[HELLP syndrome and manifestation of type I diabetes mellitus in pregnancy].

HELLP syndrome (haemolysis, elevated liver function tests and low platelets) is a multiorganic disease and has been described in combination with pre-eclampsia/eclampsia, but even without symptoms of gestosis. There are signs, that HELLP syndrome represents an "acute status of autoimmunity". Since immune mechanisms play a fundamental role together with other factors in the development of type I diabetes mellitus, a combination of autoimmune reactions could explain the development of type I diabetes during an altered immune status. We report on the course of a pregnancy complicated by HELLP syn…

[The "HELLP" syndrome: a severe sign in toxemia. Three new cases].

Three women with hypertension, admitted for severe epigastric pain in the third trimester of pregnancy had a complete HS. All 3 women recovered spontaneously 3 days after delivery. All clinical and laboratory findings rapidly disappeared. A premature labour was a neonatal death at 26 weeks. The 2 other infants had delivered by caesarean section because the cardiotocogram showed signs of fetal hypoxemia.

[Manifestations of ruptured liver hematoma as a severe complication of HELLP syndrome].

A case of a ruptured haematoma of the liver with intra-abdominal bleeding is reported. The 26-year old primigravida was admitted in the 31st week of gestation because of a HELLP syndrome. She presented additional symptoms, that are considered typical for a HELLP syndrome, complicated by a ruptured haematoma of the liver: severe right upper-quadrant pain, decrease of haemoglobin content, enlarged liver with structural irregularities and free fluid in the abdominal cavity as examined by sonography. During Caesarean section, a subcapsular haematoma of the liver, combined with a 5 cm laceration of…

Posterior reversible encephalopathy syndrome in a patient with HELLP syndrome complicating a triploid pregnancy.

Posterior reversible encephalopathy syndrome (PRES) is a rare-and not always reversible-neurological complication associated with pre-eclampsia. We report a highly unusual case of puerperal PRES occurring in the context of pre-eclampsia arising from a previously undiagnosed triploid pregnancy at 16 weeks gestation.

The HELLP syndrome in the antiphospholipid syndrome: retrospective study of 16 cases in 15 women.

OBJECTIVE: To study the characteristics of the haemolysis, elevated liver enzymes, low platelets (HELLP) syndrome in the antiphospholipid syndrome (APS) and its influence on the subsequent pregnancies. METHODS: This was a retrospective analysis of 16 episodes of HELLP complicating APS in 15 women. RESULTS: HELLP was complete in 10 cases and partial in six. It occurred during the second trimester in seven cases (the earliest at 18 weeks' gestation), the third trimester in seven cases, and the day following delivery in two cases. Pre-eclampsia was present in six cases and eclampsia in five. Ou

Comparisons of maternal and perinatal outcomes in Taiwanese women with complete and partial HELLP syndrome and women with severe pre-eclampsia without HELLP.

AIM: To analyze the variations between maternal complications and perinatal outcome among women with complete hemolysis, elevated liver enzyme levels, and low platelet count (HELLP) syndrome, partial HELLP syndrome, and women with severe pre-eclampsia and normal laboratory tests. We also examine the effect of corticosteroid therapy for treatment of HELLP. METHODS: In this retrospective study, six patients with complete HELLP syndrome and 46 with partial HELLP syndrome, were compared and contrasted with 212 patients with severe pre-eclampsia but without HELLP syndrome. RESULTS: In Protocol 1,…

First-Trimester Placental Growth Factor and HELLP Syndrome: The PREDICTION Study.

OBJECTIVES: Hemolysis, elevated liver enzymes, and low platelets (HELLP) syndrome is a complication of preeclampsia. We aimed to estimate the association between first-trimester placental growth factor (PlGF) and other preeclampsia risk factors with HELLP syndrome. METHODS: We performed a secondary analysis of a prospective cohort of nulliparous women recruited at 11 weeks 0 days - 14 weeks 0 days gestation and stratified according to pregnancy outcome: HELLP syndrome, preeclampsia without HELLP syndrome, or neither (controls). We compared the 3 groups for first-trimester maternal age, BMI, m…

Severe HELLP syndrome masquerading as thrombocytopenic thrombotic purpura: a case report.

BACKGROUND: Thrombotic microangiopathies (TMAs) occurring in the postpartum period may be difficult to manage. They present as the combination of mechanical hemolytic anemia and consumption thrombocytopenia due to endothelial dysfunction. The cause of this endothelial aggression can be multiple: thrombocytopenic thrombotic purpura (TTP), HELLP syndrome, antiphospholipid syndrome, atypical hemolytic and uremic syndrome or acute fatty liver of pregnancy. TTP results from a severe deficiency of ADAMTS13, which is a protease cleaving specifically von Willebrand factor chiefly produced by liver cel…

Aspartate-aminotransferase to platelet ratio index score for predicting HELLP syndrome.

INTRODUCTION: HELLP (hemolysis, elevated liver enzyme levels, low platelet counts)-syndrome is a rare but dramatic pregnancy-related illness. The difficult part of this syndrome is the lack of standardised diagnostic criterias and tests to be used to predict it. The aim of this study is determining the role of APRI score in the diagnosis of HELLP syndrome. MATERIAL AND METHODS: In this cross sectional, retrospective study, patients with HELLP syndrome as case group and age-matched healthy pregnants at the similar pregnancy trimester as control group were included between January 12,017 and Ma…

Plasma from patients with HELLP syndrome increases blood-brain barrier permeability.

Circulating inflammatory factors and endothelial dysfunction have been proposed to contribute to the pathophysiology of hemolysis, elevated liver enzymes, and low platelet count (HELLP) syndrome. To date, the occurrence of neurological complications in these women has been reported, but few studies have examined whether impairment in blood-brain barrier (BBB) permeability or cerebrovascular reactivity is present in women having HELLP syndrome. We hypothesized that plasma from women with HELLP syndrome causes increased BBB permeability and cerebrovascular dysfunction. Posterior cerebral arterie…

PP085. Hypertensive complication in pregnancy - HELLP syndrome - One year study (2012).

INTRODUCTION: HELLP syndrome is characterized by hemolysis with a microangiopathic blood smear, elevated liver enzymes, and low platelet count. It develops in 10-20% of women with severe preeclampsia/eclampsia. The syndrome is associated with maternal morbidities - DIC, renal failure, pulmonary edema and hepatic hematoma/rupture. Some need transfusions and others require laparotomies for intraabdominal bleeding. OBJECTIVES: Study the incidence and related risk factors of HELLP syndrome in Pregnancy, as well as the maternal/fetal outcome. METHODS: A retrospective study of admissions for HELLP…

Preeclampsia and HELLP syndrome: impaired mitochondrial function in umbilical endothelial cells.

Preeclampsia (PE) and hemolysis, elevated liver enzymes, and low platelet count (HELLP) syndrome have been linked to congenital fetal disorders of mitochondrial fatty acid oxidation (FAO). Different incidences may argue for the association of noncongenital alterations of mitochondrial energy metabolism with PE/HELLP syndrome. We studied human umbilical vein endothelial cells [HUVEC] as selected part of the feto-placental unit from uncomplicated (n = 46) and diseased (n = 27; 17 PE and 10 HELLP) pregnancies by measuring the overall FAO, carnitine palmitoyltransferase 2 (CPT2), respiratory chain…

Transient postpartum diabetes insipidus in twin pregnancy associated with HELLP syndrome.

Diabetes insipidus during pregnancy is an uncommon medical problem, and its cause is not entirely clear. We present a woman with twin pregnancy associated with HELLP syndrome, who developed diabetes insipidus during postpartum period. A hypertonic saline infusion study with measurement of plasma arginine vasopressin concentrations confirmed the diagnosis. She had mild response to 1-desamino-8-d-arginine-vasopressin (dDAVP) during the immediate postpartum period. On the 3rd postpartum day two doses of 100 microliters of dDAVP were administered, and her urinary volume gradually decreased. We cou…
