) is a large-vessel granulomatous vasculitis occurring in patients over 50-year-old. Diagnosis can be challenging because there is no specific biological test or other diagnoses to consider. Two main phenotypes of GCA are distinguished and can be associated. First, cranial GCA, whose diagnosis is usually confirmed by the evidence of a non-necrotizing granulomatous panarteritis on temporal artery biopsy. Second, large-vessel GCA, whose related symptoms are less specific (fever, asthenia, and weight loss) and for which other diagnoses must be implemented if there is neit

Annular elastolytic giant cell granuloma is an uncommon granulomatous cutaneous disease that usually affects sun-exposed skin. Non-scarring alopecia is a possible presentation. Although histopathology is mandatory for the diagnosis, dermoscopy may help to narrow down the clinical differential diagnosis. The authors report a case of annular elastolytic giant cell granuloma in the scalp of a female adult patient, showing multiple yellowish/orange follicular dots in a diffuse erythemato-whitish background in the dermoscopy.

BACKGROUND: Giant cell arteritis (GCA) is a vasculitis of the large and medium-sized arteries in the elderly whose ischemic complications adversely affect the eye. The irreversible loss of visual acuity is most often related to acute anterior ischemic optic neuropathy. Very few cases of scleritis have been described in the literature.
CASE PRESENTATION: The patient presented an obvious case of giant cell arteritis, initially revealed by an ophthalmologic involvement in the form of posterior scleritis, an ear, nose, and throat (ENT) involvement with vestibular and neurological involvement with

Giant cell arteritis (GCA) is vasculitis of large-sized vessels that can lead to vision loss. We herein report a rare case of GCA accompanied by ptosis and diplopia as early symptoms, which were caused by third nerve palsy. A 78-year-old man presented with fever, right temporal headache, right eyelid ptosis, and diplopia. GCA was confirmed by a temporal artery biopsy. The symptoms disappeared after a slight delay following the administration of prednisolone. Unlike vision loss, ptosis and diplopia are considered to be reversible and responsive to treatment. GCA should not be ruled out if patie

Giant cell tumor of the calcaneal bone is a very rare entity and generally seen in the 30 to 40 years age group. We report a case of a 17-year-old male with giant cell tumor of the calcaneus, presented with left heel pain without another obvious physical abnormality. Radiographs showed a lobulated, well-defined, lytic lesion of the calcaneus with narrow transitional zone without periosteal reaction, no extraosseal spread, and no lung metastases. Arthroscopic procedure was done directly for both diagnostic and curative procedures. All soft, grayish lesions were completely removed arthroscopical
