Gynaecomastia is a common situation, with a proliferation of glandular component of male breast secondary to an imbalance in sexual hormones in mammary tissue. A main cause of gynaecomastia is anabolic steroids consumption. Few studies have been performed in patients with high consumption of anabolic steroids, showing a high prevalence (52%). Two surgical techniques available for the correction of severe gynecomastia in the male patient are described in detail. Severe gynecomastia complicates the plan for correction by presenting the same obstacles which are found in mastopexy or breast reduction for the female patient. These include: tissue resection, skin excision, and nipple-areolar complex elevation. The amount of each of these will determine which specific procedure should be used.

Examinations of 39 men between 14 and 18 years of age revealed that half of the cases of pubertal gynaecomastia are false gynaecomasties. In reality, the patients are suffering from fibrous mastosis. The surgical correction of gynecomastia presents particular problems in the body builder who takes medical treatment of anabolic steroids. In 38 corrections, no fat suction was necessary and only an extensive surgical extirpation of the gland was performed. Superior cosmetic results were obtained with this treatment, with the only problem being increased bleeding in these patients who have very large chest-wall muscles and an enlarged circulation.

Based on the good results of another author 10 boys with marked pubertal gynecomastia were treated with the antioestrogen Tamoxifen (Nolvadex) at a dose of 20-40 mg/d orally for 2-12 months. Suction lipectomy is adapted for the correction of gynecomastia. Previous attempts using suction lipectomy for gynecomastia still required the use of sharp dissection for removal of the glandular breast tissue as well as excision of redundant skin. With this new technique, gynecomastia is corrected solely with the use of suction lipectomy. The technique is successful if the gynecomastia is due to excess fat or parenchymal hypertrophy. Gynecomastia may appear at any age of life. We divided the cases we treated into three groups depending on the size of the gynecomastia. The patients were surgically corrected and then re-examined in order to find the most suitable incision. We consider a semicircumareolar incision on the inferior areolar margin the best. This kind of incision can also be used in cases of massive gynecomastia and results in almost invisible scars.
