PURPOSE: To measure the change of peripapillary retinal vessel density (VD) in eyes with a history of acute primary angle-closure glaucoma (PACG).

A Prospective hospital based study to note the efficacy of Nd Yag Peripheral Iridotomy (PI) in the treatment of acute primary angle closure glaucoma was carried out in Nepal Eye Hospital from Jan 2007 to Jan 2008. AIMS: To compare the long-term efficacy and safety of combined trabeculectomy and cataract extraction versus trabeculectomy alone in primary angle-closure glaucoma (PACG).
METHODS: Hospital files were retrospectively examined for 99 Chinese PACG patients; 75 patients underwent combined surgery and 24 underwent trabeculectomy alone. Success rates were assessed with the Kaplan-Meier survival analysis. Drug-induced secondary angle closure is quite common and in the majority of cases simply stopping the medication leads to rapid reversal of the condition and resolution of glaucoma. We describe here a patient who presented with secondary angle closure glaucoma and myopia following mefenamic acid ingestion which was managed successfully by stopping the medication, symptomatic treatment and reassurance.

In 67 eyes of 46 patients with chronic primary angle closure glaucoma, the effect of peripheral iridectomy in lowering the intraocular pressure was investigated retrospectively. In the majority of cases the pressure was reduced. For the whole population the mean reduction was 3.9 mmHg (SD 6.7 mmHg). The presence or extent of peripheral anterior synechiae showed no significant correlation with the amount of pressure reduction. A peripheral iridectomy is still recommended as first step in the treatment of primary angle closure glaucoma.
